However, the risks and benefits of catch-up growth in preterm infants must be weighed, and evidence addressing this warrants examination. Perinatal Growth and Nutrition explores the reasons for EUGR and the long-term effects on developmental outcome and on metabolic risks. It provides clear information on the risks and benefits of faster post-natal growth and catch-up growth in preterm infants and offers tools for better assessment of growth and earlier identification of faltering growth. This book is divided into three sections. The first section covers advances in preterm infant growth standards, diagnosis and causes of EUGR, and assessments of preterm infant diets. The second section considers the extensive human literature on the effects of in utero and ex utero growth restriction and catch-up growth on long-term metabolic outcomes—such as obesity, insulin resistance, type 2 diabetes, and cardiac disease—and long-term neurodevelopmental outcomes including cognition. It also examines evidence for the effect of growth on these outcomes in term and preterm infants. The final section of the book considers ways to reduce the incidence of EUGR in preterm infants and when EUGR does occur, to optimize catch-up growth. Topics include assessment of dietary requirements of the diverse population of preterm infants, examination of tools for prescribing nutrition to neonatal intensive care unit patients, consideration of whether to customize or generalize nutrient intake, and fortification of human milk. In addition, the last chapter proposes using a Z-score growth chart for improved interpretation of growth data.

Book excerpt: This book’s main hypothesis is that Egypt’s large food subsidy system has been ineffective in reducing undernutrition; in fact, it may have contributed to sustaining and even aggravating both nutrition challenges. For a long time, the subsidy system provided only calorie-rich foods, at very low and constant prices and with quotas much above dietary recommendations. This system has created incentives to consume calorie-overladen and unbalanced diets, increasing the risks of child and maternal overnutrition and, at high subsidy levels, the risk of inadequate child nutrition. Moreover, the large public budget allocated to the food subsidies is unavailable for possibly more nutrition-beneficial spending, such as for child and maternal nutrition-specific interventions. The authors’ findings consistently suggest that—in addition to the well-known economic rationale for reforming the Egyptian food subsidy system—there are strong reasons to reform food subsidies due to nutrition and public health concerns. A fundamental food subsidy reform process has been under way since June 2014. The already-implemented changes can be expected to have reduced some incentives for overconsumption and may have positive dietary effects. However, further major reform efforts are needed to transform the current subsidy system into a key policy instrument in the fight against malnutrition. The findings of this book should be valuable to policy makers, analysts, development partners, and others concerned with improving food security and promoting healthy nutrition in Egypt and other developing countries with large social protection programs.

Using animal models, experimental research scientists have accumulated a wealth of infor mation and epidemiological studies of field workers have brought the animals and human studies together. Most times, there is an agreement on the results of animal and human experiences, but occasionally a voice of uncertainty is heard when results of animal studies are extrapolated to humans. After all, the human brain is far more complex than that of other species, and comparisons are not always accurate. Behavioral sci entists have attempted to correlate the findings of the biochemists and neurochemists to the "working" of the brain. Severe effects of malnutrition on body growth and function can usually be reversed by corrective procedures. But when such effects include the impact on brain development, the outcome can be devas tating. Underdevelopment of the brain caused by malnutrition during early life may not respond to corrective measures in later life. Undoubt edly this is a very controversial issue and the final verdict has not been reached. Unfortunately, even with today's tremendous technological innovations and applications, there are millions of people, including infants and children, who cannot obtain adequate food and are therefore exposed to damaging effects on the orderly development of the central vii PREFACE viii nervous system.
