Il existe un retard à l'instauration de l'insulinothérapie, faisant pourtant partie de la stratégie thérapeutique. Comprendre le vécu des patients sous insulinothérapie a pour but d'améliorer nos pratiques lors de l'instauration et le suivi de l'insuline. Méthode: Une étude qualitative par analyse thématique de contenu a été réalisée grâce à des entretiens semi-dirigés. L'échantillon de patients diabétiques de type 2, suivi en médecine générale, est raisonné diversifié. Les entretiens ont été menés jusqu'à saturation des données, avec triangulation des chercheurs. Résultats: Dix-neuf entretiens ont été réalisés. Les réactions à l'annonce de l'instauration d'une insulinothérapie n'étaient pas toutes négatives. Pour certains, elle était accueillie comme une alternative séduisante aux antidiabétiques oraux. La facilité d'utilisation du système d'injection, l'absence de douleur et la gestion facile du traitement étaient des atouts majeurs. L'insulinothérapie était jugée comme efficace et plus physiologique que les autres médicaments, avec également moins d'effets secondaires. Néanmoins, elle était parfois vécue comme un handicap, une rupture avec une vie «antérieure». C'était ainsi la prise de conscience de leur statut de patient diabétique. La relation des patients diabétiques avec leurs proches était souvent modifiée depuis le diagnostic du diabète, mais pas forcément depuis l'instauration de l'insuline. Ils étaient en effet impliqués dans le suivi des règles hygiéno-diététiques. Par contre, un manque d'informations des patients était retrouvé, notamment lors de l'instauration de l'insuline en ambulatoire. L'espoir en un avenir meilleur, était également remarquable. Conclusion : L'amélioration de l'encadrement des patients, par le développement de l'éducation thérapeutique personnalisée, devrait permettre à moyen terme, de pallier au manque d'information des patients. Il est fondamental d'inclure les aidants dans cette démarche éducative. En ayant une meilleure connaissance de leur maladie et de leur traitement, le vécu de l'insulinothérapie par nos patients ne pourra que s'améliorer.

Nous nous sommes intéressés aux ressentis des patients concernant leurs attentes et leurs besoins après le passage à l'insuline. Objectif : Identifier les attentes et les besoins des patients diabétiques de type 2 après instauration d'une insulinothérapie à l'hôpital. L'objectif secondaire est de mettre en évidence les croyances et représentations des patients. Matériel et Méthodes : Une méthodologie qualitative par entretiens semi-dirigés a été choisie. L'analyse thématique, manuelle et croisée des entretiens a permis d'identifier les attentes, les besoins, les croyances et les représentations des patients. Résultats : Entre janvier et septembre 2013, quatorze patients des Alpes-Maritimes ont été interrogés. En ce qui concerne l'insulinothérapie, les patients souhaitaient stabiliser ou améliorer leur diabète et améliorer leur qualité de vie. Certains n'attendaient rien ce qui traduit une absence de compréhension de la maladie et du traitement. Nous avons constaté que les attentes étaient principalement liées aux acteurs qui entourent les patients : l'hôpital, le médecin généraliste, la famille, l'entourage, les autres patients diabétiques. Les besoins, peu exprimés dans notre étude, étaient surtout d'ordre organisationnels. Les croyances étaient surtout liées à l'espoir de guérison avec l'insuline, tandis que les représentations étaient : la stigmatisation sociale, la culpabilité, la gravité de la maladie. Le médecin généraliste est amené à aider le patient à dépasser ces réticences vis à vis de l'insuline et de la maladie. Conclusion : Développer l'éducation thérapeutique, l'entretien motivationnel par des auxiliaires de santé au sein de maisons de santé, mettre en place des groupes de discussion animés par le médecin généraliste, créer des ateliers culinaires et sportifs ambulatoires pour les patients diabétiques, pourraient être des pistes susceptibles de faciliter l'acceptation et la compréhension de la maladie et du traitement.

Book excerpt: BUTS DE L'ETUDE : Etat des lieux des connaissances des patients diabétiques de type 2 de leur maladie et de l'application des règles hygiéno-diététiques dans leur quotidien, et place du médecin généraliste dans le suivi de leur diabète. INTRODUCTION : Le diabète de type 2 est une maladie chronique dont le traitement repose autant sur l'application de règles hygiéno-diététiques (alimentation, activité physique, régulation du poids) que les médicaments. Il semble qu'en France la part médicamenteuse soit plus importante que chez nos voisins européens. MATERIELS ET METHODES : Il s'agit d'une étude quantitative prospective transversale. 200 questionnaires ont déposés à la consultation diabète de l'hôpital de Fréjus et dans 10 cabinets de médecins généralistes des villes de Fréjus-Saint Raphaël (Var, 83). RESULTATS : Le taux de réponse a été de 21% (42/200). 42 patients ont été inclus dans l'étude. Il s'agissait majoritairement de femmes (54,1%), d'âge moyen de 64 ans et d'IMC moyen de 30,19 kg/m2. La majorité de ces patients a déclaré avoir reçu une information à propos de la prise en charge non médicamenteuse du diabète. Le généraliste est apparu comme la première source d'information (80,5%). L'information était majoritairement délivrée par oral (72,7 %) et jugée pertinente par 82,7 % des patients. 42,9 % des patients avaient un bon niveau de connaissances de leur pathologie, et 54 % un très bon niveau d'application des recommandations hygiéno-diététiques. Les deux principaux obstacles freinant l'application de ces mesures étaient la lassitude et la prise des repas à l'extérieur. Le généraliste était jugé compétent pour le suivi de la maladie diabétique par 83,3 %, et 68,3% ont souhaité bénéficier d'aide(s) supplémentaire(s) pour la gestion de leur maladie au quotidien, dont le principal était la consultation diététique. CONCLUSION : La prise en charge non médicamenteuse du patient diabétique de type 2 à Fréjus-Saint Raphaël est globalement satisfaisante, d'après les patients. Les recommandations sont plutôt bien appliquées dans la vie quotidienne. Le médecin traitant tient une place prédominante. Les mesures visant à améliorer l'éducation thérapeutique des patients diabétiques de type 2 doivent être poursuivies et renforcées.

Formal patient diabetes education to promote self-management (PDE) at different levels of complexity and comprehensiveness has emerged from instructions by physicians, nurses, and dieticians in physicianþs offices and hospitals. Since 1990 there has been a shift from traditional approaches of PDE to focus on patient-centered perspectives, self-efficacy, self-management and empowerment issuses. Quantitatives research on the use of formal PDE as a therapeutic tool promote self-management in adults with type 2 diabetes has produced mixed results in terms of improved metabolic control and reduced risks for cardiovascular disease. The long-term diabetes control outcomes of formal PDE when used to promote self-management in adults with type 2 diabetes have yet to be established. Further investigations are needed to determine which types of programs or what components are most effective in improving the ability of adults with type 2 diabetes to self-manage their disease; or which category of patients might benefit most. Findnings from qualitative research studies helped to better understand the context in which formal PDE should be applied to be successful in promoting self-managemenet behaviors in adults with type 2 diabetes.
