With the introduction of the national competency framework for registered nurses (2015), we realised there was a gap in the training we were offering both to enable staff to fulfil their competencies but also to give them the confidence in caring for patients who have devastating brain injuries (DBI) that may become organ donors.Methods: Simulation as a training method has been shown to stimulate, engage and inspire nurses and is used throughout medical and healthcare training. Collaboratively the trust specialist nurses and clinical lead for organ donation, worked alongside the senior sisters and link nurses to formulate a DBI day where we combine theory and simulation to journey through the experience of a patient with a DBI in the ITU from admission, management, coning, neuro death testing, family conversations and approaching for organ donation.Results: Over twelve months we have successfully run three days which have been fully attended and supported, we actually have a waiting list of staff who wish to attend. The feedback has shown we are offering a positive, informative day which has revitalised the passion for organ donation within the nursing team driving referrals and ensuring the best practice guidance is followed. Discussion: The day has enabled our staff to feel more confident about their role and the role the Specialist nurses for organ donation have in supporting families. This increased awareness has subsequently enhanced the care our potential donors and families receive enabling them to make decisions about donation in a supportive environment at a time that is right for them.

Following the 1997 study, the Department of Health and Human Services requested a follow up study to promote such efforts. The central activity for this study was a workshop held in Washington, D.C., on May 24-25, 1999. The workshop provided the opportunity for extensive dialogue on non-heart-beating organ donation among hospitals and organ procurement organizations (OPOs) that are actively involved in non-heartbeating organ and tissue donation and those with concerns about whether and how to proceed. The findings and recommendations of this report are based in large measure on the discussions and insights from that workshop. Non-Heart-Beating Organ Transplantation includes seven recommendations for developing and implementing non-heart-beating-donor protocols. These recommendations were based on the findings and recommendations from the 1997 IOM report and consensus achieved among participants at the national workshop. These practices are ethically legitimate but are not compatible with traditional rules of medical ethics that doctors must not intentionally cause the death of their patients and that vital organs can be obtained for transplantation only from dead donors. In this book Miller and Truog undertake an ethical examination that aims to honestly face the reality of medical practices at the end of life. They expose the misconception that stopping life support merely allows patients to die from their medical conditions, and they dispute the accuracy of determining death of hospitalized patients on the basis of a diagnosis of "brain death" prior to vital organ donation. After detailing the factual and conceptual errors surrounding current practices of determining death for the purpose of organ donation, the authors develop a novel ethical account of procuring vital organs. In the context of reasonable plans to withdraw life support, still-living patients are not harmed or wronged by organ donation prior to their death, provided that valid consent has been obtained for stopping treatment and for organ donation. Organ donation refusal from relatives of potential donors with brain death significantly reduces organ availability. We report a brief analysis about family conflicts in decision-making and causes for refusing donation; moreover, we describe new family-centered strategies in the intensive care unit (ICU) and our systematic communication approach between medical staff and patients' relatives. In 2016 we conducted a single-center, non-randomized, controlled and before and after study in our ICU, an 18-bed intensive care unit (ICU) of a university hospital. We compared the rate of consent for organ donation before and after the introduction of the new communication approach. The application of a new communication approach between medical staff and relatives of brain-dead patients was associated with a significant increase in the rate of consent to donation. The positive results of the 3-year period 2013-2015 have been confirmed in the 2-year period 2016-2017. Our results highlight the importance of empathy and counselor support of relatives in the ICU.
