One indication of the dimensions of this problem is that in the United States lung cancer has become the leading cause of cancer death in women as well as men. In 1912 there was a "nearly complete consensus of opinion that primary malignant neoplasms of the lung (were one) of the rarest forms of disease," according to Adler. By 1937, however, it had become clear that the incidence of lung cancer was increasing significantly; this increase has been progressive ever since. It is now well known that some lung cancers give rise to a variety of hormones which, at times, produce clinical manifestations. The association of hormone production with a "nonendocrine" tumor raises many questions, the answers to which may shed some light on the etiology of this prevalent form of cancer. This fascinating prob lem has stimulated a wide variety of studies in both the clinical and the basic sciences. A number of the more recent studies in this field were discussed at the International Symposium on Peptide Hor mones and Lung Cancer held in Marburg, West Germany, on June 18-20, 1984. This volume contains the papers that were pre sented on this occasion.

Many of the discoveries from the biological studies have impacted on the pathologist in the form of disturbing evidence for a single stem cell origin for all the cell types of lung cancer and in the shape of new facilitation in diagnosis by appli cation of immunoperoxidase techniques. Monoclonal antibodies raised against oncogene products, growth factor receptor sites, "bystander" cell membrane proteins can all be applied to cytology specimens and frozen or paraffin fixed tissue sections to aid diagnosis and some can be used in sequential serum assay to monitor therapy and predict prognosis. Adding to these extraordinary tools, the sophistication of electron micro scopy and immuno-electrcn microscopy, new techniques for preparation of tissue and novel methods for studying vital cells in "kinesis", you sense the flavour of the future of lung cancer which is captured in this book.

Book excerpt: In the work described in this thesis, peptidylglycine a-amidating monooxygenase (PAM) was extracted from human H889 and DMS53 small cell lung cancer (SCLC) cells, PC-3 prostate cancer cells and SW1783 glioblastoma cells. Optimization of PAM assay conditions was carried out to maximize PAM activity. The extracted enzymes and optimized assay conditions provided a platform to assess substrate and inhibitor kinetic parameters and to compare the enzyme specificities of PAM from different human cancer sources, as well as differences between human PAM and commercial frog PAM. Potent and selective PAM inhibitors were then developed on the basis of structure-activity-relationships as well as the recognition by PAM of the peptide chain of natural substrates. Furthermore, a direct, rapid, efficient and sensitive HPLC method was developed in order to carry out whole cell inhibition studies and to directly monitor the inhibitory effects on natural hormone and prohormone levels. This method includes online extraction, separation, derivatization and fluorescence detection of peptide hormones, and in principle could be used for any peptide hormone with a free primary amine for fluorescamine tagging. Calcitonin (CT) and its prohormones were studied due to their high expression in DMS53 cells. This is the first time that the levels of CT and its prohormones, CTG, CTGK and CTGKK, have been separated and quantified separately. The detection limits of the present HPLC method were determined to be around 1 ng/mL of cell medium. This method was then applied to whole cell inhibition studies, and this is the first time that inhibitors of CT biosynthesis in human cancer cells were studied, allowing the direct observation of the effects on CT levels and its prohormones. As CPE and PAM are involved in the specific cleavage of C-terminal basic amino acids and the last step of peptide bioactivation, respectively, CPE was also targeted to moderate CT levels. The newly developed whole cell PAM inhibition assay allowed direct determination of the effects of PAM and CPE inhibition on CT production.

The challenge is due to the recognition of the possibility of prevention in at least 70-80 % of all the cases and the extreme difficulties encountered in the treatment of this neoplasm. Despite the knowledge of prevention measures such as cessation of cigarette smoking the incidence continues to increase in many countries. The increase is particularly notable in females in the west ernized countries where the death rate in females in certain regions sur passes that of breast cancer. Furthermore, in many developing countries lung cancer is now being diagnosed with increasing frequency in both sexes and it is expected to be a major cause of death in those countries later in this century or the beginning of next century if the tobacco consumption will continue its rapid rise. With respect to therapy the 1970'es brought considerable progress in understanding of the clinical behaviour of lung cancer thereby establishing the importance of distinguishing between the major histologic types. Thera peutic advancement was particularly experienced in small cell carcinoma with the introduction of combination chemotherapy after this special dis ease entity among lung cancers was recognized as being a disseminated dis ease in almost all cases at the time of diagnosis. It was expected that the improvement in therapy would have continued in the early 1980'es, not only for small cell lung cancer but also for the other cell types.
