Book excerpt: The dam is located on Kings Creek, approximately 4 miles north of the Village of Estella, Elkland Township, Sullivan County, Pennsylvania. The Roscoe Burgess Dam can be located on the Overton U.S.G.S. 7.5 minute quadrangle. The onsite inspection of the Roscoe Burgess Dam consisted of: (1) Visual inspection of the retaining structure, abutments and toe. (2) Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works. (3) Observations affecting the runoff potential of the drainage basin. (4) Evaluation of the downstream area hazard potential. In general, the dam and appurtenant structures appear to be in fair condition. No major erosion areas were observed during the inspection. No seepage was observed along the downstream slope of the structure or along the toe area. Dense brush exists along the downstream slope of the structure and hampered attempts at close visual inspection of the downstream slope and toe area. The brush and small trees on the downstream slope and in the area of the toe of the dam should be removed under the direction of a registered professional engineer. Once the slope and toe area are cleared, the area should be reinspected to verify that no seepage existed at the structure. No drainline was observed at the structure. This is considered a deficiency in some means should be provided to drain the reservoir.

The deteriorated condition of the spillway ways and the adjacent low areas of the embankment are the primary deficiencies which require maintenance work to assure the operational safety of this facility. In accordance with the recommended guidelines, the spillway design flood (SDF) for this facility is in the range of the 100 year flood to the 1/2 PMF. Based on the size of this dam and the degree of downstream hazard, the selected SDF is the 100 year flood. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and spillway discharge capacity will pass the SDF (100 year flood) prior to overtopping the embankment. Therefore, in accordance with the criteria outlined and evaluated in Section 5.5 of this report, the spillway for Ross Dam is considered to be adequate.

The assessment of the general condition of the dam is based upon visual observations and review of available data. Detailed investigations and analyses involving topographic mapping, subsurface investigations, material testing, and detailed computational evaluations are beyond the scope of a Phase I investigation; however, the inspection is intended to identify any need for such studies which should be performed by the owner. In reviewing this report, it should be realized that the reported condition of the dam is based on observations of field conditions at the time of inspection along with data available to the inspection team. In cases where the reservoir was lowered or drained prior to inspection, such action, while improving the stability of the dam, removes the normal load on the structure and may obscure certain conditions which might otherwise be detectable if inspected under the normal operating environment of the structure.

Based on visual inspection, available records, calculations and past operational performance, PA-467 Dam is judged to be in good condition. The existing spillway can pass the Probable Maximum Flood(PMF) without overtopping of the dam. The spillway capacity is rated as adequate. There is a slope stability analysis for the embankment, and it indicates that the embankment has adequate factors of safety. There is no evidence of significant problems threatening the embankment.

Book excerpt: The assessment of Sugar Creek Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The inspection and review of Sugar Creek Dam did not reveal any problems which would require emergency action. The dam appears to be in fair condition and in need of minor maintenance. The Sugar Creek Dam is a high hazard-small size dam. The spillway design flood (SDF) for a dam of this size and classification is 1/2 PMF to PMF. The PMF has been selected as the spillway design flood based on the downstream potential for loss of life. The spillway and reservoir are considered capable of controlling approximately 50% of the PMF. The 0.1 feet of overtopping of the embankment crest during the 1/2 PMF event for a duration of 1 hour was not considered sufficient to cause failure of the structure. Based on the judgement of the evaluating engineer, the location and duration of the overtopping as well as the existence of a concrete curb on the upstream and downstream edge of the embankment crest would sufficiently contain the overtopping to the spillway area minimizing damage to the embankment and spillway. The spillway is considered inadequate.

Book excerpt: The visual inspection, operational history, and hydrologic/hydraulic analysis indicate that the facility is in fair condition. Deficiencies noted by the inspection team included heavy overgrowth across the embankment crest and slopes, rodent burrows along the downstream embankment face, a deteriorated emergency spillway, and minor seepage beneath the emergency spillway and around the outlet conduit. These deficiencies are, for the most part, attributable to a general lack of adequate maintenance since the facility was phased out of operation in 1973. The size classification of the facility is small and its hazrad classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probably Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only 16 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

There is no program for regular inspection of the dam.

Book excerpt: Humboldt Dam is a homogeneous earthfill embankment with a masonry core-wall. The embankment is 974 feet long and 41 feet high at maximum section. A masonry gravity retaining wall, which acts as the spillway training wall, is at the right end of the embankment. The outlet works, which is near the middle of the embankment, consists of a masonry and corrugated metal intake structure with sluice gates, a 12-inch diameter cast-iron pipe(CIP), and a valve house at the toe of the dam. The intake structure is located on the upstream slope of the embankment; a bridge extends to it. Water supply pipes also extend from the intake structure. The masonry gravity spillway is at the right abutment of the dam. Its crest is 3.0 feet below the design elevation of the top of the dam and is 33.0 feet long. The spillway channel has a variable bottom width and a masonry gravity training wall on the left. The spillway channel extends along the right side of the valley.

Book excerpt: Big Brown Dam consists of an earth embankment approximately 290 feet long with a maximum height of approximately 37 feet from the downstream toe, and a crest width in the range of 8 to 11 feet. The downstream slope of the dam is 1-1/2 horizontal to 1 vertical and is protected with well-established grass. The upstream slope is protected by riprap extending to the crest level of the dam. The flood discharge facilities for the dam consist of a primary spillway incorporated into the outlet facilities and an open channel emergency spillway located on the right abutment. The outlet facilities consist of a 20-inch cast-iron supply line and a 20-inch cast-iron blow-off pipe. Based on the evaluation of the existing conditions, the condition of Big Brown Dam is considered to be poor. Extensive swampy areas were observed along the toe of the dam. This condition, together with the steep downstream slope of the embankment raises concern as to the continued stability of the embankment. The flood discharge capacity of the dam was found to be at the lower limit of the recommended spillway capacity range of 50 percent to full PMF relative to the size and hazard classification of the dam.

Although there is some seepage thru the dam, there is no evidence of structural instability. There is no formal inspection and maintenance program or warning system and evacuation plan in effect at Acre Pond Dam. The dam is not properly maintained.

The inspection consisted of: Visual inspection of the retaining structure, abutments and toe; Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works; Observations affecting the runoff potential of the drainage basin; and Evaluation of the downstream area hazard potential. The dam appeared to be in fair condition. From a brief survey conducted during the inspection, it was noted that the low spot on the top of dam existed adjacent to either edge of the spillway on the retaining walls. The earthen embankment section rises gently from the spillway to either abutment.
