Book excerpt: Introduction : les effets indésirables médicamenteux sont sous-notifiés aux centres de pharmacovigilance particulièrement dans le cadre des chimiothérapies. Cette thèse s'inscrit dans le volet médico-sociologique de l'étude Sircade et vise à explorer les pratiques de prescription et de gestion des effets indésirables des généralistes à travers leur rapport aux patients et aux spécialistes et à mieux comprendre les réponses émises dans le cadre d'un questionnaire quantitatif de l'ORS. Elle contribue à émettre des hypothèses pour expliquer le faible recours à la pharmacovigilance. Méthode : une enquête qualitative par entretiens collectifs auprès de médecins généralistes a été réalisées : 2 focus group en zone urbaine et en zone semi-rurale ont été prévus. Des vignettes cliniques ont été présentées sur le thème de la prescription d'un médicament à effet indésirable fréquent, du symptôme pouvant être en rapport avec un effet indésirable rare, la survenue d'un effet indésirable sous chimiothérapie ou sur un médicament prescrit par un spécialiste. Une analyse thématique puis conceptuelle a ensuite été réalisée à partir du codage des verbatims. Résultats : l'analyse thématique a mis en évidence des pratiques variées autour de la prescription. Trois profils ont pu être identifiés : le médecin aux pratiques centrées sur les savoirs, le médecin aux pratiques centrées sur les patients et le médecin déclareur intégrant la notion de santé publique. Les freins à la déclaration sont le manque de certitude quant à l'effet indésirable, le manque de connaissance de la pharmacovigilance et le manque d'intérêt des généralistes pour la santé publique. Les généralistes ne se sentent pas impliqués dans la gestion des chimiothérapies et de leurs effets indésirables. Discussion : la sensibilité des médecins aux EI médicamenteux est entrée dans l'identité professionnelle. Pour autant il faudra encore développer la culture de la pharmacovigilance afin d'augmenter la visibilité des effets indésirables.

Based on anthropological fieldwork carried out in a group of neighbourhoods in the periphery of Niamey, the study highlights the political dimensions of public service provision in a local arena where international development interventions and national plans meet local realities and where a wide range of actors and institutions, discourses, meanings, and practices are mobilized in the offering of and the regulation of access to public services. It focuses on the political, economic, and socio-cultural aspects of public service provision, too often hidden behind contemporary buzzwords of development such as community participation and decentralization that dominate global debates about education and healthcare in developing countries. The study brings forth the strategies of urban residents in dealing with daily challenges in the consolidation of service provision and in education and health-seeking trajectories. It shows that access to a satisfactory treatment of illness or a successful school career is premised on the ability to navigate on the medical and education markets, which are made up of a plurality of providers and of official and unofficial costs and transactions. Further, these public services engage different actors such as community committees, traditional chiefs, local associations, the municipality and elected municipal councillors, emergent leaders, NGOs, and international development aid. The study demonstrates that despite the uncertainty of state support in health and education provision and a widespread dissatisfaction with these public services, the image of the state as service provider is reproduced on a day by day basis through local efforts at securing public services.

These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.
