Book excerpt: Lacking the capacity to store oxygen, the human organism depends on having an unimpaired and continuous supply of oxygen for its survival. Life-threatening disturbances can be encountered by the clinician at any step in the oxygen cascade from ambient air to tissue metabolism. It is therefore necessary for the mechanisms of oxygen uptake, transport, distribution and diffusion into tissues as well as the unaffected cellular metabolism to be well understood and monitored. An international symposium focusing on these issues was held in Münster, FRG, May 11-12, 1990. This book contains the lectures held at the symposium, and thus provides an excellent basis for further discussion.

Available in PDF, EPUB and Kindle. Book excerpt: This presentation describes various aspects of the regulation of tissue oxygenation, including the roles of the circulatory system, respiratory system, and blood, the carrier of oxygen within these components of the cardiorespiratory system. The respiratory system takes oxygen from the atmosphere and transports it by diffusion from the air in the alveoli to the blood flowing through the pulmonary capillaries. The cardiovascular system then moves the oxygenated blood from the heart to the microcirculation of the various organs by convection, where oxygen is released from hemoglobin in the red blood cells and moves to the parenchymal cells of each tissue by diffusion. Oxygen that has diffused into cells is then utilized in the mitochondria to produce adenosine triphosphate (ATP), the energy currency of all cells. The mitochondria are able to produce ATP until the oxygen tension or PO2 on the cell surface falls to a critical level of about 4–5 mm Hg. Thus, in order to meet the energetic needs of cells, it is important to maintain a continuous supply of oxygen to the mitochondria at or above the critical PO2 . In order to accomplish this desired outcome, the cardiorespiratory system, including the blood, must be capable of regulation to ensure survival of all tissues under a wide range of circumstances. The purpose of this presentation is to provide basic information about the operation and regulation of the cardiovascular and respiratory systems, as well as the properties of the blood and parenchymal cells, so that a fundamental understanding of the regulation of tissue oxygenation is achieved.

Book excerpt: Te practice of intensive care medicine is at the very forefront of titration of treatment andmonitoringresponse. Te substrateofthiscareisthe criticallyill patientwho,by defnition, is at the limits of his or her physiologic reserve. Such patients need immediate, aggressive but balanced life-altering interventions to minimize the detrimental aspects of acute illness and hasten recovery. Treatmentdecisionsandresponsetotherapyareusually assessed by measures of physiologic function, such as assessed by cardio-respiratory monitoring. However, how one uses such information is ofen unclear and rarely supported by prospective clinical trials. In reality, the bedside clinician is forced to rely primarily on physiologic principles in determining the best treatments and response to therapy. However, the physiologic foundation present in practicing physicians is uneven and occasionally supported more by habit or prior training than science. A series of short papers published in Intensive Care Medicine since 2002 under the heading Physiologic Notes attempts to capture the essence of the physiologic perspectives that underpin both our understanding of disease and response to therapy. Tis present volume combines the complete list of these Physiologic Notes up until July 2006 with the ass o cia t ed r e vie w a r tic les o v er t h e s a m e in t er val t ha t a ls o addr ess e d t hes e cen tral issues.

Experts from basic science as well as clinical research provide information that improves understanding of how to treat O2-transport disturbances in daily practice. Special interest is focussed on the question of supply dependency of O2-uptake and its role in multi-organ failure in the critically ill. Other topics are the clinical monitoring of O2-transport and O2-utilisation, oxygen toxicity, and the role of O2-radicals and the reperfusion syndrome in organ transplantation and cardiopulmonary resuscitation.

Book excerpt: Disturbances in peripheral O extraction can be produced in dogs treated with 2 endotoxin and thereby provide an opportunity to test theories for the origin of pathological O supply dependency or to try different treatment modalities. The 2 most serious deficiency in the current animal models is the inability to mimic the increased O demand that is observed in patients at 02 delivery rates in excess of 2 normal. A particular feature of this increased O demand is that it apparently does 2 not stimulate increased 02 extraction, although the limitation in O extraction has 2 not been explored in patients by lowering 02 supply, for obvious reasons. At least two possibilities to account for increased 02 demand could be investigated in animal models, however. The amount of 02 that is utilized in extramitochondrial pathways, which is normally on the order of 10%, may be greatly increased in ARDS and sepsis by O radical formation. There is presently no information 2 concerning how much 02 might be used in this way. Another strong possibility is that mitochondrial injury, perhaps as a result of 02 radical formation, uncouples oxidative phosphorylation. Some evidence presently in the literature supports this idea [19]. Indeed, the association of increased blood lactate levels with higher than expected 02 demands makes uncoupling a very attractive hypothesis that warrants further investigation in animal models using such agents as 2,4-dinitrophenol. Available in PDF, EPUB and Kindle. Book excerpt: During the past decade there has been a considerable expansion in the understanding, assessment and treatment of critically ill patients. An attempt to portray our present knowledge of this diverse field in a comprehensive textbook is therefore a formidable and daunting undertaking. However, many colleagues in different disciplines and from different countries convinced us of the need for such a work and, greatly encouraged by their interest, we embarked upon the project. Any single text, even a very long one, cannot cover every aspect of critical illness. Our first task was, therefore, to define those areas of the subject that had to be included. This we did on a systematic basis, and although the final emphasis of the book is very largely directed to clinical matters, we felt it pertinent to include in some detail accounts of relevant physiology and technology. Care of the Critically Ill Patient is divided into twelve sections, each concerned with major facets of critical illness; each of the sixty-eight chapters includes a topical and often extensive bibliography. The many chosen contributors form an international group of specialists whose combined expertise embraces the topics that we have selected.

Book excerpt: The International Society on Oxygen Transport to Tissue (IS OTT) was founded in 1973 as a scientific society providing a forum for bioengineers, basic scientists (physiologists, biochemists and physicists) and clinicians (including anesthesiologists, intensive care specialists, pediatricians, neonatologists, internists, surgeons and other specialists) to facilitate the exchange of scientific information among those interested in any aspect of the transport and/or utilization of oxygen in tissues. From the ranks of its members, many fundamental discoveries and inventions have been made involving the many aspects of oxygen transport and utilization by biological tissues. The ISOTT proceed ings, now in its 14th edition, has become a standard work in the field as witnessed by the inclusion in the Science Citation Index of all volumes published so far. The 19th ISOTT Meeting was held in Cura~ao from August 24th through August 30th, 1991. The Cura~ao Meeting attracted 145 registrants and 45 accompanying persons. The format originated by Dr. Ian Llngmuir in 1985, consisting of posters ac companied by an abbreviated oral summary, was again successfully handled with slight modifications. The meeting was introduced by 6 review lectures covering the whole field of oxygen transport from bioengineering, the problem of diffusion in lung, blood and tissue through pathology of oxygen uptake in the lung, oxygen supply dependency of the critically ill to artificial oxygen carriers. A special session dealt with oxygen sup ply under ambiant pressure changes.
