Book excerpt: Mosquito Run Dam is owned by Williamsport Municipal Water Authority and is classified as a 'Significant' hazard - 'Small' size dam. The dam was found to be in good overall condition at the time of inspection. A spillway design flood (SDF) in the range of the 100-year flood to the 1/2 Probable Maximum Flood (1/2 PMF) is required for Mosquito Run Dam. Because the dam is on the low end of the 'Small' size category in terms of height and storage capacity, the 100-year flood was chosen as the SDF. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway capacity is greater than the inflow to the impoundment during the 100-year flood. The spillway is therefore considered 'Adequate.' The inspection revealed certain items of remedial work which should be performed by the owner without delay.

Book excerpt: The visual inspection, operational history, and hydrologic/hydraulic analysis indicate that the facility is in excellent condition. The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility is capable of passing and/or storing the PMF. Consequently, its spillway is considered adequate. It is recommended that the owner immediately (a) provide positive drainage for the area between the left spillway wingwall and left abutment hillside and (b) repair and seal all concrete cracks observed along the left spillway wingwall. Book excerpt: The visual inspection, operational history, and hydrologic/hydraulic analysis indicate that the facility is in good condition. Deficiencies noted by the inspection team included slumping and bulging of the rock surface on the steep downstream embankment slope, minor seepage along the downstream embankment toe and minor deterioration of the control tower. In addition, the growth of large trees in the immediate area of the downstream toe is a hindrance to expedient visual evaluation of the embankment. The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probable Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and possibly loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only 27 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

Book excerpt: DuBois Creek Dam is located on DuBois Creek, approximately 2.1 miles southwest of Hartford, Pennsylvania. The inspection was performed on 28 October 1980. A light rain was falling at the time of inspection. The dam and appurtenant structures were found to be in poor overall condition at the time of inspection. Seepage was observed at three locations passing through deteriorated portions of the concrete core wall. These seepage areas are the result of progressive deterioration of the exposed areas of the concrete core wall. The seepage has caused erosion gullies and slight irregularities on the downstream face of the dam.

Book excerpt: Lake Walter Dam is located on Spring Run, approximately one mile upstream of its confluence with Meshoppen Creek in Springville Township, Susquehanna County, Pennsylvania. The condition of Lake Walter Dam is considered to be good. The only significant condition noted which requires further attention is the lack of erosion protection in the emergency spillway discharge channel. The swampy condition at the toe of the dam is not considered to be significant at this time; however, the condition should be monitored to document if seepage is developing.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the dam is considered to be in poor condition. The size classification of the facility is small and the hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) ranges between the 1/2-PMF (Probable Maximum Flood) and PMF. Since the dam is near the lower end of the small size classification range and because of the lack of extensive downstream development, the SDF for this facility is considered to be the 1/2-PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 20 percent of the PMF prior to dam overtopping. a breach analysis indicates that failure under 1/2-PMF conditions would probably not lead to increased property damage or loss of life at existing residences. Thus, based on the screening criteria contained in the recommended guidelines, the spillway is considered to be inadequate, but not seriously inadequate. The facility was phased out of operation in 1965 and has since not been subject to a schedule of routine maintenance. Since the facility no longer serves its original purpose (water supply) and in essence, has been abandoned, it is recommended that the owner dismantle the embankment in accordance with PennDER, Division of Dam Safety, regulations.

Book excerpt: Page's Lake Dam is owned and operated by the Lakeside Outing Club and is classified as a 'Significant' hazard - 'Intermediate' size dam. The dam was found to be in good overall condition on 27 October 1980. Hydraulic/hydrologic evaluations, performed in accordance with procedures established by the Baltimore District, Corps of Engineers, for Phase I Inspection Reports, revealed that the spillway will not pass the 1/2 Probable Maximum Flood (1/2 PMF) without overtopping the dam. A spillway design flood (SDF) in the range of the 1/2 PMF to the Probable Maximum Flood (PMF) is required for Page's Lake Dam. The 1/2 PMF was chosen as the SDF because the dam is on the low end of the 'Intermediate' size category in terms of storage. During the 1/2 PMF, the dam is overtopped by a maximum depth of 3.34 feet for a total duration of 11.83 hours. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately initiate an engineering study to further evaluate the spillway capacity and develop recommendations for remedial measures to reduce the overtopping potential of the dam.

This book was released on 1980 with total page 107 pages. Available in PDF, EPUB and Kindle. Book excerpt: Lake Jean Dam is a zoned earth embankment approximately 26 feet high and 780 long, including spillway. The emergency spillway is an uncontrolled, rectangular, concrete and rock cut chute channel located at the right abutment. The crest of the spillway is 20 feet long and is spanned by a concrete bridge which supports a portion of the bituminous roadway that covers the embankment crest. Drawdown capability is provided by means of a 30-inch diameter cast iron blowoff pipe controlled at the inlet by a 30-inch diameter sluice gate. The facility consists of a main dam (Lake Jean Dam) and two appurtenant saddle dams (east and west dikes). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 73 percent of the PMF prior to overtopping the east dike. Thus, based on the screening criteria contained in the recommended guidelines, the spillway is considered to be inadequate, but not seriously inadequate. Should the saddle dams be regraded to their design elevation, the spillway would then pass floods in excess of the PMF and would be considered adequate.

Book excerpt: Hathaway Pond Dam is located in Ararat Township, Susquehanna County, Pennsylvania. The reservoir was originally created to provide water for power purposes. The reservoir slopes are moderate with a good cover of vegetation. There are three ponds upstream from Hathaway Pond Dam on two tributary streams. The western stream contains Romobe Lake (PennDER I.D. No. 58-10) which is 1750 feet upstream from Hathaway Pond Dam. The eastern tributary contains Ball Lake and an unnamed smaller pond in series. Ball Lake is 2450 feet upstream and will retain considerable storage while the smaller unnamed pond was considered to have no effect on Hathaway Pond Dam. Two homes, a trailer, and township road are located 1800 feet downstream from Hathaway Pond Dam. These could suffer economic damage in the event of a dam failure.

Phase I Inspection Report written by John A. Dziubek and published by . This book was released on 1981 with total page 67 pages. Available in PDF, EPUB and Kindle. Book excerpt: Warner Dam is owned by William S. Newlin, Jr., and is classified as a 'Low' hazard - 'Small' size dam. The dam was found to be in fair overall condition at the time of inspection. Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.
