Book excerpt: A visual inspection of this dam and the engineering analyses performed revealed that there are a number of structural deficiencies on this structure. The structural stability analyses indicate that the factors of safety against both overturning and sliding are less than desireable. When the dam is subjected to severe loading conditions (ice load, flood flows), the safety factors fall to critical levels. Further investigation of the stability is needed including subsurface investigations and concrete coring. This information should then be incorporated into a detailed stability evaluation. Appropriate modifications to the dam should then be made. It is recommended that within 6 months of the date of notification of the owner these investigations should be commenced, within 18 months, necessary modifications to improve the stability of the structure should be completed. The hydrologic/hydraulic analysis performed indicates that the spillway does not have sufficient capacity to discharge the peak outflow from one-half the Probable Maximum Flood (PMF). However, a high tailwater condition could be expected for this storm event and a dam failure would not significantly increase the hazard to loss of life from that which would exist just before an overtopping induced failure. Therefore, the spillway capacity of this structure has been rated as inadequate.

The dam, however, has a number of problem areas which requirefurther investigation and remedial work. The structural stability analysis indicates unsatisfactory stability would result from loadings which could occur during all investigated conditions. A structural stability investigation should be commenced within 6 months to determine the characteristics of the uplift forces acting on the dam, the properties of the existing dam and the effect of these conditions on the stability of the dam. The hydrologic/hydraulic analysis indicates that the spillway will pass only 26% of the Probable Maximum Flood (PMF). The dam will be overtopped by 6 feet and 1-1/2 feet by the PMF and 1/2 PMF respectively. Failure of the dam during the 1/2 PMF event would significantly increase the downstream hazard from that which would exist just prior to failure of the dam. Based on an initial screening, and preliminary computations, there appears to be a serious deficiency in spillway capacity so that if a severe storm were to occur, overtopping and failure of the dam would take place, significantly increasing the hazard to loss of life downstream from the dam.

Book excerpt: The Ephratah Dam is 760 feet long, buttressed concrete and cyclopean masonry dam containing an ogee spillway, 3 arched sections, a concrete gravity section and an earth embankment section. The maximum height of the dam is 65 feet. The ogee spillway is 251 feet long and has a crest elevation of 974.15. A 3 feet diameter reservoir drain is located at the base of the middle arched section. The flow of the drain is controlled by a manually operated butterfly valve located in the building at the toe of the dam. The power generation intake system is located on the upstream face of the south abutment. A 6.5 feet diameter wood stave pipe carries water from the intake 2 miles to the power plant. All regulating outlets are operational. The concrete portions of the dam exhibit signs of major deterioration as indicated by the extensive seepage which appears on the downstream face. Gunite and epoxy injection treatments have failed to control the flow. The discharge capacity of the spillway is inadequate for all flow in excess of 17% of the Probable Maximum Flood (PMF). The spillway is not considered seriously inadequate, based on the Corps of Engineer's Screening Criteria, since the dam is a gravity structure and the stability analyses indicate that the dam is not unstable during a PMF event.

A number of minor deficiencies were noted on this structure. These deficiencies include: debris around the trash racks of the orifice in the riser intake structure, debris (logs) on the lower half of the upstream slope as measured from the crest to the normal pool elevation, slight erosion along abutment- embankment contacts on lower third of downstream slope, damaged internal drainage pipes above plunge pool, small animal burrow on downstream slope just above riprap around the principal spillway outlet pipe, wet areas beyond downstream toe in waste area along east side of outlet channel and natural flood plain on the west side of the outlet channel.

Book excerpt: The Beaver Dam Lake, formerly known as Salisbury Mills Dam, consists of left and right non-overflow sections with an included spillway. The dam is about 335 feet long with a maximum height of about 35 feet. The width of the wall at its top, which is also the crest of dam, varies from 3.5 feet to 3.75 feet. The principal spillway is an ungated masonry and concrete gravity structure which is divided into two sections by an 8-foot wide stepped brick pier. The left and right spillway sections are about 30 feet and 35 feet long, respectively. Examination of available documents and a visual inspection of the dam and the appurtenant structures did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some deficiencies which require further investigation and remedial action. Using Corps of Engineers screening criteria, it has been determined that the dam would be overtopped for all storms exceeding approximately 37 percent of Probable Maximum Flood (PMF).

Using the Corps of Engineers screening criteria for review of spillway adequacy, it has been determined that the dam would be overtopped for all storms exceeding approximately 75 percent of the PMF. The spillway is, therefore, considered to be 'inadequate'. A review of the structural stability analysis performed by the designer indicated calculated safety factors against sliding to be less than recommended minimum value. A more detailed analysis which was performed as a part of this Phase I investigation produces a similar conclusion. Therefore, it is recommended that within 6 months of notification to the owner a detailed field investigation and monitoring program be undertaken to determine the actual hydrostatic uplift pressures along the base of the dam and that further detailed structural stability analyses be performed using this information. Debris should be removed from the upstream face of the dam as part of the routine maintenance program. Finally, leakage along the south side of the south tainter gate should be monitored on a daily basis with these observations documented in the dam operator's log book.
