Deficiencies noted during the inspection included the lack of functional drawdown facilities and minor seepage through the masonry near the right abutment. Based on the size and hazard classification of the dam, the recommended Spillway Design Flood (SDF) varies between 1/2 the Probable Maximum Flood (PMF) and the PMF. The 1/2 PMF has been selected as the SDF due to the size of the dam and reservoir, and the downstream conditions. The hydrologic and hydraulic computations indicate that the combination of reservoir storage and spillway discharge capacity will pass only 3 percent of the PMF without overtopping the dam. Overtopping the dam could cause failure, which would lead to a significant increase in downstream loss of life and property damage. Therefore, the spillway for Lake Quinn Dam is considered to be seriously inadequate, and the dam is judged to be unsafe, non-emergency.

Book excerpt: Greene-Dreher Dam is a 20-foot high earthern embankment, 1,000 feet long which impounds a 152 acre-foot flood water retention pool. Total estimated volume of fill is approximately 30,000 cubic yards. The zoned embankment was constructed on natural soils without a cutoff trench or a grout curtain. The embankment contains three material zones; Zone 1, a relatively impervious core of silt and and silty sand; Zone 2, composed of silty gravel and silty sand; and an outer shell of Zone 3 material, described as rock larger than 6 inches in diameter. The embankment crest is 12 feet wide at a design settled elevation of 2,036 feet, and both upstream and downstream slopes are 3H:1V. The downstream section of the embankment contains a filter blanket approximately 2 feet thick, 30 feet wide and 320 feet long. The filter blanket is drained by a 6-inch diameter perforated corrugated metal pipe connected to a 6-inch diameter pipe which outlets adjacent to the principal spillway outlet.

As the spillway cannot pass the 1/2 PMF without overtopping at less than 1/2 PMF would cause failure which would significantly increase hazard to loss of life downstream, the spillway capacity is seriously inadequate and the facility is rated unsafe, nonemergency. Although there is no visible evidence of structural instability, the erosion of the downstream slope below the outlet pipe and the leakage below the toe of the dam are of concern. Should the observed erosion and leakage remain unchecked, it could, in time, affect the structural integrity of the dam. The invert elevation of the outlet pipe is but 2.1 feet below the spillway crest. There are no means to lower the reservoir level below the present level of the pipe inlet in case of emergency. The maintenance of the dam is limited to mowing grass on the crest and a portion of the upstream slope of the embankment. The balance of the dam is poorly maintained.

The hydrologic and hydraulic computations indicate that the combination of reservoir storage and outlet works discharge capacity will not pass the SDF (1/2 PMF) prior to overtopping the embankment. Under present conditions, the discharge/storage capacity is 11% of the PMF prior to overtopping. In accordance with the criteria outlined and evaluated in the discharge capacity for Rosner Pond Dam is considered to be seriously inadequate. The dam in its present condition is considered to be unsafe, non-emergency.

Book excerpt: Based on available records, visual inspection, calculations, and past operational performance, Craft Pond Dam is judged to be unsafe, nonemergency, because the spillway capacity is rated as seriously inadequate. The recommended Spillway Design Flood (SDF) for the size and hazard classification of the dam varies between 1/2 of the Probable Maximum Flood (PMF) and the PMF. The selected SDF is the 1/2 PMF. The existing spillway will pass only about 23 percent of the Probable Maximum Flood (PMF) before overtopping of the dam occurs. It is judged that the dam could not withstand the depth and duration of overtopping that would occur for the 1/2 PMF. Failure of the dam would cause an increased hazard for loss of life downstream. Overall, the dam is in good condition. The maintenance of the dam is considered to be good; only a few minor maintenance deficiencies were observed. The other deficiencies observed were low spillway walls and an inoperable outlet works.

Book excerpt: Based on the visual inspection, past performance and the available engineering data, the dam and its appurtenant structures appear to be in good condition. In accordance with the Corps of Engineers' evaluation guidelines, the size classification of this dam is intermediate and the hazard classification is high. The spillway capacity combined with the available storage is sufficient to pass the PMF (Probable Maximum Flood) peak inflow without overtopping the dam. The spillway is therefore considered to be adequate. These calculations are based on a maximum stoplog elevation of 10 feet above the wetwell floor. The following recommendations are presented for immediate action by the owner: (1) That the upstream end of the outlet conduit be inspected, (2) That the downstream section of the outlet conduit be inspected on an annual basis for signs of deterioration, (3) That all brush be removed from the embankment and that the downstream wall be kept clear of all growth, (4) That a formal surveillance and downstream warning system be developed to be used during periods of high or prolonged precipitation, and (5) That a program be developed for regular inspection and maintenance.
