Aucun biomarqueur de stress ne faisant consensus, une analyse objective de certains d'entre eux pourrait faire émerger des indicateurs potentiels. Il s'agit d'une étude de faisabilité comparant la variation des principales hormones du stress et de la variabilité sinusale chez les médecins urgentistes du CHU de Clermont-Ferrand pour 2 systèmes de garde : 14 heures versus 24 heures. Il s'agit également d'une étude de récupération par un suivi post-garde de 48 heures. Le comité de protection des personnes a émis un avis favorable pour cette étude observationnelle sur 19 médecins urgentistes volontaires. Sont comparés les taux des hormones du stress et les marqueurs de variabilité sinusale les 3 jours consécutifs d'une G24/G14, et lors d'une journée témoin au retour de vacances (JARV) : J0 est le jour de la G14/G24, J1 le repos de sécurité, J2 une journée administrative (JA) (JAG24/JAG14). Les paramètres suivis sont : urinaires (cortisol, catécholamines, IL8), salivaires (cortisol, DHEA-S, leptine, IgAs, lysozyme), variabilité sinusale (holter), charge de travail (nombre total d'entrées, de lits disponibles et de déchocs), stress et fatigue physique/mentale (EVA), psychologiques (Hamilton, Maslach Burn-out Inventory, HAD, Karasek). Les prélèvements salivaires sont recueillis toutes les 12 heures sur 3 jours. Les prélèvements urinaires et la variabilité sinusale sont analysés en G24/G14 et en JAG24/JAG14. Outre le suivi de tous ces paramètres, la journée témoin permet la passation des questionnaires psychologiques. La variabilité sinusale est exploitée en analyse temporelle avec l'intervalle de temps moyen entre des complexes QRS normaux (NN), le pourcentage de NN variant de plus de 50 ms (pNN50) et la racine carrée de la différence des intervalles NN successifs élevée au carré (r-MSSD pour the square root of the mean of the sum of the squares of differences between adjacent NN intervals). pNN50 et r-MSSD élevés traduisent une forte activité parasympathique. L'analyse spectrale des espaces NN distingue un intervalle à Haute Fréquence (HF pour High Frequency) lié à l'activité parasympathique et un intervalle à Basse Fréquence (LF pour Low Frequency) lié préférentiellement à l'activité sympathique. Un logarithme LF/HF faible traduit un équilibre en faveur du parasympathique. 17/19 médecins urgentistes ont réalisé le protocole en entier. Fatigues physique et mentale sont supérieures lors d'une G24 versus G14, avec des effets prolongés dans le temps (résultats toujours significatifs au 3e jour). L'analyse de la variabilité sinusale retrouve une activité sympathique supérieure lors d'une G24 (pNN50 G24 G14, p = 0,0318 ; r-MSSD G24

The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."
