Available in PDF, EPUB and Kindle. Book excerpt: Peripheral nerve injuries are a high-incidence clinical problem that greatly affects patients' quality of life. Despite continuous refinement of microsurgery techniques, peripheral nerve repair still stands as one of the most challenging tasks in neurosurgery, as functional neuromuscular recovery is rarely satisfactory in these patients. Therefore, the improvement of surgical techniques and the clinical application of innovative therapies have been intensively studied worldwide. Direct nerve repair with epineural end-to-end sutures is still the gold standard treatment for severe neurotmesis injuries but only in cases where well-vascularized tension-free coaptation can be achieved. When peripheral nerve injury originates a significant gap between the nerve stumps, nerve grafts are required, with several associated disadvantages. Therefore, the development of scaffolds by tissue engineering can provide efficient treatment alternatives to stimulate optimum clinical outcome. Nerve conduit tailoring involves reaching ideal wall pores, using electrospinning techniques in their fabrication, surface coating with extracellular matrix materials, and adding of growth factors or cell-based therapies, among other possibilities. Also, intraluminal cues are employed such as the filling with hydrogels, inner surface modification, topographical design, and the introduction of neurotrophic factors, antibiotics, anti-inflammatories and other pharmacological agents. A comprehensive state of the art of surgical techniques, tissue-engineered nerve graft scaffolds, and their application in nerve regeneration, the advances in peripheral nerve repair and future perspectives will be discussed, including surgeons' and researchers' own large experience in this field of knowledge.

Book excerpt: Human adult peripheral nerve injuries are a high incidence clinical problem that greatly affects patients' quality of life. Although peripheral nervous system has intrinsic regenerative capacity, this occurs in an incomplete or poorly functional manner. When a nerve fiber loses its continuity with consequent damage of the basal lamina tubes, axon spontaneous regeneration is disorganized and mismatched. These phenomena translate in an inadequate nerve functional recovery and consequent musculoskeletal incapacity. Nerve grafts still remain the gold standard in peripheral injuries treatment. However, this approach contains its disadvantages such as the necessity of primary surgery to harvest the autografts, loss of a functional nerve, donor site morbidity and longer surgery procedures. Therefore, biomaterials and tissue engineering can provide efficient resources and alternatives to nerve injury repair not only by the development of biocompatible structures but also, introducing neurotrophic factors and cellular systems to stimulate optimum clinical outcome. In this chapter, a comprehensive state-of-the art picture of tissue-engineered nerve grafts scaffolds, their application in nerve regeneration along with latest advances in peripheral nerve repair and future perspectives will be discussed, including our own large experience in this field of knowledge.

Book excerpt: This book is the result of the 20th International Summer School in Brain Research, organized in August 1997 in Amsterdam, by the Netherlands Institute for Brain Research at the Royal Netherlands Academy of Sciences. It is the first book that provides a complete overview of the field of neurodegeneration and regeneration including spinal cord injury, neurodegenerative diseases and therapy. Divided into five sections, the first two sections give an overview of fundamental research on nerve cell death, neuronal survival, neurite outgrowth and guidance. Extensive attention is given to the role of neurotrophins, their receptor tyrosine kinases and cell-adhesion molecules in development and regeneration of the nervous system. The third section of the book is devoted to research involving human neurodegenerative diseases and emerging treatment strategies. Section four focusses on recent advances in the understanding of pathophysiological mechanisms underlying neurodegenerative diseases, including Alzheimer's, Parkinson's and Huntington's diseases along with prion diseases. Novel insights into the neuropathological hallmarks of these diseases, as well as into transgenic animal models, the involvement of environmental factors, and genomic and mRNA changes that can cause neurodegeneration. The final section of this volume reveals recent developments in the use of cell and gene therapy to treat neurodegenerative disease and lesion-related deficits. Implantation of genetically modified cells, direct gene transfer with viral vectors and the first clinical trials with encapsulated genetically modified cells in patients suffering from amyotrophic lateral scelerosis are examples of new therapeutic strategies treating neurodegenerative diseases. The book is of particular interest to departments of neuroscience, neurological clinics and departments, the pharmalogical industry and medical libraries.

Book excerpt: This reference work presents the origins of cells for tissue engineering and regeneration, including primary cells, tissue-specific stem cells, pluripotent stem cells and trans-differentiated or reprogrammed cells. There is particular emphasis on current understanding of tissue regeneration based on embryology and evolution studies, including mechanisms of amphibian regeneration. The book covers the use of autologous versus allogeneic cell sources, as well as various procedures used for cell isolation and cell pre-conditioning , such as cell sorting, biochemical and biophysical pre-conditioning, transfection and aggregation. It also presents cell modulation using growth factors, molecular factors, epigenetic approaches, changes in biophysical environment, cellular co-culture and other elements of the cellular microenvironment. The pathways of cell delivery are discussed with respect to specific clinical situations, including delivery of ex vivo manipulated cells via local and systemic routes, as well as activation and migration of endogenous reservoirs of reparative cells. The volume concludes with an in-depth discussion of the tracking of cells in vivo and their various regenerative activities inside the body, including differentiation, new tissue formation and actions on other cells by direct cell-to-cell communication and by secretion of biomolecules.
