Book excerpt: Now published in two volumes to accommodate new chapters on the many advances in understanding and treatment options, this volume represents the definitive reference on inflammatory bowel disease, a spectrum of diseases that is receiving increasing attention as our understanding of the etiological factors increases and diagnostic tools are refined. Basic research accelerated at the beginning of this decade and is now yielding new, more targeted treatments than were available just a few years ago. Volume 1 is on IBD and Ulcerative Colitis, and Volume 2 is on IBD and Crohn's Disease. All areas that were covered in the second edition have been expanded and updated. New sections include one addressing etiology and pathophysiology of IBD, including environmental influences and effect of age at onset, and one with a focus on aggravating factors in the IBD patient such as infectious agents, non-steroidal anti-inflammatory drugs, and pregnancy.

Book excerpt: Now published in two volumes to accommodate new chapters on the many advances in understanding and treatment options, this set of volumes represents the definitive reference on inflammatory bowel disease, a spectrum of diseases that is receiving increasing attention as our understanding of the etiological factors increases and diagnostic tools are refined. Basic research accelerated at the beginning of this decade and is now yielding new, more targeted treatments than were available just a few years ago. Volume 1 is on IBD and Ulcerative Colitis, and Volume 2 is on IBD and Crohn's Disease. All areas that were covered in the 2nd edition have been expanded and updated. New sections include the rapidly expanding knowledge of genetics and the role of the intestinal flora and environmental factors in etiology and pathogenesis. Among the 168 chapters, there are 20 on biologic therapies and 30 on surgical management. The consultant/authors clearly state their approach to important issues, such as the duration of immunomodulator and of anti-TNF-a use and the options for managing isolated low-grade dysplasia. A strong focus on the individual patient is woven throughout both volumes, including the benefits and risks of potentially life-altering therapies and surgeries. Entire sections detail concerns about the well-being of each person. This book provides information for health professionals who help both pediatric and adult patients navigate through the lifelong shadow of a chronic, probably genetically determined ordeal.

These disease conditions include food allergies, such as celiac disease, and the inflammatory bowel diseases, such as ulcerative colitis and Crohn's disease, although their exact etiological mechanisms remain to be revealed. --Book Jacket.

This book was released on 1998-12-31 with total page 340 pages. Available in PDF, EPUB and Kindle. Book excerpt: Ulcerative colitis and Crohn's disease remain a great therapeutic challenge to the medical community. In recent years knowledge about the pathogenesis of these diseases has progressed rapidly but the cause of the diseases remains completely unknown. It has become clear that dysregulation of the mucosal immune system is the basis for the chronic evolution of the diseases in a genetically susceptible population. Exciting new therapeutic approaches have been attempted in the last couple of years and cytokine and anti-cytokine treatments in particular seem very promising, especially in intractable disease. The format of the Falk Symposium 106 on `Advances in Inflammatory Bowel Diseases', held in Brussels, Belgium, June 18-20, 1998, was somewhat innovative as each session attempted to link the new insights into pathogenetic mechanisms with new therapeutic approaches, resulting in optimal information transfer. The classic therapeutic schemes were updated with a special focus on step-wise build-up of therapy.

This book was released on 2011-06-16 with total page 217 pages. Available in PDF, EPUB and Kindle. Book excerpt: Advances in medical, biomedical and health services research have reduced the level of uncertainty in clinical practice. Clinical practice guidelines (CPGs) complement this progress by establishing standards of care backed by strong scientific evidence. CPGs are statements that include recommendations intended to optimize patient care. These statements are informed by a systematic review of evidence and an assessment of the benefits and costs of alternative care options. Clinical Practice Guidelines We Can Trust examines the current state of clinical practice guidelines and how they can be improved to enhance healthcare quality and patient outcomes. Clinical practice guidelines now are ubiquitous in our healthcare system. The Guidelines International Network (GIN) database currently lists more than 3,700 guidelines from 39 countries. Developing guidelines presents a number of challenges including lack of transparent methodological practices, difficulty reconciling conflicting guidelines, and conflicts of interest. Clinical Practice Guidelines We Can Trust explores questions surrounding the quality of CPG development processes and the establishment of standards. It proposes eight standards for developing trustworthy clinical practice guidelines emphasizing transparency; management of conflict of interest ; systematic review-guideline development intersection; establishing evidence foundations for and rating strength of guideline recommendations; articulation of recommendations; external review; and updating. Clinical Practice Guidelines We Can Trust shows how clinical practice guidelines can enhance clinician and patient decision-making by translating complex scientific research findings into recommendations for clinical practice that are relevant to the individual patient encounter, instead of implementing a one size fits all approach to patient care. This book contains information directly related to the work of the Agency for Healthcare Research and Quality (AHRQ), as well as various Congressional staff and policymakers. It is a vital resource for medical specialty societies, disease advocacy groups, health professionals, private and international organizations that develop or use clinical practice guidelines, consumers, clinicians, and payers.

This book was released on 2001-06-30 with total page 288 pages. Available in PDF, EPUB and Kindle. Book excerpt: The aetiology of the chronic inflammatory bowel diseases - Crohn's disease and ulcerative colitis - is still enigmatic. The therapeutic approach has therefore traditionally focused on anti-inflammatory principles, including corticosteroids and aminosalicylates. Since a significant proportion of patients is steroid-dependent or refractory and because of the problematic side-effects of long-term systemic steroids, active immunosuppression has gained acceptance in the field. The classical immunosuppressants azathioprine and 6-mercaptopurine have long been evidence-based in IBD, but underused. Recently, methotrexate and cyclosporine have also been proven to be effective in certain situations. Newer drugs like tacrolimus, mycophenolate and others may be similarly useful but their potential is still unclear. Finally, the immune modifiers including IL-10 and TNF-antibodies have been successfully subjected to controlled trials. Other experimental drugs discussed in this book are on the horizon. This volume is the Proceedings of Falk Symposium 119 held in Freiburg-im-Breisgau, Germany, October 3-4, 2000, and covers systematically the field of immunosuppression in inflammatory bowel diseases. It is hoped that it will further the prudent use of these drugs in the proper clinical situations by increasing our understanding of both the mechanisms of action and the clinical benefit to the patient.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides a comprehensive and complete overview of biomarkers in clinical practice for inflammatory bowel disease (IBD) bringing together the literature in a clear and concise manner. The book bridges the gap between growing knowledge at the bench and current and future applications of biomarkers in clinical practice. The central structure of the book focuses on prognostic and predictive biomarkers in IBD with an emphasis on the fields of research and scientific techniques (genomics, proteomics and metabonomics) that have led to biomarker discovery and places these biomarkers within a clinical context to help understand their utility in clinical practice. This book will be of use to clinicians who have an interest in using biomarkers in clinical practice as well as clinician researchers and scientists involved in the biomarker research pipeline. The author team comprises experts from around the world in order to bring together the literature in an effort to inform clinicians and researchers about the current state-of-the art in biomarker discovery. It is intended to assist future research efforts and indicate how biomarkers might be best applied to clinical practice both at present and in the future.
