Book excerpt: Objectif : déterminer less principales difficultés auxquelles sont confrontés, selon eux, les héroïnomanes en demande d'une prise en charge de leur dépendance par leur médecin généraliste. Il s'agit d'une enquête épidémiologique transversale réalisée à l'aide d'un questionnaire quantitatif et anonyme délivré aux patients héroïnomanes bénéficiant d'un médicament de substitution aux opiacés préscrit par les centres de soins spécialisés pour toxicomanes de l'agglomération Clermontoise. Nous avons ainsi recueilli 94 questionnaires remplis par des usagers reçus en consultation d'addictologie entre mars et avril 2014. Près de 70% des héroïnomanes interrogés déclarent avoir un médecin généraliste référent et plus de la moitié d'entre eux accepterait que ce dernier s'occupe de son traitement de substitution. Leur dépendance aux opiacés n'est cependant pas prise en charge par leur médecin généraliste pour des raisons qui, selon les patients, sont avant tout liées au médecin lui-même. Les principaux motifs évoqués sont, de leur point de vue, le manque de connaissances et de compétences de leur médecin généraliste dans le domaine de la toxicomanie et les représentations qu'ils en ont. De cette enquête émane la nécessité d'améliorer la prise en charge des patients héroïnomanes en demande de substitution par les médecins généralistes en agissant sur deux paramètres majeurs : faciliter l'accès de ces derniers à la formation en toxicomanie et lutter contre les représentations négatives qui sont associées à cette pathologie.
