Book excerpt: The International Society on Oxygen Transport to Tissue (ISOTT) was founded in 1973 "to facilitate the exchange of scientific information among those interested in any aspect of the transport and/or utilization of oxygen in tissues". Its members span virtually all disciplines, extending from various branches of clinical medicine such as anesthesiology, ophthalmology and surgery through the basic medical sciences of physiology and'biochemistry to most branches of the physical sciences and engineering. The seventeenth annual meeting of ISOTT was held in 1989 for four days, from July 21 to 24, at the Max Planck Institute for Experimental Medicine and the adjoining University Hospital (Klinikum), in Goettingen, Federal Republic of Germany. It attracted 147 active registrants and approximately 40 accompanying persons. The very successful format originated by Dr. Ian Longmuir in 1985, consisting of posters accompanied by an abbreviated oral summary, was continued with slight modification. Virtually all of the presentations utilized this format, with each poster session preceded by a formal discussion during which the presenter briefly reviewed the poster aided by a few slides. All posters remained in place for the entire four days of the meeting.

In past years it has been concerned mainly with the development of perfusion tracers, the biodistribution of which reflect the regional blood flow to areas of major organs such as the heart and brain. However, a major new direction of interest now lies in the development of receptor-binding radio-tracers which can be used to perform in-vivo characterisation of diseased tissues and it is likely that much of the future research in this field will follow this direction. The difficulties in developing such tracers are considerable. The researcher must first identify a promising target for radiopharmaceutical development. High specific activity radioactive molecules must be designed and synthesised which will both bind to the target receptor with high affinity, and also have the physicochemical characteristics which will allow them to reach the target site in sufficient quantity while at the same time showing minimal uptake in non-target tissues. Thus the knowledge base required for radiophannaceutical development has now expanded beyond the limits of radiopharmaceutical chemistry to include aspects of biochemistry, molecular biology and conventional drug design. The portfolio of basic knowledge required to support current radiopharmaceutical development is changing and scientists working in this arena need to be trained in this regard. At the same time, the very latest developments in the field need to be communicated to the scientific community in order to stimulate the advancement of this exciting new direction of research.

Success is attributable to its National Chairman, Jerry Lewis, to its effective corporate membership, and to the many physicians and scientists who give their time freely to advise on policies, to review grant applications, and to participate in meetings such as this. I should like to acknowledge a large number of other individuals to whom we are indebted: the broad segment of the American public which continually and generously supports our spectrum of services. The Muscular Dystrophy Association, next year, should raise in excess of $115,000,000. These contributions are derived from more than 10 million American families. These families are not only pledging their money but expressing their hopes that we will find answers to the tragic problem of neuromuscular disease. The rapid growth of the over-eighty-five population, the group which, as recent studies have confirmed and as many of us clinicians have long suspected, has an even higher prevalence than previously quoted of dementing disorders, is the major cause of this. We are thus challenged by, as Bernard Issacs used to call it, "the survival of the unfittest," as well as the oPtimistic approach of "bringing life to years," as John F. Kennedy said. The fact is that we, as a society, tend to confuse "treatment" and "cure" (and "prevention"). As the proceedings of the conference which this book represents emphasize, there is considerable work going on about the potential prevention of, or at least the reduction of, symptomatology in these illnesses by interventions genetically, chemIcally, and so forth. However, the more we find out, the more complicated it becomes, and the more heterogeneous Alzheimer's and the related disorders appear to be, not only in their manifestations (as clinicians have long recognized) but also in the individual initiating and underlying processes. For these reasons, absolute preventive techniques or the likelihood of an intervention which will reverse the process in a high proportion of patients, do not appear to be just around the corner.

Their economic impact is considerable as they infect humans, livestock, poultry and companion animals. Murine hepatitis virus (MHV) infection of the mouse and rat central nervous systems are the subject of intense study; these investigations are providing insights into the potential role of viruses in human neurological diseases and, more generally, into mechanisms causing neurological damage. The single-stranded, positive-sense RNA genomes of two species of these enveloped viruses (IBV and MHV) have been cloned completely and one of them (lBV) sequenced in its entirety, revealing a genome size of some 27000 nucleotides. This has made possible more incisive investigations into the nature of those polypeptides, encoded by more than half of the genome, which are likely to contribute, in the main, to RNA polymerase/replicase activity. Intriguingly, ribosomal frameshifting is exhibited within the mRNA coding for these polypeptides. The cloning/sequencing phase of coronavirology for which the 1980's will be partly remembered, has provided a sound framework for furthex: studies of the virus structural proteins and also some provocative insights relevant to these studies. The large spike glycoprotein(s), responsible for membrane fusion and bearing important antigenic sites, varies amazingly in length and composition both within as well as between coronavirus species. Receptors on host cells have been identified. The integral membrane glycoprotein (M) has been shown to use internal hydrophobic sequences to direct translocation within membranes.

Cigarette smoking has been implicated in both the initiation and exacerbation of the atherosclerotic process. Data to support this implication derives primarily from epidemiologic studies where the relationship between the incidence of atherosclerosis in people who smoke cigarettes has been shown to have a strong correlation. There are few well established explanations for this phenomenon, and basic molecular, biochemical, and cellular mechanisms associated with smoking and the development of atherosclerosis remain both undefined and virtually unexplored. Even the epidemiologic correlation between cigarette smoking and the development of atherosclerosis needs further critical studies. It is known that individuals who do not smoke cigarettes develop athero sclerosis and it is also known that in people who smoke but have normal or low blood cholesterol/lipoprotein levels, the incidence of development of atherosclerosis is no different from that which is found in a non smoking population. Answers which explain such observations must address fundamental biological mechanisms. Toward this end, the purpose of this volume is to assemble, in a single publication, information which will address the questions; what basic cellular and/or molecular mechanisms are associated with the development of atherosclerosis and how does cigarette smoking influence such mechanisms to initiate or exacerbate the atherosclerotic process? Clearly, the development of atherosclerosis is a complex, multifactorial biological event.

These include cellular signal transduction via the G-protein cycle, and in the generation of many cellular mediators, such as the platelet activating factor (PAF) and the eicosanoids that participate in the initiation and propagation of inflammation, to mention a few. This symposium was organized to obtain an overview of current investigations on this enzyme from the standpoint of its chemistry, molecular biology and physiology. Another important focus of this symposium concerns the regulation of PLA2, including endogenous and synthetic inhibitors and activators of this enzyme. To review these important areas in PLA2 research we invited scientists who made significant contributions in this field. The papers in this volume are organized to emphasize the recent advances in several areas of investigation, including: (I) the structure and mechanism of action of PLA2, (2) mechanism of activation of PLA2, (3) molecular biology, physiology and endogenous inhibitors of this enzyme and finally, (4) clinical investigations emphasizing the pathophysiological role of this enzyme in human diseases.
