Book excerpt: Background and Goal of Study:The Face, Leg, Activity, Cry, Consolability scale (FLACC) and the Colored Analogue Scale(CAS) are both recommended tools in pediatric postoperative pain assessment. Nurses oftenreport lower pain intensity scores compared to children and parents (1). This might impairgood pain management. Aim: to further investigate differences in-hospital pain assessmentbetween nurses, children and parents after adenotonsillectomy.Materials and Methods:After IRB approval, a prospective cohort study was carried out investigating intra-hospital painafter adenotonsillectomy in daytime surgery. Inclusion: eligible were children between 6 and12 years old, ASA 1&2. Exclusion: known mental retardation.Instruments: a. the FLACC-scale scored by 2 independently nurses (1 permanent researchnurse) at 2h and 4h postoperative; b. a CAS consists of two extremes, u2018no painu2019 (white color)and u2018most painu2019 (red color), on which both the child and parent slide a marker on a vertical lineto indicate the childu2019s pain at 2h and 4h postoperative (0 indicating no pain and 10 very muchpain). Statistics: 1. agreement between nurses FLACC scores using intraclass correlationcoefficient (ICC); 2. ICC between child/parental CAS scores; 3. discriminant validity:McNemar test after dichotomizing pain scores into 2 groups (non to mild pain vs. moderate tosevere pain) using cut-off values of respectively u22653 on the FLACC and u22653 on thechild/parental CAS ratings. Results and Discussion:This study included 57 children (64 % girls) with mean age of 108 (months), SDu00b125.7. ICCbetween FLACC scores at 2h were: r =.72; P=.000 and at 4h: r = .33; P =.07). ICC betweenchild/parental CAS at 2h: r=.72; P=.000 and at 4h r=.63; P=.001. After dichotomizing scoresat 2h [FLACC2h: n=2 had moderate to severe pain vs. CASchild2h: n=32; P

Book excerpt: In light of the more recent attitude that even premature infants have the capacity to experience pain, 19 papers from the first biennial International Forum on Pediatric Pain (1996: Nova Scotia) treat psychometric, neurophysiological, neurobiological, behavioral, and clinical issues in the measurement, management, and research on pain in a population for whom the standard self-report is untenable--except through specially- designed measures (e.g. the Poker Chip Tool and Pain Experience History-Child form). Available in PDF, EPUB and Kindle. Book excerpt: Advances over the past two decades have enabled physicians to revolutionize the manner in which they can assess and manage children’s pain. Thirty years ago it was thought that young children did not experience pain and therefore it was not necessary to treat it. Today professionals from a variety of disciplines have contributed data that have revolutionized medical perspectives. Technological advances now enable doctors to treat acute pain in fetuses, premature neonates, infants, toddlers, children, and adolescents with increasing precision and efficacy. Research highlighting the context of chronic pain has moved them away from a mind-body dichotomy and toward an integrated, holistic perspective that leads to substantial improvement in children’s adaptive functioning as well as subjective discomfort. This book covers these topics and is intended for anyone who provides medical care to children. Each chapter provides an overview of the problem, followed by a "hands on" description of relevant assessment and intervention strategies. The role of the primary care practitioner is highlighted, both as a front-line resource as well as a consumer of specialized pediatric pain treatment services. Each chapter ends with a summary and specific bullet points highlighting the most central elements, making for quick and easy reference.

Now in its second edition, it provides a thorough overview of the science and practice of anesthesia. Part I describes the evaluation of the patient, the different approaches to anesthesia, and the post-operative care of the patient in pain. Part II introduces the essentials of physiology and pharmacology and their role in understanding the principles of anesthesia. The final part presents a step-by-step description of 14 clinical cases. These clinical vignettes give a very real introduction to the practicalities of anesthesia and will give the non-anesthetist physician an idea of how to prepare a patient for a surgical procedure. All chapters have been expanded and updated and an entirely new chapter on safety in healthcare has been added. This is the perfect introductory text for medical students, junior doctors and all operating theatre and critical care staff.

Available in PDF, EPUB and Kindle. Book excerpt: This text provides a comprehensive review and expertise on various interventional cancer pain procedures. The first part of the text addresses the lack of consistency seen in the literature regarding interventional treatment options for specific cancer pain syndromes. Initially, it discusses primary cancer and treatment-related cancer pain syndromes that physicians may encounter when managing cancer patients. The implementation of paradigms that can be used in treating specific groups of cancer such as breast cancer, follows. The remainder of the text delves into a more common approach to addressing interventional cancer pain medicine. After discussing interventional options that are commonly employed by physicians, the text investigates how surgeons may address some of the more severe pain syndromes, and covers the most important interventional available for our patients, intrathecal drug delivery. Chapters also cover radiologic options in targeted neurolysis and ablative techniques, specifically for bone metastasis, rehabilitation to address patients’ quality of life and function, and integrative and psychological therapies. Essentials of Interventional Cancer Pain Management globally assesses and addresses patients’ needs throughout the cancer journey. Written by experts in the field, and packed with copious tables, figures, and flow charts, this book is a must-have for pain physicians, residents, and fellows.
