L'analyse des témoignages recueillis dans notre enquete a révélé une évolution en matière d'éducation précoce. Nous ne saurions néanmoins parler d'évolution sur le plan technique. Selon les professionnels, il s'agirait tout au plus d'un affinement des potentialités techniques et de leurs utilisations. L'évolution se ressent plus nettement sur le plan relationnel : parents, orthophonistes, tous témoignent d'un changement de regard sur la vie, l'enfant, la communication et la manière d'accompagner l'autre. Ce changement de regard redonne à autrui son identité, point clé de l'évolution. Redonner une identité en acceptant la différence : celle de l'enfant mais également celle des parents, incapacités sous le choc du diagnostic dans leur fonction parentale.

De nombreux enfants sourds font pourtant part des difficultés, d'interrogations, de remarques particulières relatives à leur surdité. Leurs parents et les orthophonistes sont souvent destinataires de ces questionnements. Si certaines particularités ont ainsi été constatées, peu de propositions sont faites pour améliorer leur vécu parfois douloureux. En nous basant sur ce constat, nous avons souhaité créer un support illustrant diverses situations vécues par les enfants sourds comme difficiles ou sujettes à des interrogations. Il serait un médiateur intéressant pour encourager les échanges et la communication sur différents thèmes et limiter ainsi leurs angoisses et leurs incompréhensions. Ce support serait utilisé lors de l'accompagnement orthophonique de l'enfant sourd et de sa famille. Pour réaliser ce projet, nous avons interrogé des parents d'enfant sourd et des personnes sourdes avec des questionnaires. Leurs témoignages ont permis d'appréhender leur vécu de la surdité et d'identifier les difficultés, les interrogations, les remarques récurrentes ainsi que les besoins des enfants sourds. Nous avons ainsi créé dix planches de dessins constituant notre support. Ce dernier a enfin été présenté à des orthophonistes, des parents d'enfant sourd ainsi qu'à des enfants et adultes sourds pour valider sa pertinence. L'enthousiasme des personnes rencontrées, appréciant la forme et le fond, confirme nos hypothèses initiales. Ce matériel semble correspondre à un besoin réel d'échange, de dialogue et de communication entre l'enfant, sa famille et l'orthophoniste sur des thèmes relatifs à la surdité.

L'évaluation orthophonique est spécifique: elle évalue à la fois les compétences de l'enfant et les interactions communicatives parents-enfant. Nous avons proposé aux parents de 4 très jeunes enfants sourds un questionnaire parental auto-administré, l'Inventaire Français du Développement Communicatif (IFDC). Nous nous sommes interrogés sur le fait qu'un tel questionnaire puisse contribuer non seulement à l'ajustement du projet orthophonique aux besoins de l'enfant, mais aussi à l'ajustement de l'implication parentale dans le quotidien de la surdité et à l'ajustement des relations orthophoniste-parents. Nous avons observé les séances de prise en charge orthophonique de ces enfants avec leurs parents, avant et après propositions de l'IFDC. Puis nous avons proposé des interviews aux parents et aux orthophonistes participant à cette étude. Les données recueillies montrent une appropriation particulière de cet outil par chaque parent et orthophoniste. Un tel questionnaire semble intégrable à la prise en charge du très jeune enfant sourd. La proposition de ce questionnaire a eu des effets sur l'implication des parents dans la prise en charge de leur enfant, sur la relation parents-orthophoniste, ainsi que sur l'appropriation du handicap par les parents. Des perspectives intéressantes sont apparues quant à l'évaluation pour les orthophonistes.

L'âge d'implantation des enfants étant de plus en plus précoce, il convient de mettre en oeuvre des moyens adaptés et de reconnaître les parents comme acteurs principaux du développement de la communication et du langage de leur enfant. Certains parents bilingues, qui maitrisent peu le français, se limitent dans leurs interactions verbales. Cette situation peut ralentir le développement phonologique et linguistique des enfants. Il convient alors de leur proposer des outils personnalisés, adaptés à leur niveau de langue française. Nous avons créé un outil d'accompagnement parental, basé sur l'utilisation des onomatopées. pour quatre jeunes enfants sourds profonds et leurs parents. Tous les enfants présentaient un développement du langage oral restreint et leurs parents, bilingues, maîtrisant peu le français. parvenenaient difficilement â ajuster leur comportement de communication verbale. L'outil prend la forme d'un livret d'abord présenté en séance, puis â utiliser â la maison. L'objectif est d'offrir aux parents un support d'échange avec leurs enfants, simple et encourageant la production orale de ces derniers. Nous avons évalué le langage oral des enfants et observé les interactions avec leurs parents, avant et après la mise en place du livret. La comparaison entre les résultats des deux évaluations montre une augmentation des interactions parent-enfant, initiées par les parents, et plus adaptées. Nous remarquons également de plus nombreuses tentatives d'oralisation chez les enfants et un enrichissement de leurs productions. L'utilisation d'outils spécifiques dans l'accompagnement des familles bilingues est donc nécessaire et pertinente. Les onomatopées représentent un support intéressant pour encourager le développement des interactions et le développement phonologique.

Dédiée aux parents, son organisation tient cependant compte des besoins organisationnels d'un CAMSP. Dans ce présent mémoire, nous avons conçu cette brochure d'information à partir du cahier des charges existant, nourri de nos propres réflexions issues de l'accompagnement parental et d'échanges fructueux avec un groupe de relecture. Deux questionnaires destinés aux parents et aux professionnels ont permis d'affiner son fond et sa forme avant sa modification finale.
