Book excerpt: Asthma, allergy and chronic obstructive lung disease are common throughout the world and are increasing in incidence, particularly in the developing world. This volume provides a state-of-the-art account of the identification of new targets and the development of new therapies for these conditions. Some 40 chapters by clinical academics and senior members of the pharmaceutical industry detail the latest breakthroughs in research and development. In asthma, a promising approach is the use of therapy directed against specific Th2 responses through biological antagonists of IL-5, IL-4 and IL-13. There have also been major advances in our understanding of innate immune responses to pathogen-associated molecular patterns, and in the area of Toll-like receptors. Up to date and comprehensive, this book will be of particular relevance to those working in the pharmaceutical industry (in preclinical research and clinical development), to academic researchers in the field of respiratory medicine, and to respiratory health care specialists.

Available in PDF, EPUB and Kindle. Book excerpt: Readers will be aware that asthma is one of the commonest chronic diseases of industrialised societies, with an increase in morbidity and mortality, despite the availability of a range of different drugs. In an effort to improve the treatment of asthmatic patients, international guidelines have recently recognised asthma as an inflammatory disease, and there has been a change in emphasis from bronchodilator drugs towards earlier use of anti-inflammatory treatments. This volume brings together the latest developments in our understanding of the mechanisms of asthma, considers the safety ofbeta-adrenoceptor agonists, and explores latest developments in the use of phosphodiesterase (PDE) inhibitors in asthma therapy. The second international conference on "New Drugs in Allergy and Asthma" was held in Davos on 8-10 July 1992. There were six sessions with specialist chairmen: PDE isoenzymes (c. D. Nicholson), atopy and the environment (8. Villiger), adhesion molecules (R. H. Gundel), cytokines (M. Baggiolini), airway hyperreactivity (S. Makino) and neurotransmitters (J. G. Widdicombe). The success of the meeting was ensured by a high standard of presentation, with extensive reference to recent experimental data. The chairmen were instrumental in fostering stimulating discussion periods, that considerably enlivened the meeting. Particular thanks are due to the Swiss Institute for Asthma and Allergy Research (SIAF) and also to the e1inicians of Davos for their assistance and hospitality. In addition, a number of pharmaceutical companies were kind enough to support this meeting that was devoted to academic aspects of drug discovery. John Morley FRCPath.

Book excerpt: Chronic Obstructive Pulmonary Disease (COPD) is a progressive, largely irreversible lung condition characterised by airflow obstruction. Although cigarette smoking is the single most important risk factor in its development, other associations and risk factors are thought to have increasing relevance throughout the world. COPD is usually managed in primary care, although it is commonly under-diagnosed, and is one of the most common medical conditions necessitating admission to hospital. The second edition of the ABC of COPD provides the entire multidisciplinary team with a reliable, up-to-date and accessible account of COPD. Extensively updated by experienced clinicians - including new chapters on spirometry, inhalers, oxygen, death, dying and end of life issues - this ABC is an authoritative and practical guide for general practitioners, practice nurses, specialist nurses, medical students, paramedical staff, junior doctors, non-specialist doctors and all other health professionals working in both primary and secondary care.

In the last 10 years, substantial progress has been made in terms of understanding some of the mechanisms that drive severe asthma; there have also been concomitant advances in the recognition of specific molecular phenotypes. This ERS Monograph covers all aspects of severe asthma – epidemiology, diagnosis, mechanisms, treatment and management – but has a particular focus on recent understanding of mechanistic heterogeneity based on an analytic approach using various ‘omics platforms applied to clinically well-defined asthma cohorts. How these advances have led to improved management targets is also emphasised. This book brings together the clinical and scientific expertise of those from around the world who are collaborating to solve the problem of severe asthma.

But that does not mean it is easy to understand or manage. In everyday practice, pulmonologists face areas of controversy in COPD, for which evidence-based medicine is often unavailable. This ERS Monograph considers where the current controversies in COPD lie, discussing areas such as screening, premature birth, asthma–COPD overlap syndrome, treatment, rehabilitation and palliative care. This book will be of great interest to both clinicians and scientists, and aims to stimulate further discussion about this diverse and fascinating disease. "...contains a vast amount of information on the disease, its prevalence, signs and symptoms, diagnositc tests and treatment options. The book's format makes it quick and simple to find out what you need to know, and its size would make it easy to take to work for use in practice [...] invaluable for anyone working with patients with the disease." Even if the steps you take don't cure the problem, they can relieve symptoms and allow you to go about your daily life, or at least help you until you're able to see a doctor. Some remedies, such as changing your diet to deal with heartburn or adapting your home environment to cope with chronic pain, may seem like common sense. You may have questions about when to apply heat or cold to injuries, what helps relieve the itch of an insect bite, or whether certain herbs, vitamins or minerals are really effective against the common cold or insomnia. You'll find these answers and more in Mayo Clinic Book of Home Remedies. In situations involving your health or the health of your family, the same questions typically arise: What actions can I take that are immediate, safe and effective? When should I contact my doctor? What symptoms signal an emergency? Mayo Clinic Book of Home Remedies clearly defines these questions with regard to your health concerns and guides you to choose the appropriate and most effective response.

Meanwhile, the practice of anesthesiology has expanded to the preoperative environment and numerous locations outside the OR. Anesthetic Pharmacology: Basic Principles and Clinical Practice, 2nd edition, is an outstanding therapeutic resource in anesthesia and critical care: Section 1 introduces the principles of drug action, Section 2 presents the molecular, cellular and integrated physiology of the target organ/functional system and Section 3 reviews the pharmacology and toxicology of anesthetic drugs. The new Section 4, Therapeutics of Clinical Practice, provides integrated and comparative pharmacology and the practical application of drugs in daily clinical practice. Edited by three highly acclaimed academic anesthetic pharmacologists, with contributions from an international team of experts, and illustrated in full colour, this is a sophisticated, user-friendly resource for all practitioners providing care in the perioperative period.
