Healthcare organizations have recognized the advantages of electronic documentation, such as legibility, organized data, and the simultaneous use of the record by multiple caregivers and the elimination of paper records. In keeping with a common response to change, nurses can be very resistant to relinquishing the traditional paper records. By collecting the nurses' perceptions of electronic documentation, information will be generated on a pivotal point in the success or failure of implementing and using electronic documentation. The framework from Davis's 1986 Technology Acceptance Model uses perceived usefulness of the system and ease of use as ways of determining if nurses will use a system or not. The study population was 230 licensed vocational nurses and registered nurses who used electronic documentation and delivered direct patient care in the Women's Services department of a local hospital. The tool used to measure the nurses' perceptions was the Nurses Attitudes towards Computerization Questionnaire by Stronge and Brodt. The results of the study indicate that 52% of nurses feel that the Women's Services department of a local hospital. The results of the study indicate that 52% of nurses feel that computers made their job easier and that they had more time for professional tasks. The results also indicated that 64% of the nurses believed that computers offered a remarkable opportunity to improve patient care and that computers decrease repeated documentation. The majority of nurses (56%) disagreed with the idea that computers increase healthcare costs and that their use increased nurses' workloads.

The need for healthcare data is increasing both from within and outside healthcare organizations. It is becoming more difficult to meet demands with paper records. Electronic documentation improves healthcare quality, safety, and efficiency. Electronic records are legible, organized, complete, and accessible by multiple users. Nurses can be resistant to using computers. Therefore, it is important to assess nurses' perceptions of ECD. The Technology Acceptance Model (Davis, 1986) is the organizing framework which suggests that perceived usefulness and ease of use determine an individual's intention to use the system. The study population was 190 registered nurses (RNs) who use ECD and provide direct patient care in inpatient settings within two Midwest hospitals. Nurses Attitudes Towards Computerization Questionnaire (Stronge & Brodt, 1985) was used to measure nurses attitudes toward computerization. The results of the study show how ECD impacts RNS in terms of time saving, reduced paperwork, job satisfaction, patient care, and benefits to the organization. More than 50% believe ECD allows RNs more time for professional tasks and improves communication between departments. However, more than 56% believe they spend more time on computers than they spend with patients. About 52% think ECD is redundant and does not save repeated documentation. It was not conclusive if ECD reduces or increases health care costs.

Findings of a literature review suggest that usability (ease of use, functionality, navigation and impact on workload), the organizational context (support from leadership, level of training, level of on-going support, physical environment and implementation process) and individual nurse characteristics (sex, age, nursing unit, years of experience as a registered nurse, country of nursing education, years of experience using an EHR, previous EHR use and formal informatics training) influence nurses' use of these systems. Thus, the purpose of this doctoral research was to better understand the relationships between the variables that make up usability, organizational context, individual nurse characteristics, and nurses' perceptions of EHR use. This study was conducted using a sequential mixed methods design with two phases. Phase One consisted of a cross sectional survey that was piloted and then administered to nurses in an acute care teaching hospital in Toronto, Canada. The aim of the survey was to obtain information about nurses' perceptions of the usability of the EHR, the organizational context, their individual nurse characteristics and their use of the system. Phase two involved focus groups to better understand the findings identified in the survey. Multivariable and hierarchical linear regression was conducted. A multivariable model made up of the variables ease of use, navigation and impact on workload, explained 13% of the variance in nurses' perceptions of EHR use, however navigation was the only significant predictor in the model. In the data from the focus groups, nurses described how they navigated through the EHR, and which functionalities supported or hindered their use of it. Results of this study provide insights into factors that may influence nurses' use of EHRs in an acute care hospital setting that have implications for research, nurse leaders, vendors, healthcare settings and nursing practice.

Book excerpt: ABSTRACT The purpose of this research was to explore nurses' perceptions of the effectiveness of nursing documentation of patient status during a clinical event when using electronic documentation with or without embedded standardized languages. The theoretical framework for this study was based on principles of information theory. This study was significant in two very important ways; first, in contrast to prior studies, the perceptions of nurses were focused on the documentation of a clinical event. Second, this study explored the nurses' opinions about the strengths and limitations of using structured languages (specifically, the North American Nursing Diagnosis Association (NANDA), the Nursing Intervention Classification (NIC), and the Nursing Outcomes Classification (NOC)) for telling the patient's story during a clinical event, as well as collecting nurses' suggestions for improving electronic documentation. Semi-structured interviews of 37 nurses were conducted in two acute care hospitals. Both hospitals used electronic documentation, but only one used embedded standardized nursing languages. Half the interviewees were asked questions from the perspective of the nurse documenting a clinical event; half were asked questions from the perspective of a nurse reviewing another nurse's documentation of a clinical event. Recorded interviews were transcribed, and the transcripts analyzed using qualitative content analysis. A panel of judges was used to establish reliability of the coding scheme. The results showed that nurses perceived aspects of three categories (usability, legibility, and communication) as strengths of the documentation system. Nurses perceived aspects of three categories (usability, communication, and workarounds) as limitations of the documentation system. Potential solutions to improve the documentation system were defined related to three categories (usability, communication, and collaboration). Usability was perceived by the nurses as a strength of the electronic documentation with embedded nursing languages. Usability of the electronic documentation system with nursing languages was also perceived as a limitation. Improving language usability was identified as a potential solution to improve the electronic documentation system with embedded nursing languages.

This qualitative study used a four-question interview tool to identify nursing perceptions with using the current EHR and their perceptions with implementing a new EHR. This field study was conducted at a mid-Michigan medical center in support of their transition to a new EHR in the near future. Lewin's Change Theory was adopted for this study to identify the driving and restraining forces based on themes obtained from nursing interviews. The overall perceptions revealed that most of the nurses were open to using a new EHR, although the implementation phase left them with many concerns. The concerns centered around not getting adequate technical support, lack of communication, and not being able to give their input with design functions of the system. Taking Action Against Clinician Burnout: A Systems Approach to Professional Well-Being builds upon two groundbreaking reports from the past twenty years, To Err Is Human: Building a Safer Health System and Crossing the Quality Chasm: A New Health System for the 21st Century, which both called attention to the issues around patient safety and quality of care. This report explores the extent, consequences, and contributing factors of clinician burnout and provides a framework for a systems approach to clinician burnout and professional well-being, a research agenda to advance clinician well-being, and recommendations for the field.
