Abstract: The last oral health survey conducted by the government of Hong Kong in 2001 found that dental caries was common among the kindergarten children and that most of their dental caries was untreated. The overall aims of this research project were to find out the oral health status of the kindergarten children in Hong Kong and the outcomes of providing a private practice based dental service programme to these children. To achieve the first aim, an oral health survey was conducted in 2007. Through stratified simple random sampling, 12 kindergartens in Hong Kong Island were selected. All children attending grades 1 to 2, aged 3 to 5 years, in the kindergartens were invited to participate. Children with parental consent were clinically examined by one of two calibrated examiners. A questionnaire on the child's demographic background and oral health related behaviours was completed by the parents. Response rate of the survey was 89% and 1,343 children were examined. Their mean age was 52 months (SD=7) and 51.5% were boys. Around one third (35.3%) of the children had experienced dental caries. The mean dmft score was 1.5, increasing from 1.2 at age 3 to 2.0 at age 5 (ANOVA, p=0.016). Soft, cavitated caries (dt) accounted for 90% of the dmft score. The prevalence and severity of dental caries in the children were associated with the children's socio-demographic background, and their dietary and tooth-brushing habits. To find out the feasibility of implementing a private clinic based dental service program for the preschool children a total of 245 children attending grade 1 in 9 kindergartens were invited to participate. Baseline information of the children was collected through a clinical examination and a parental questionnaire at April 2007. After baseline data collection, the study children were randomly allocated into the test or the control group. Children in the test group were offered free dental services at regular intervals in 3 private dental clinics. Evaluation took place in July 2009 after the completion of the 24-month intervention programme through conducting clinical examinations and a parental questionnaire. After 24 months, 88% of the control and test group students remained in the study. The mean dental appointment attendance rate of the test group children was 53%. Individualized instructions on oral hygiene were provided to 82% of the children and 52% of the children had received topical fluoride application. Treatments were provided to more than 30% of the children and the mean number of dental fillings placed is 1.1. The mean total running cost per child was around HK$100 a year. The kindergarten children who were covered by the study dental service programme had better oral health than that of the children who were not. The study programme was effective in preventing development of new dental caries and in reducing the amount of untreated caries among the test group children. However, it was not effective in improving the oral health related behaviours and oral hygiene status of the children. The study programme was acceptable to the kindergartens, the parents and the children by the satisfaction study. Consumer satisfaction was reasonable high. All rights not granted by the above license are retained by the author. Abstract: Early childhood caries is a condition of rapidly progressing rampant dental caries in infants and young children. It is considered as a major public health problem affecting pre-school children and it is the most common dental disease which affects 28% of children within two to five years of age. ECC not only induces pain and discomfort, but can also affect communication, nutrition status, learning abilities, speech and quality of life, which may progress into adulthood and pose a heavy burden on the healthcare system in long-term. In Hong Kong, over 50% of children were affected by dental caries in 2001 but over 70% of children had never been to a dentist for a regular check up at age five. Thus, much of the tooth decay was remained undetected and untreated. Dental caries can be preventable and achievable. Collaboration between families, early care and health care professionals is required to promote effective oral health care. Numerous studies have found that educational programs and workshops are effective in promoting oral health and can provide children a lifelong opportunity to be free from preventable oral disease. In order to promote oral health to achieve the mission of the Department of Health in Hong Kong- at least 65% of 5 years old children are free from caries by the year 2020, an evidence-based guideline for a community-based programme in oral health education was developed in the proposed setting after a critical appraisal of the reviewed evidence. The comprehensive intervention plan, including communication plan with stakeholders, training of staffs and pilot testing will be carried out to facilitate the implementation of the innovation. The oral health programme will be evaluated for its effectiveness in achieving the patient outcomes, health providers' outcomes and system outcomes in the proposed settings. Abstract: Objectives: To find out how often the dentists and dental hygienists provide oral health education on interdental cleaning and to investigate the factors and potential barriers on the provision of such education. To compare how dentists and hygienists with different educational and practical backgrounds differ in the views on the provision of oral health education on interdental cleaning Design: Cross-sectional survey Participants: Self-reporting questionnaires were sent by mail to all dentists from the general register and electronically to the dental hygienists in Hong Kong. Information on their background information, frequency, type and pre-operative assessment of provision of oral hygiene education on interdental cleaning, together with the perceived reasons or barriers that they encountered were collected. Results: A total of 563 fully completed questionnaires were returned (response rate=31%). Only 4.1% of the dental practitioner reported rarely or never provided interdental cleaning education to their teenage and adult patients. Specialisation area was found to be a significant predictor of provision of interdental cleaning education, with adjusted odds ratio (OR) of 19.4 (95%confidence interval 5.19 -72.67). Endodontics and oral surgery specialists were least likely to provide interdental cleaning education to teenage and adult patients. The most commonly reported reasons/barriers for not providing oral hygiene education on interdental cleaning were lack of clinical time (45.2%), followed by patient not interested (30.2%), and lack of patient's compliance (29.3%). Lack of clinical time, lack of patient's compliance, and lack of educational tools significantly lowered the likelihood of providing interdental cleaning education by 57%, 55% and 94% (all p-values Conclusion: Most of the Hong Kong dental practitioners are willing to provide oral hygiene education and give recommendations on interdental cleaning despite the low percentage in practicing interdental cleaning among the Hong Kong population. The specialization of area, perceived lack of clinical time, lack of patient's compliance and lack of educational tools may lower the likelihood of providing such education.
