Book excerpt: Biocompatible and corrosion resistant regarding biological fluids, titanium is still an inert material: it does not participate actively at the tissue-integration around the implant. Surface modification of titanium at nanoscale can promote cell adhesion and differentiation by modulate genes expression due to a phenomenon of mechano-transduction. In this thesis, we have focused on the development, the characterization and the direct application of our nanostructured surface of medical devices. First of all, our study focused on the preparation and physicochemical characterization. After obtaining reproducible surfaces on small samples, our research has focused on the biological characterization of the surface. In-vivo studies conducted in rabbits have allowed us to show similar biomechanical attachment and good osseointegration of the nanostructured surface compared to commonly used surfaces on the market. The application of this new surface on more complex titanium implant such as tracheal prosthesis, has allowed us to observe delamination phenomenon of the nanostructure layer. Therefore, our research was oriented towards the problem of mechanical strength of the surface with the realization of nano scratch test and tribology. A topic in the zeitgeist, as a new EU regulation for medical devices which incorporate nanomaterials will take effect in 2017. To conclude, this work enable to propose a nanosurface with promising results of tissues integration required for medical device.

We have developed a new manufacturing protocol that enables integration of these bioactive materials in a titanium scaffold, overcoming the mechanical limitations of coatings. However, the biological behavior of these new materials is not yet described. Objectives: The aim of this study was to evaluate the behavior of human fetal osteoblasts and human gingival fibroblasts in contact with Titanium (Ti), Ti with 5% hydroxyapatite (TiHA) or Ti with 5% beta tricalcium phosphate (Ti-bTCP). Materials and methods: Ti, TiHA and Ti-bTCP discs were produced with homogeneous surface roughness. Human fetal osteoblasts and human gingival fibroblasts were cultured on discs for 14 days using previously established methods. Morphology and cellular adhesion were analyzed using scanning electron microscopy (SEM) after 1 and 7 days. Cell viability and proliferation were evaluated at pre-defined time-points (1,3,7 and 14 days) using a commercial resazurin-based method. Alkaline phosphatase (ALP) activity was evaluated at 7 and 14 days for osteoblasts. All results were presented as mean fluorescence intensity (AU) u00b1 standard deviation (SD). In this study, we modified titanium surfaces through hydrothermal treatment with a mixed solution of phytic acid and calcium hydroxide, then evaluated their bioactive and osteogenic activities. In vitro tests, calcium-decorated titanium surface enhanced the cell adhesion, proliferation and osteogenic differentiation of human bone marrow mesenchymal stem cells (hBMSCs). In vivo tests, Micro- CT and histological evaluations indicated that the modified implants promoted new bone formation and osseointegration between the implants and the host bone. These findings provided evidence that this simple modification would be promising solution to improve bioactivity and osseointegration, which would be beneficial not only for titanium implants but also other implanted biomaterials.Background: Titanium (Ti) or titanium alloys coated with various bioactive calcium phosphates (CaPs), including hydroxyapatite (HA) and u03b2-tricalcium phosphate (u03b2-TCP), are considered as satisfactory implant materials. CaP coatings provide nanostructural or microstructuctural characteristics on titanium or titanium alloys, thereby increasing the adhesion of osteoblasts, which enhance the osseointegration and shorten the healing time.Aim: To prepare and evaluate the effect of calcium-containing chemical conversion coating on promoting new bone formation and early osseointegration in vivo.Material and methods: We designed and prepared a phytic acid/calcium hydroxide composite coating on the pure titanium implant (Ti-SLA-2/10) through hydrothermal treatment with a mixed solution of phytic acid (PA) and saturated calcium hydroxide solution [Ca (OH)2]. The pure titanium implant with a sandblasted/acid-etched (SLA) surface was used as the control group (Ti-SLA). The surface morphology, elemental composition, surface roughness, wettability, and calcium ion release were investigated separately. Twenty-four SLA and calcium-containing SLA implants were immediately inserted into the mandibles of twelve New Zealand white rabbits. The biological effects were evaluated by Micro-computed tomography (Micro-CT) analysis and histological evaluation after 1, 2, and 4 weeks of healing. Results: Phytic acid/calcium hydroxide composite coating with superior hydrophilicity were successfully prepared on titanium implants. The surface characteristics showed Ti-SLA-2/10 implants with multiple pits and craters can release calcium ions continuously. In vivo, the volume of new bone around the Ti-SLA-2/10 implants was significantly higher than that of the Ti-SLA implants, but more fibrous tissue was observed surround the Ti-SLA implants after 1 week of healing. However, although stable, the conventional titanium implants are not very bioactive which in some instances could reduce their efficacy. This thesis described the research progress of using polymeric ultrafine-particles with bioactive additives to enrich the surface of titanium substrate, thereby increasing the biocompatibility and osteo-inductivity of the biomaterial. During this doctoral project, three different types of polymers were examined, initially the conventional polyester, and later the novel epoxy as well as the epoxy/polyester hybrid polymers. Physical characterizations confirmed that all of the coating powders were ultrafine particles, and homogeneous surfaces were constructed from these particles with the correct incorporation of the functional and flow additives. The biocompatibility of the enriched surfaces were confirmed after examining their water contact angles, cell attachment/proliferation abilities, and their mitochondrial activities. After proliferation and differentiation, the osteo-inductivity of the surfaces were studied by labelling the mineral deposits that were formed on the surfaces. In addition to the biological performance, the adhesive strength of the enrichment layer was also assessed. After comparing between calcium oxide and calcium phosphate as the bioactive additive, and a series of different epoxy and polyester based polymers in the presence/absence of micron-sized TiO2, the results showed that the calcium oxide and micron-sized TiO2 containing epoxy/polyester hybrid surface was the best candidate for future clinical in vivo studies. From this study, a better understanding in the factors that affect the biocompatibility and osteo-inductivity of titanium substrate were obtained in order to create more effective biomaterials.
