Stomach cancer represents roughly 2 percent (25,500) cases of all new cancer cases yearly in the United States, but it is much more common in Japan, Great Britain, South America, and Iceland. It is associated with high salt in the diet, smoking, and low intake of fruits and vegetables.Infection with H. pylori is the main risk factor in about 80 percent or more of gastric cancers. It is more common in men. A very small percentage of diffuse-type gastric cancers are thought to be genetic. Hereditary Diffuse Gastric Cancer (HDGC) has only recently been identified and research is ongoing, but genetic testing and treatment options are available for families at risk. Metastasis occurs in 80-90 percent of individuals with stomach cancer, with a five year survival rate of 75 percent in those diagnosed in early stages and less than 30 percent of those diagnosed in late stages. Book excerpt: Despite the fact that the incidence of gastric cancer is declining in the Western world, it remains a significant problem with respect to accurate diagnosis and treatment since it has a high mortality rate. In June 1989 an International Conference was held at the University of Rome "La Sapienza" entitled "New Trends in Gastric Cancer: Background and videosurgery". During this meeting background information on the aetiopathogenesis of gastric cancer was presented together with talks and video presentations on the latest advances in the treatment of gastric carcinoma, both from the European and Japanese experience. Because of the poor prognosis of gastric carcinoma there is increasing pressure for early detection. Some of the problems in the early detection of gastric carcinoma are discussed together with methods of surveillance of high-risk subjects. It is generally accepted that the surgical approach to gastric carcinoma should take into account the site and extent of the lesion and there are chapters on new methods for pre and intraoperative staging of the disease which allow a more logical approach to surgery. A comparison between Japanese and Western rule and results was attempted and reasons for the differences were investigated. Since the field is still evolving not all aspects could be covered, and those angles not approached in this book will be addressed in a second International Conference to be held in Rome in June 1990.

In areas with higher incidences, the decline has been less obvious and slower. Nevertheless, gastric adenocarcinoma remains one of the most common causes of cancer-related death worldwide. The poor outcome has been attributed to late detection of the condition, particularly in Americas and Europe, aggressive pathogenesis and lack of symptoms during early stages of the tumor development. In addition, sporadic stomach cancer mostly affects elderly individuals. In the majority of countries with low incidence, the average age at the disease presentation is above 65. Therefore, gastric adenocarcinoma, among other diseases associated with old age, raises health concerns in countries with changing demographic age profiles that show a trend of an increase in the proportion of the population aged over 60. The low 5-year survival rate of patients underscores the critical need for the development of more accurate diagnostic tools and safe targeted chemotherapeutics. However, the heterogeneity of molecular changes represents one of the most pressing issues in the current research of gastric cancer, impeding the translation of genetic aberrations into novel applications for medical practice.

Book excerpt: The digestive organs are the most frequent site of cancer in the world, accounting for approximately 30% of all malignant tumors. This prominent position has been present for many decades in spite of marked shifts in the freq uency of cancer of specific organs. The most remarkable shift has been the decrease in gastric cancer rates occurring concomitantly with an increase in colon cancer rates in most 'western' industrialized societies. Important exceptions to this rule, as well as other epidemiologic evidence, indicate that the opposite trends for gastric and colon cancers are not inevitable consequences of each other. Although genetic ally determined precancerous syndromes are well recognized, it is generally agreed that environmental factors play an overriding role in digestive cancer causation. The most obvious environmental factors seem to be the result of what we eat, drink, or smoke. Although the nutritional component of the diet is of unquestionable importance, the nonnutrient elements in our diet have also proven to be influential causative factors. Several studies have focused on the microenvironment at the level of the mucosa or the digestive organs as a micro cosmos where forces promoting and inhibiting carcinogenesis are operating for a prolonged period of time. Their interaction eventually determines the presence or absence of a malignant tumor. Our understanding of such modulating forces, hopefully, will someday allow us to modify the microenvironment in a favorable way and attain the goal of cancer prevention.
