Research has added to the knowledge regarding simulation development, evaluation, and implementation. The purpose of this quasi-experimental study was to add to the knowledge base regarding the influence of high-fidelity simulation on gain and retention of knowledge. Forty-four junior level baccalaureate nursing students participated in either a lecture only (n = 19) or lecture with simulation (n = 25) experience. The students were tested pre-intervention, post-intervention, and at 4 months utilizing a multiple choice knowledge evaluation test that was developed by the researchers. The results of this study demonstrate that although there was significant learning with the simulation and lecture experience over the lecture only experience (t = -3.39, df = 29, p = .002), there was not significant impact on the 4 month retention of knowledge between the groups (t = -.30, df = 42, p = .766). Simulation design was tested through the National League for Nursing simulation tools, including the: Simulation Design Scale, Educational Practices in Simulation Scale, and the Student Satisfaction and Self-Confidence in Learning Scales. The scenario was found to be well developed as compared to other pulished studies.

Thirty-eight first year associate degree nursing students enrolled in a medical-surgical course in a southeastern community college participated in a diabetes simulation as part of their course. Students then voluntarily completed a survey indicating self-confidence following the simulation. Descriptive statistics were used to determine the overall mean of student responses to each question on the National League for Nursing (NLN) Student Satisfaction and Self Confidence in Learning Tool. Students rated the teaching helpful and effective with a mean score of 4.47 (sd = .951). Students indicated that simulation materials used were motivating and helped with learning with a mean score of 4.53 (sd = .862). Participants indicated confidence in mastery of simulation content with a mean score of 3.63 (sd = 1.172). Participants rated confidence in development of skills and knowledge attainment with a mean score of 4.11 (sd = 1.06). Students rated knowledge of how to use simulation activities to learn nursing skills with a mean score of 4.18 (sd = .955). The study suggested confidence in mastery of skills and content covered and perception of simulation as helpful and effective occur concurrently following the simulation experience. Data analysis indicated student's knowledge of application of simulation activities and confidence in the development of needed skills from the simulation occur mutually.

These issues have led to increased use of high fidelity simulation (HFS) experiences to replace traditional clinical rotations (Benner, Sutphen, Leonard & Day, 2010; Lasater, 2007a; Orsolini-Hain & Malone, 2007). Previous research has shown that traditional clinical rotations are instrumental in development of clinical reasoning skills and HFS experiences are valuable in developing technical skills. However, it does not provide insight into the impact of HFS experiences on development of clinical reasoning skills (Lapkin, Levett-Jones, Bellchambers & Fernandez, 2010). The purpose of this quasi-experimental pilot study was to determine the relationship between HFS clinical experiences and development of clinical reasoning skills in nursing students. Findings will influence the use of HFS to replace traditional clinical experiences and provide a foundation for future research on the efficacy of HFS in development of clinical reasoning skills. Analysis of data showed that there was no significant statistical difference in development of clinical reasoning skills between prelicensure nursing students attending traditional clinical experiences and those participating in simulated clinical experiences. Book excerpt: It is imperative that registered nurses are skilled critical thinkers so that they are able to safely and effectively respond to the multifarious, complex, and often critical needs of their patients within a dynamic and constantly evolving healthcare environment. Nursing faculty are professionally and ethically bound to ensure graduates are workforce ready, and are therefore required to deliver learning opportunities that effectively and reliably facilitate the development of critical thinking skills. Simulation has been critically lauded in the literature as a learning strategy that supports critical thinking. However there is a paucity of empirical evidence to support these assertions, and little is known about 'how' simulation fosters the development of critical thinking within the context of undergraduate nursing. This phenomenological study sought to determine, from the perspective of undergraduate nursing students, the specific aspects of clinical simulation training that support or hinder the development of critical thinking. A convenience non-probability sample of 27 undergraduate Bachelor of Nursing students enrolled and completed two days of High-Fidelity Simulation training. Data was collected via participant demographic questionnaires and focus groups. Utilizing Colaizzi's (1978) phenomenological method of data analysis, verbatim transcriptions of the participant discourse during the focus group sessions were scrutinised for patterns and themes. Undergraduate nursing students identified that the impacts, both positive and negative, of simulation on their ability to think critically depend on the way they are prepared for the simulation; the simulation environment; the simulation scenario; the degree of authenticity; the opportunity to work in a team and take on an assigned role; and the opportunity to reflect and receive feedback. There was unanimous agreement amongst participants that the overall simulation experience supported development of cognitive and metacognitive skills by affording the opportunity to practice the process of critical thinking. This study presents information that will assist nursing faculty to develop and manage simulation experiences for undergraduate nursing students to support critical thinking.

Book excerpt: Interpersonal task-related conflict that arises within nursing practice can contribute to issues regarding patient safety. Exposure to conflict within practice occurs as soon as the undergraduate nursing student's first clinical placement. Yet studies have shown that students, as well as practitioners, are not utilizing a conflict management style conducive to safe patient care. Few educational interventions have incorporated the teaching of conflict resolution skills to nursing students. Therefore, a high fidelity simulation intervention (two scenarios, each followed by a debriefing session) was developed and refined to teach the two-challenge rule to Year 2 nursing students. The Medical Research Council Framework informed the intervention's design and refinement strategy, while Kolb's Experiential Learning Theory provided the foundation to implement the entire, refined intervention. A Phase 1 study with two successive components, refinement and single-group post-test, was conducted to determine the feasibility for a future randomized controlled trial (RCT) of this high fidelity simulation intervention. Forty eligible Year 3 nursing students participated in the refinement phase during a 2-week period. For the single-group post-test component, 35 Year 2 nursing students participated in the entire, refined high fidelity simulation intervention which included a 5-week spacing period. The refinement strategy improved the high fidelity simulation intervention's overall design features. For Scenario 1, the Simulation Design Scale (SDS) mean score reached a high on Day 5 of 4.86 (SD = 0.14) in contrast to a score of 3.45 (SD = 0.17) on Day 1. For Scenario 2, the SDS specific mean score was 4.75 (SD = 0.16) on Day 5, which represented a mean score increase of 1.01 from the score on Day 1. Of the Year 2 students who participated in the refined intervention, 88.7% rated its quality at 4 or higher on the Educational Practices Questionnaire. Year 2 nursing students (82.9%) also rated their satisfaction and self-confidence after participating in the entire refined intervention at 4 or higher on the Satisfaction and Self-Confidence in Learning scale. Additionally, the Year 2 students utilized the two-challenge rule 74.3% of the time after participating in the second high fidelity simulation scenario. Overall, the results of the Phase 1 study were favourable and will contribute to planning a future RCT.

This quantitative study examined the effects of high-fidelity simulation on senior nursing students perceived self-efficacy and perceived practice readiness. The sample (N=48) consisted of senior associate degree nursing students at a community college in the Southeast. The findings showed high-fidelity simulation had a positive effect on students perceived self-efficacy and perceived practice readiness at certain points in the study, while other times not demonstrating a positive correlation. Implications for use of simulation as a teaching strategy in nursing education and recommendations for future research were proposed.

High fidelity simulation is important to nurse clinical educators as an alternate method to provide experiences that assimilate, expand, and enhance their self-confidence. National League for Nursing states that nurses' self-confidence improves the performance level in life saving conditions (National League for Nursing, 2013). The problem statement of this Direct Practice Improvement (DPI) project was the following: It is not known to what extent using high fidelity simulation in adult cardiac life support (ACLS) renewal courses increase the self-confidence of nursing staff. The purpose of this quantitative project was to determine if high fidelity simulation increases nurses' confidence level while performing ACLS. The clinical question was the following: Does using high fidelity simulation in ACLS renewal classes increase nursing staff's confidence level and improve their performance at work? The theoretical foundation for this project incorporated Benner's Novice to Expert model. The design of this DPI project was a pre- and post- survey. A paired t-test was used to analyze the data. The population was 64 registered nurses who self-enrolled for an ACLS class using simulation. Statistical findings of this project suggest that high fidelity simulation increases the self-confidence level of the nurses.
