Book excerpt: The Consolidated guidelines on HIV, viral hepatitis and STI prevention, diagnosis, treatment and care for key populations outline a public health response for five key populations (men who have sex with men, trans and gender diverse people, sex workers, people who inject drugs and people in prisons and other closed settings). They present and discuss new recommendations and consolidate a range of recommendations and guidance from current WHO guidelines. Particularly for key populations, social, legal, structural and other contextual factors both increase vulnerability to HIV, viral hepatitis and STIs and obstruct access to health and other essential services. These guidelines highlight the critical importance of addressing structural barriers in all settings as a priority. In most countries, inadequate coverage and poor quality of services for key populations continue to undermine responses to HIV, viral hepatitis and STIs. All countries should prioritise reaching key populations and supporting key population communities to lead the response and provide equitable, accessible and acceptable services. The 2024 HBV guidelines provide updated evidence-informed recommendations on key priority topics. These include expanded and simplified treatment criteria for adults but now also for adolescents; expanded eligibility for antiviral prophylaxis for pregnant women to prevent mother-to-child transmission of HBV; improving HBV diagnostics through use of point-of-care HBV DNA viral load and reflex approaches to HBV DNA testing; who to test and how to test for HDV infection; and approaches to promote delivery of high-quality HBV services, including strategies to promote adherence to long-term antiviral therapy and retention in care. The 2024 guidelines include 11 updated chapters with new recommendations and also update existing chapters without new recommendations, such as those on treatment monitoring and surveillance for liver cancer.

Book excerpt: he starting point for this guideline is the point at which a woman has learnt that she is living with HIV and it therefore covers key issues for providing comprehensive sexual and reproductive health and rights-related services and support for women living with HIV. As women living with HIV face unique challenges and human rights violations related to their sexuality and reproduction within their families and communities as well as from the health-care institutions where they seek care particular emphasis is placed on the creation of an enabling environment to support more effective health interventions and better health outcomes. This guideline is meant to help countries to more effectively and efficiently plan develop and monitor programmes and services that promote gender equality and human rights and hence are more acceptable and appropriate for women living with HIV taking into account the national and local epidemiological context. It discusses implementation issues that health interventions and service delivery must address to achieve gender equality and support human rights.

Book excerpt: Despite effective approaches to prevention, STD and HIV infection rates remain fairly constant. Targeting, implementation, and monitoring of interventions have posed widespread problems, and the recent spate of cuts to prevention budgets has made these roadblocks even more challenging. It is clear that working in sexual health requires both a deeper understanding of STI/HIV epidemiology and an ongoing quest for up-to-date, realistic prevention strategies. The New Public Health and STD/HIV Prevention offers readers leading-edge access to both. Focusing on social determinants of sexual health, at-risk populations, critical factors in approaches to prevention, and reviews of new research, this authoritative volume explores areas as varied as HPV prevention, technology-based interventions, migration as a factor in disease transmission, and competencies key to effective leadership in the field. Dispatches from the frontlines of theory, research, and practice in the U.S. and abroad include: Personal risk, public impact: balancing individual rights and STD/HIV prevention. Distribution of prevention resources and its impact on sexual health. Prevention measures in diverse populations of women. Toward a better approach to preventive interventions with men who have sex with men. Adolescent sexual health and STIs. Reducing disparities in sexual health: lessons from the campaign to eliminate infectious syphilis. Public health professionals of all backgrounds interested in or working in improving sexual health will find The New Public Health and STD/HIV Prevention an indispensable guide to conceptualizing the problems and clarifying possible solutions.

Book excerpt: Although efforts have been made and continue to be made to reduce the rate of HIV transmission in the U.S. and globally, the rates continue to increase in the majority of countries. In the U.S., members of minority communities remain especially at risk of HIV transmission. An individual’s discovery that he or she has contracted HIV, or that a loved one has contracted the illness, often raises significant issues that necessitate interaction with mental health professionals. Mental Health Practitioner’s Guide to HIV/AIDS serves as a quick desk reference for professionals who may be less familiar with the terminology used in HIV/AIDS care and services.

Available in PDF, EPUB and Kindle. Book excerpt: HIV Treatment as Prevention: Primer for Behavior-Based Implementation provides the first practical guide to integrating behavioral prevention with antiretroviral therapies for people living with HIV infection. This brief book discusses the historical and social context embedding the shifting landscape in HIV prevention, where the use of effective treatments have become the focus of HIV prevention. While using treatments for prevention is promising, the history of HIV prevention offers several important pitfalls that must be avoided if HIV treatments are to ultimately succeed in preventing new HIV infections. Lessons learned from the successes and failures of other biomedical technologies used in HIV prevention, specifically syringes, condoms, and HIV testing are critical to the success of using HIV treatments for prevention. HIV Treatment as Prevention: Primer for Behavior-Based Implementation summarizes the scientific evidence for advancing the use of antiretroviral therapies for HIV prevention. The evidence makes clear that HIV treatments can prevent HIV transmission, but will fail if behavioral aspects of treatment and HIV transmission are ignored. Of greatest concern are medication adherence and risks for contracting other sexually transmitted infections. Placing HIV treatment within the context of behavioral interventions for maintaining medication adherence and reducing sexual risk behaviors is therefore essential to the future of HIV prevention. HIV Treatment as Prevention: Primer for Behavior-Based Implementation highlights two pioneering behavioral interventions aimed at maximizing the effects of antiretroviral therapies for preventing HIV transmission. One of the interventions, developed by the Author’s research team, is discussed in detail and the intervention manual is included as an Appendix.

This volume's unique multidisciplinary biopsychosocial approach combines relevant medical information regarding disease states with the cultural, social, and psychological facets of successful prevention programs. Becker and Rankin outline specific interventions that address the needs of particularly vulnerable populations: women, ethnic minorities, and the gay community. Useful chapter summaries which review the pros and cons of different approaches will aid both the student and the practitioner.

This book was released on 2021-12-24 with total page 750 pages. Available in PDF, EPUB and Kindle. Book excerpt: One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

The spread of new pathogens and the threat of antimicrobial resistance pose particular challenges in combating these diseases. Major Infectious Diseases identifies feasible, cost-effective packages of interventions and strategies across delivery platforms to prevent and treat HIV/AIDS, other sexually transmitted infections, tuberculosis, malaria, adult febrile illness, viral hepatitis, and neglected tropical diseases. The volume emphasizes the need to effectively address emerging antimicrobial resistance, strengthen health systems, and increase access to care. The attainable goals are to reduce incidence, develop innovative approaches, and optimize existing tools in resource-constrained settings.
