Book excerpt: Contexte : Depuis l'avènement des trithérapies, l'infection par le VIH est devenue une maladie chronique, ce qui nécessite de repenser l'offre de soins et le type de suivi médical. Quelles sont les attentes des médecins de ville (MG) pour accepter de suivre les patients vivant avec le VIH (PVVIH)? Que pensent les patients d'un suivi double ville-hôpital ? Méthodes : Deux études prospectives transversales déclaratives : 39 généralistes de Saint-Denis enquêtés par courrier postal entre janvier et mars 2013 ; 62 PVVIH et suivis en hôpital de jour à l'hôpital Delafontaine de Saint-Denis, interrogés entre février et mai 2012. Résultats : 64% des omnipraticiens dionysiens interrogés sont prêts à suivre des PVVIH et souhaitent une réimplantation de réseaux ville-hôpital (76%) avec un centre référent et formateur (68%). Un partenariat avec l'ARS et le COREVIH locaux, est l'élément clé pour un parcours de soins coordonné des PVVIH. Pourtant, les patients favorables à un suivi en ville sont peu nombreux (24.2%). Ils ont en général recours au MG plus fréquemment et apprécient notamment sa prise en charge globale (56.5%). L'infectiologue référent est plébiscité par la majorité des PVVIH pour ses compétences (74.3%) et pour la relation de confiance qu'ils entretiennent avec lui (69.2%). Conclusion : Au-delà de l'aspect économique, l'enjeu d'un suivi en ville des PVVIH est réel. La création d'un « maillage régional » des offres de soins en santé sexuelle devrait permettre de toucher une plus grande partie de la population et d'intégrer le patient dans une prise en charge coordonnée, avec des professionnels de santé clairement identifiés et travaillant en équipe, de façon complémentaire.
