Book excerpt: Developing novel and more effective treatments that improve quality of life for individuals with autism spectrum disorders is urgently needed. To date a wide range of behavioral interventions have been shown to be safe and effective for improving language and cognition and adaptive behavior in children and adolescents with ASD. However many people with ASD can receive additional benefit from targeted pharmacological interventions. One of the major drawback in setting up therapeutics intervention is the remarkable individual differences found across individuals with ASD. As a matter of fact the medications that are currently available address only symptoms associated with ASD and not the core domains of social and communication dysfunction. The pathogenesis paradigm shift of ASD towards synaptic abnormalities moved the research to pathway to disease that involve multiple systems and that are becoming the forefront of ASD treatment and are pointing toward the development of new targeted treatments. Some new therapeutics have been tested and others are being studied. In this context single gene disorders frequently associated with ASD such as Rett Syndrome, Fragile X and Tuberous Sclerosis have been of significant aid as neurobiology of these disorders is more clear and has a potential to shed light on the altered signaling in ASD. However much research is needed to further understand the basic mechanisms of disease and the relationship to idiopathic ASD. Clinical trials in children are underway with agents directed to core symptoms and to the associated disorders in the search of new therapeutics and progress are expected with possible new option for therapeutics in ASD in the upcoming future. Children and Adolescents with ASD and their families can provide important information about their experience with new treatments and this should be a priority for future research. In addition, research performed on genetic mouse models of ASD will keep on providing useful information on the molecular pathways dysrupted in the disease, thus contributing to identify novel drug targets.

Although many consider the etiology to be unknown, the majority of professionals working in the field of autism and parents of children with autism consider autism to be a brain disorder. As such, most research has emphasized the study of the brain. From this perspective, the clinical work with this population focuses primarily on techniques such as Applied Behavioral Analysis, sensory integration, modeling, and social skills development. The work done thus far with this population should be commended and not discounted. We are now ready to augment the present state-of-the-art work with this population by introducing Attachment Relational Therapy (ART). This therapy is similar to DIR(r)/Floortime(tm) and other relationally based therapies in that it emphasizes the relationship between the child and the therapist or caregiver. The major difference is that Attachment Relational Therapy introduces a treatment process (plan) that at its core is based on understanding the etiology of autism. It is author Karen Savlov's belief that the cause of autism is due to an incomplete attachment, which occurs within the first two to three years of life. Once the etiology is understood through this new lens, then the therapist or parent can understand how to engage with the child. From this alternative perspective, it is Savlov's belief that children on the spectrum have not had the advantage of a completed attachment. She calls this perspective Incomplete Attachment, and it is her belief that what one sees when observing children on the spectrum is a child who is waiting for the attachment process to be completed. The child is doing the best he/she can to cope with this predicament. All the behaviors such as flapping arms, nonverbal communication, echolalia, lack of responsiveness to others or inability to communicate one's needs, can all make sense when taken from this perspective.The author suggests that at the core of autism is the child's inability to use herself in relationship to others. This lack of a sense of self-agency evolves during the attachment phase of development and Savlov hypothesizes that the child is left in a waiting position and remains in this incomplete state of attachment until the caregiver and/or therapist understands this predicament and can respond appropriately. Within these pages, she hopes to illustrate how the child with autism can be mobilized to utilize herself through the use of understanding (empathy), acceptance, recognition, and validation.The writings contained in this volume will go into detail about this viewpoint and how one works with children, adolescents, and adults from this perspective. It is Savlov's belief that Autism Spectrum Disorders can also inform our understanding of psychological development in general and specifically Theory of Mind. Indeed they are often used in clever ways to facilitate daily routines. We have waited much too long in science to listen to the very people that we are trying to define, understand and help. Being autism a social problem by definition, it is remarkable that not a single diagnosis inventory measures the dyadic social interaction that takes place between the examiner and the examinees. Indeed we have conceived the autistic person within a social context where we are incapable –by definition– of accepting those differences. The burden is rather placed on the affected person to whom much too often we refer to in the third person as “non-verbal, without intentionality, without empathy or emotions, without a theory of mind”, among other purely psychological guesses. It is then too easy and shockingly allowed to “reshape” that person, to mold that person to better conform to our social expectations and to extinguish “behaviors” that are socially unacceptable, even through the use of aversive punishing reinforcement techniques if need be. And yet none of those techniques have had a single shred of objective scientific evidence of their effectiveness. We have not objectively measured once, nor have we physiologically characterized once any of those perceived features that we so often use to observationally define what we may think the autistic phenotype may be. We have not properly quantified, beyond paper-and-pencil methods, the effectiveness of interventions in autism. Let us not forget when we do our science, that we are all part of the broad human spectrum.
