Available in PDF, EPUB and Kindle. Book excerpt: Acute otitis media (AOM) is the most common infection for which antibiotics are prescribed for children in the United States. Evidence based guidelines such as those developed by the American Academy of Pediatrics, and Cincinnati Children's Hospital Medical Center, help provide the most up-to-date care for children with AOM including assessment and treatment of pain. The purpose of this study was to determine if clinicians in primary care settings, were assessing and treating pain symptoms during the office visit. Charts (n=172) were reviewed for 24 pediatric providers. There were no validated assessment tools identified for pain assessment in this primary care setting. Forty eight percent of charts had documentation of pain symptoms, 52% did not. Nurse practitioners and pediatricians more often asked about behaviors affected by pain than did the general practitioners or physician assistants. Fifty one children or 29.7% of patients were treated for pain. Clinicians were more likely to make a recommendation of pain medication or treatment for patients where pain was documented (p = .003). Contrary to evidence based guidelines, 24% of patients received cold and allergy medications for AOM symptoms. Suggested future research projects may include quantifying pain responses with the use of validated assessment tools and clinician's adherence to evidence based practice guidelines.
