Recent developments include the increased use of electrophysiology, the resurgence of primary percutaneous intervention in acute coronary syndromes, the use of percutaneous devices in patients previously considered inoperable, and the withdrawal of aprotinin. Against this landscape, this invaluable resource has been fully updated. New chapters are dedicated to right heart valves, pulmonary vascular disease, cardiac tumours and cardiac trauma. All other chapters have been updated according to the latest international guidelines. Written and edited by an international author team with a wealth of expertise in all aspects of the perioperative care of cardiac patients, topics are presented in an easy to digest and a readily accessible manner. Core Topics in Cardiac Anaesthesia, Second Edition is essential reading for residents and fellows in anaesthesia and cardiac surgery and clinical perfusionists.

Book excerpt: Ischemic Preconditioning: The Concept of Endogenous Cardioprotection consolidates, in one volume, both current knowledge and the most recent advances in ischemic preconditioning. The editors have invited investigators at the forefront of ongoing research to provide their scholarly and candid comments concerning each of these issues. This volume includes a comprehensive review of infarct size reduction with ischemic preconditioning, and the most recent data on the effects of preconditioning on ischemia and reperfusion-induced arrhythmias, myocardial metabolism, contractile function, and the coronary vasculature. The role of altered energy metabolism, stress-induced proteins, ATP-sensitive potassium channels, and adenosine -- the major hypotheses that have been proposed to explain the cardioprotective effects of ischemic preconditioning -- are critically reviewed by investigators who have been instrumental in developing these concepts. In addition, the editors raise the intriguing possibility that ischemic preconditioning may be more than simply a laboratory curiosity. Using a multidisciplinary approach, this volume challenges the readers to contribute their own expertise to address the unanswered questions concerning this endogenous, cardioprotective phenomenon.

Book excerpt: Stress reaction is likely to play a crucial role in a variety of degenerative diseases including cancer and cardiovascular diseases. The process of stress adaptation may appear to be simple, but in reality this is a very complex process and we are only beginning to understand the mechanism of adaptation. In January, 1998, scientists from around the world assembled to discuss the potential applicability of the concept of stress adaptation in the clinical arena. This volume contains original research papers presented on this subject during the conference Stress Adaptation, Prophylaxis and Treatment held in Calcutta, India, and serves as an up-to-date source of information for scientists, as well as clinicians interested in applying the concept of stress adaptation to the cure of diseases.

Available in PDF, EPUB and Kindle. Book excerpt: Ischaemia-Reperfusion Injury is concerned with the consequences of interrupting and restoring blood flow to tissues. Many common clinical conditions are caused by interruption of blood flow to tissues (eg, ischaemic heart disease, peripheral vascular disease, stroke); blood flow is also interrupted deliberately in many surgical procedures (eg, cardiac surgery, arterial surgery, transplant surgery, limb surgery with tourniquet). In treating such conditions or after performing such operations the aim of the clinician is to restore the blood supply to the ischaemic tissue. Paradoxically, restoration of blood flow to ischaemic tissues can lead to further tissue damage with the potential for severe local and systemic injury. This book focuses on the clinical, pathological and biochemical processes involved in ischaemia-reperfusion injury and gives an overview of the strategies that may be adopted to mitigate or prevent such injury. This book will be of interest to both clinicians and scientists who have to deal with or have interest in this difficult but important problem.

Available in PDF, EPUB and Kindle. Book excerpt: Detailed analyses of regional myocardial blood flow, function, metabolism and morphology in ischemic and reperfused myocardium have led to the identifi cation of important phenomena, i. e. , myocardial hibernation, myocardial stun ning and ischemic preconditioning. Both the hibernating and the stunned myocardium characterize viable though dysfunctional as distinguished from necrotic tissue. With the advent of reperfusion procedures, the distinction between reversibly injured, hypofunctional myocardium from irreversibly injured, hypofunctional myocardium is of utmost clinical importance. The pathophysiological distinction of hibernating and stunned myocardium is con troversial, but reperfusion is mandatory anyway. Ischemic preconditioning is the most powerful maneuver known so far to delay infarct development. Its clinical significance has been suggested from retrospective analyses of data from patients suffering a myocardial infarction as well as from controlled PTCA studies. Whether or not preconditioning can be achieved pharmacologically in the clinical setting remains to be established. The mechanisms and signal cascade underlying myocardial hibernation, myocardial stunning and ischemic preconditioning are not clear in detail. Over the last year, focussed issues on myocardial hibernation, myocardial stunning and ischemic preconditioning were published in Basic Research in Car diology; they have received great interest and a good response. Therefore, these focussed issues are now combined and published as a separate monograph. We express our gratitude once more to all our colleagues who contributed to this monograph, to Ms. Ibkendanz of Steinkopff, and to Ms. Philipp and Mr. Heinrichs from Bayer AG Germany, who supported this additional publication.

Available in PDF, EPUB and Kindle. Book excerpt: This book bridges the gap between fundamental and translational research in the area of heart disease. It describes a multidisciplinary approach, and demonstrates biochemical mechanisms associated with dysregulation of redox signaling, which leads heart disease. Presenting recent studies on improved forms of ROS scavenging enzymes; specific inhibitors for different ROS generating enzymes; and oxidant induced signaling pathways and their antagonists that allow subtle modulation of redox signaling, it also discusses the spatial and temporal aspects of oxidative stress in the cardiovascular system, which are of vital importance in developing better strategies for treating heart disease. Each chapter offers researchers valuable insights into identifying targets for drug development for different types of heart disease.
