Book excerpt: La prescription de multithérapies antirétrovirales puissantes prolonge maintenant l’espérance de vie des patients infectés par le VIH, mais s’accompagne de l’émergence de nouvelles causes de décès. Nous avons donc analysé rétrospectivement les causes de décès survenus entre 2003 et 2005 chez les patients infectés par le VIH suivis à l’hôpital Bicêtre. Nos données ont été recueillies à l’aide du formulaire de l’étude nationale ANRS « Mortavic 2005 » et comparées d’une part à celles des décès en France, d’autre part à celles des patients vivants de la file active de Bicêtre. Les décès d’origine hépatique et liés au VIH ont fait l’objet d’une étude spécifique. Nous avons recensé 51 décès, dont 45 ont pu être exploités, sur une file active totale de 1224 patients en 2005. L’âge médian au décès était de 42 ans, avec une durée médiane d’infection à VIH longue (11,2 ans). La majorité des patients étaient traités avant le décès (89%), avec un taux médian de lymphocytes CD4 de 159/mm3 et une charge virale médiane de 4,36 log10 copies/mL. Soixante-cinq pour cent des patients étaient fumeurs, 60% étaient co-infectés par le virus de l’hépatite B et/ou C. Dans notre centre, les hépatites virales représentent la première cause de décès (36%), suivies par les pathologies SIDA (29%) et les cancers non SIDA et non liés aux hépatites (16%). Les patients contaminés par toxicomanie intraveineuse (56%) et la coinfection par l’hépatite C (58%) sont surreprésentés à Bicêtre par rapport à l’enquête Mortavic. Une partie des décès survenus dans notre centre aurait pu être prévenue soit par un dépistage et une prise en charge plus précoces de l’infection, soit par un sevrage des addictions, voire par un traitement spécifique des hépatites. La modification des recommandations d’experts a suivi cette constatation à l’échelon national et a incité l’HAS à renforcer la place du médecin traitant dans la prise en charge globale des patients. Son rôle dans le dépistage précoce de l’infection à VIH, le suivi des patients, la prévention des co-morbidités et la coordination des soins devient primordial.

Book excerpt: The most recent Ebola epidemic that began in late 2013 alerted the entire world to the gaps in infectious disease emergency preparedness and response. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. This publication summarizes the presentations and discussions from the workshop.
