Visual inspection revealed several deficiencies in this structure. The following items were noted: There are three depressions and an animal burrow on the crest of the dam; Trees and brush are growing on the upstream and downstream face of the dam; The spillway discharge channel has trees, boulders, and natural ground which restricts flow over the spillway weir; A 3-foot diameter culvert in the discharge channel has the downstream end higher than the upstream end; There are tire ruts 0.4 foot deep on the crest of the dam; and There is a small amount of spalling and deterioration of the low section of the concrete weir.

It has been determined that the dam would be overtopped for all storms exceeding approximately 23 percent of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged 'seriously inadequate', and the dam is assessed as unsafe, non-emergency. It is recommended that, within three months of owner notification, detailed hydrologic and hydraulic investigations of the structure should be undertaken to more accurately determine the site-specific characteristics of the watershed and their effects upon the overtopping potential of the dam. The results of this investigation and analyses will determine the appropriate remedial measures required to achieve a spillway capacity adequate to discharge the outflow from at least the 1/2 PMF. In the interim, a detailed emergency action plan must be developed and implemented during periods of unusually heavy precipitation. Also, around-the-clock surveillance must be provided during these periods.

The spillway is therefore, adjudged as seriously inadequate and the dam is assessed as unsafe, non-emergency. The classification of 'unsafe' applied to a dam because of a seriously inadequate spillway is not meant to connote the same degree of emergency as would be associated with an 'unsafe' classification applied for a structural deficiency. It does mean, however, that based on an initial screening, and preliminary computations, there appears to be a serious deficiency in spillway capacity so that if a severe storm were to occur, overtopping and failure of the dam would take place, significantly increasing the hazard to loss of life downstream from the dam.

The maximum spillway discharge capacity is 7.9 percent of the PMF peak outflow. The spillway is, therefore, adjudged as 'inadequate'. The following remedial measures should be performed within 12 months from notification: (1) Missing portions of the upstream stone masonry wall should be repaired. (2) Brush and shrubbery should be removed from the embankment, spillway-reservoir drain structure and downstream channel. Provide a program of periodic cutting and or mowing of the dam surfaces, spillway-reservoir drain structure and the downstream channel. (3) The collected debris at the spillway-reservoir drain structure and in the downstream channel should be removed and hauled away from the dam. (4) A program of periodic inspections and maintenance of the dam and appurtenances should be provided, including yearly operation and lubrication of the reservoir drain and its control facilities. This information should be documented for future reference. (5) An emergency action plan for the project should be developed.

Book excerpt: Helderberg Lake Dam consists of a 285 foot long earth embankment with concrete cutoff wall and a 55 foot wide concrete channel spillway. The maximum height above the original stream bed is approximately 35 feet. Both the upstream and downstream slopes of the earth embankment are 1 vertical to 2 horizontal, and the crest width is 12 feet. The dam was reconstructed in 1944 after a failure of the original structure in 1938. The spillway crest was increased in length from 35 feet to 55 feet and lowered 2 feet. The original cutoff wall which is founded on bedrock was lengthened and increased in height by 2.25 feet. A 20 in. drain pipe was installed and incorporated into the tunnel that was in the original dam. The examination of documents and visual inspection of Helderberg Lake Dam and appurtenant structures did not reveal conditions which constitute a hazard to human life or property. The discharge capacity of the spillway is inadequate for all storms in excess of 50% of the PMF (Probable Maximum Flood).
