The embankment is about 625 feet long with a maximum height of approximately 14 feet. The top width of the dam is a minimum of 150 feet and the earth embankment side slopes are 3 horizontal to 1 vertical (3H:1V) both upstream and downstream. The top of the dam is used as a golf course fairway. A drop spillway, with a round crested weir, is located adjacent to the right (looking downstream) abutment. The crest of the weir is 3 feet below the top of the dam, and the length of the weir is 47 feet. The drop spillway is of stone masonry construction except for the rounded weir crest section which is poured concrete. The distance from the drop spillway weir crest to the apron is about 7 feet. The apron is 20 feet long and slopes down from each side wall. The transverse end sill is of negligible height at the side walls and is about 2 feet high in the center. Two 24-inch semi-circular corrugated steel pipes are constructed into the end sill and act as control structures. Flow passes over the drop spillway, through the two corrugated pipes, and into Lower Lake.

The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 40 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.

Book excerpt: Hemlock Lake Dam is an earth embankment about 450 feet long and 27 feet high. An ungated overflow spillway is located at the right abutment. A paved roadway has been constructed on the dam crest and a bridge has been constructed over the spillway. The dam was constructed in the early 1950's to provide a lake for recreational purposes. Hemlock Lake Dam has a maximum storage capacity of 117 acre-feet and a maximum height of 27 feet. The dam is classified as 'Small' size. Due to the potential for excessive property damage and the loss of more than a few lives in the damage center which consists of 5 houses with door sills as low as 5 feet above the stream bed located within 1,500 feet downstream of the dam, the structure classified as a 'High' hazard. Examination of the results of the hydrologic and hydraulic analyses indicates that the spillway is capable of passing approximately 26% of the Probable Maximum Flood (PMF) without the dam being overtopped. The selected Spillway Design Flood (SDF) for this 'Small' size, 'High' hazard potential structure is the PMF. The water surface elevation at the damage center is 2.4 feet higher for the breach condition than for the non-breach condition. The spillway is classified as 'Seriously Inadequate, Unsafe, Non-Emergency' since breaching of the dam significantly increases the downstream hazard potential. Based on visual observations and a review of the information obtained from the Pennsylvania Department of Environmental Resources (DER), Hemlock Lake Dam appears to be in poor condition.

The top of the dam is used as a public roadway and has a bituminous surface. The spillway is located near the left abutment of the embankment. A fish guard-screen is attached to the upstream side of the concrete ogee section. The roadway crosses the spillway discharge channel immediately downstream from the weir (Photograph 7, Appendix C). The bridge is a concrete slab deck structure supported on two piers and two abutment walls. The outlet control structure is constructed differently from that shown on Plate IV, Appendix E. It actually consists of a valve chamber with the valve located on the inside of the upstream wall. Water is discharged directly into the valve chamber and flows from there through a 24inch diameter outlet pipe under the embankment to the downstream toe. The valve chamber is covered with a large overhanging concrete slab and is accessible by a footbridge from the crest of the dam. (Author).

The embankment is approximately 400 feet long and has a maximum height of 41 feet. A 90-foot long overflow spillway is located between the embankment and the left abutment. The dam is provided with 5 feet of freeboard above the spillway, crest. Spillway training walls extend both upstream and downstream of the spillway, as shown on Plate 4. A concrete apron extends 100 feet downstream of the spillway, and varies in width from 90 feet at the spillway to 25 feet at the outlet channel. A trapezoidal outlet channel is lined with grouted riprap for approximately 250 feet downstream of the apron. A 24-inch diameter reinforced concrete pipe is provided as a low level outlet. Flow through the pipe is controlled by a gate valve at the upstream toe of the embankment. The operating stem expands from the valve to the top of dam, and is supported on concrete columns embedded in the embankment.

It consists of a reinforced concrete riser structure near the upstream toe of the dam, a 24-inch diameter conduit, and a concrete outlet structure at the downstream toe of the dam. The auxiliary spillway is an excavated, trapezoidal channel at the left abutment of the dam. Based on visual inspection, available records, calculations, past operational performance, and according to criteria established for these studies, Maple Lake Dam is judged to be in good condition. Based on existing conditions, the main and auxiliary spillways will pass an approximate minimum of 48 percent of the Probable Maximum Flood (PMF before overtopping of the dam occurs. No stability problems were evident for the embankment at the time of the visual inspection, but a potential hazard to stability exists due to erosion that might occur when there is flow in the auxiliary spillway.

Book excerpt: Pecks Pond Dam is located on Bush Kill Creek in Porter Township, Pike County, Pennsylvania. The facility is situated within 1000 feet of Pennsylvania Route 402, about 5 miles south of Interstate 84 and about 25 miles north of the city of East Stroudsburg, Pennsylvania. Observations made during the visual inspection indicate the embankment is in good condition. No evidence of seepage through the downstream embankment face, sloughing, excessive settlement, animal burrows, or signs of major maintenance neglect were observed. Minor erosion along the upstream embankment face and behind the upstream portion of the right spillway wingwall were noted, but, are not considered significant at this time. Provisions for erosion protection along the upstream embankment slope were apparently not included in the original design or in the design for the 1934 modifications. Minor cracks observed along the concrete cap that partially covers the crest were recently filled and adequately repaired. Some of the dense brush located along the downstream embankment toe had to be cut back by the inspection team in order to achieve a clear view of this area. It is suggested that control of this excess vegetation be specifically included as part of future routine maintenance.

The dam inpounds a reservoir with a surface area of 250 acres and a storage capacity of 1,085 acre-feet at normal pool level. The top of the dam is 20 feet wide; the upstream and downstream side slopes are approximately 2.5 horizontal to 1.0 vertical (2.5H:1V). No information is available concerning the properties of the embankment materials. Within the embankment is a tapered reinforced concrete cutoff wall that extends from bedrock to 3.5 feet below the top of the embankment. The reinforced concrete spillway, which has maximum height of 13 feet from base to crest, consists of 3 sections separated by bridge piers. Each section id broad-crested weir with a vertical upstream face and a flat, sloping downstream face. The crest of the center section is 48.5 feet long and crest of each of the two end sections is 45.75 feet long. The crest of the center section, which is 3 feet wide, is 0.5 feet below the crest of the end sections; each of the end sections is 2.5 feet wide.

The community is nestled in the mountains of Pike County less than five miles west of U.S. Route 209 which parallels the Delaware River in this area. Observations made during the visual inspection indicate the embankment and adjacent dike are in good condition. Heavy brush growth and small trees were observed along the embankment crest and upstream face. No evidence of seepage through the downstream embankment face, excess settlement, sloughing, erosion or animal burrows was encountered by the inspection team. Some rock slope protection has been displaced immediately adjacent the spillway right wingwall, leaving small bare areas on both the upstream and downstream embankment faces. The top of the concrete corewall exhibits excessive cracking and spalling along its entire length except for that portion associated with the dike along the right abutment which is in relatively good condition.

The facility is located approximately 1,500 feet upstream and west of wild Acres Lake, and about five miles from U.S. Route 209 which parallels the Delaware River in this area. Observations made during the visual inspection indicate the embankment is in fair condition, but in need of general maintenance. No evidence of seepage through the downstream embankment face, sloughing, erosion or animal burrows was observed. The embankment crest is noticeably low (in excess of 1-foot) near the spillway relative to the remainder of the crest. About two-thirds of the embankment is overgrown along an area extending approximately 400 feet to the right of the spillway. The vegetation is characterized as primarily brush and high weeds with clumps of small trees strewn throughout. In contrast, much of the embankment crest and upstream face are bare and unprotected with only light vegetation visible. Several long tire ruts scar the crest near the spillway. A small area characterized by swamplike vegetation is located about 350 to 375 feet from the right abutment along the downstream embankment toe. Seepage was observed at the downstream embankment toe near the outlet conduit flowing at a rate of about two to three gpm. The flow is suspected of being either seepage through the embankment foundation, or perhaps drainage from a concealed toe drain. A precise assessment of the condition is difficult without adequate design data are drawings.

The cut-off trench has a base width of 12 feet and extends into the foundation. The cut-off trench is backfilled with a zone of silty sand and silt of low permeability. The central core of the dam is composed of silty sand and silt to an elevation 9.5 feet below the top of the dam. The remainder of the dam is a pervious zone of silty gravel. The earth embankment has side slopes of 3 horizontal to 1 vertical (3H:1V) on the upstream side and 2H:1V on the downstream side. Berms exist on the upstream and downstream slopes of the dam. The berm on the upstream side is 8 feet wide and slopes towards the reservoir on a 12.5 percent grade; its lowest elevation is at the normal pool 1708.4. The berm on the downstream slope is 36 feet wide and slopes downstream on a 1.4 percent grade. This berm is used as a roadway with its lowest elevation at 1710.(Author).

In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probable Maximum Flood) and the PMF. Since the facility is classified near the lower bounds of the small category, the SDF is considered to be the 1/2 PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 15 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency.
