Book excerpt: Contexte : La pathologie fracturaire chez la personne âgée est un motif fréquent d’admission aux urgences. Elle résulte d’un mécanisme lésionnel simple (la chute) survenant sur un terrain prédisposant (la fragilité osseuse). La Haute Autorité de Santé a émis des recommandations concernant la prise en charge des patients âgés aux urgences, des patients âgés chuteurs et de l’ostéoporose. En revanche, peu de données sont disponibles sur la mise en pratique de ces recommandations sur le terrain. Méthode : Etude descriptive, rétrospective, réalisée au CHRU de Lille. A chaque étape du parcours hospitalier, recueil des données socio-démographiques, des principaux facteurs de risque modifiables de chute et d’ostéoporose, ainsi que des principaux facteurs de risque de morbi-mortalité post-fracturaire des patients de 75 ans et plus hospitalisés pour fracture à basse énergie, à partir de leur dossier informatisé et papier. Résultats : 105 patients ont été inclus, avec une hospitalisation à l’issue des urgences dans 83% des cas, dont les 3⁄4 en traumotologie. Une chirurgie était nécessaire dans 72% des cas. En cas de fracture, des facteurs de risque comme la prise de psychotropes, la polymédication et la carence en calcium ont été recherchés dans plus de 90% des cas. D’autres facteurs de risque, pourtant simples à rechercher, comme une hypotension orthostatique ou un déficit en vitamine D sont largement délaissés et recherchés dans moins de 20% des cas. En cas de pathologie fracturaire, l’évaluation et la prise en charge de la douleur est assurée chez plus de 9 patients sur 10. La prévention des morbi-mortalités liées au décubitus prolongé est réalisée dans plus des 3⁄4 des cas lorsque le patient est hospitalisé en traumatologie, en CSG ou en SSR. Cette prévention n’est réalisée que dans moins de 15% des cas au SAU chirurgical. Conclusion : Les principaux facteurs de risque de fracture du sujet âgé restent largement sous-évalués durant le parcours de soin hospitalier. La prévention des facteurs de risque de morbi-mortalité post-fracturaire semble également perfectible. Pour améliorer le pronostic vital et fonctionnel de ces patients, le développement de stratégies de prise en charge multidisciplinaires « orthogériatriques » parait souhaitable.

Biohydrogen is considered a promising future biofuel because of its intrinsic clean and high-energy content properties and the way it is produced. In addition to being produced through environmentally friendly biological means, its conversion to energy yields only pure water, which is an ideal energy carrier in reducing greenhouse gas emissions from fossil fuel combustion. Unlike other well-developed biofuels such as bioethanol and biodiesel, biohydrogen production is still in the early stage of development. A variety of technologies are being developed for biohydrogen production. This chapter presents a review of the state-of-the-art and perspectives of bioprocess design for biohydrogen production research in the context of pathways, microorganisms, metabolic flux analysis, process design, and reactor system.
