The overarching goal of this new edition is identical to the first and second -- to provide the clinician and the scientist with a common resource for understanding the profound achievements in the clinical, translational, and basic sciences of neuromuscular transmission disorders. In addition to several new authors and an extensive update of all chapters, this third edition includes summaries of pre-clinical research standards for autoimmune MG, along with a broad summary of MG clinical trial performance. The now greater understanding of the clinical presentation of MuSK-related MG and identification of potential new autoantigens, including LRP-4, is discussed. The development of treatment guidelines by groups in Japan, the United Kingdom, Germany, and an international consortium is also outlined. Myasthenia Gravis and Related Disorders, Third Edition, is an invaluable resource for meeting the many and varied needs of clinicians who treat patients with myasthenia gravis.

Available in PDF, EPUB and Kindle. Book excerpt: Autoimmune myasthenia gravis (MG) is a classical autoimmune disease, for which the target antigen, nicotinic acetylcholine receptor, has been cloned, sequenced and biochemically characterized. Antibodies to acetylcholine receptors destroy acetylcholine receptor at the neuromuscular junction, thus leading to defective neuromuscular transmission, muscle fatigue, and weakness. In the last few years, rapid advances have been made in unraveling the cellular and molecular mechanisms involved in the pathogenesis of MG, both in the animal model, experimental autoimmune MG (EAMG), and in human MG. Significant advances are being made in characterizing the cells and molecules involved in the autoimmune response to the acetylcholine receptor (AChR). These advances are leading to the development of specific methods of immunointervention in EAMG. Further understanding of the intricate involvement of the major histocompatibility complex (MHC) and non-MHC genes, T cell receptors (TCR), costimulator molecules, and specific cytokines in the afferent and efferent autoimmune response of AChR should pave the way to future antigen/clone-specific therapy of MG. This book is the outcome of the MG workshop proceedings in Mysore, India, 1998. The majority of the chapters in this book are contributed by world-renowned authors and their students. The book not only contains a timely review of specific topics, but also up-to-date findings. Immunologists and neurologists will find, in this volume, the latest in MG/EAMG cutting-edge research. Clinicians will be interested in the applications of the various immunointervention strategies into clinical trials in MG patients. Finally, students will not only be interested in reading the latest in EAMG/MG research, but will also find information to help them develop a future strategy to unravel the precise mechanism of disease. To summarize, in this book, the readers should find up-to-date information related to immunological mechanisms involved in MG pathogenesis and various modalities for possible approaches to immunointervention to treat MG.

Available in PDF, EPUB and Kindle. Book excerpt: Myasthenia gravis is a rare potentially fatal chronic autoimmune disorder. Circulating autoantibodies directed against components of the neuromuscular junction of skeletal muscles, most commonly nicotinic acetylcholine receptor and associated protein in the postsynaptic membrane, block neuromuscular transmission resulting in muscle weakness. This muscle weakness typically worsens with continued activity, improves on rest, and is of variable severity ranging from mild ocular muscle weakness to severe generalized muscle weakness, involving the respiratory muscle with impending respiratory failure. The worldwide prevalence of myasthenia gravis is 100-200 per million population, affecting more than 700,000 people all over the world. The prevalence rate has increased since the 1950s due to improved diagnostic precision and decreased mortality rate.

Book excerpt: Myasthenia gravis involves a chronic weakness of voluntary muscles, which improves with rest and worsens with activity. Weakness occurs when the nerve impulse does not adequately reach the muscle cells, which is caused by blockage of the action of transmitters. Third Age Media, Inc. offers this information as part of its online health encyclopedia.

Book excerpt: An in-depth look at a rare disease Myasthenia gravis (MG) is a rare autoimmune disorder of the neuromuscular junction, characterized by muscle fatigability. Patients often initially present with ocular symptoms, but in most cases the disease spreads beyond the eye muscles to more generalized involvement of bulbar, facial, neck, proximal limb and respiratory muscles. With adequate treatment, most patients with MG are able to live productive lives with few or no symptoms, but a distinct subset of patients do not respond to conventional treatment. With new treatment options on the horizon, it is important that these patients are identified. 'Fast Facts: Recognizing Refractory Myasthenia Gravis' takes an in-depth look at: • the immune-mediated nature of MG • classification of MG by disease type and severity, and antibody status • clinical presentation and diagnostic work-up • conventional management options • how patients with treatment-refractory MG present • the assessment tools that can be used to identify non-responders. This informative resource will be of value to neurologists, neurology trainees and ophthalmologists caring for patients with this rare disease, as well as patients with MG who wish to have a deeper dialog with their doctor or patient group. Contents: • Definition and epidemiology • Pathophysiology and classification • Diagnosis and management: an overview • Assessment of disease severity and treatment response

Available in PDF, EPUB and Kindle. Book excerpt: Myasthenia gravis (MG), an acquired immune syndrome affecting proteins at the neuromuscular junction, causes significant muscular functional impairment and can compromise patients’ quality of life, including fatigue, disability, and increased mortality rates. Continued efforts in the last decades have begun to produce significant progress in our understanding of this rare condition, yet considerable challenges remain for improving diagnosis and developing effective treatments for MG. The present volume comprises papers stemming from the “12th International Conference on Myasthenia Gravis and Related Disorders,” held May 21–23, 2012 at the New York Academy of Sciences in New York City. The papers collected in this first of two Annals volumes highlight some of the key developments in neuroscience and immunology in MG and related syndromes presented at the conference, including structure and function of the neuromuscular junction, advances in immunology and their relationship to myasthenia gravis, clinical and laboratory developments, outcome measurements and clinical trial development, and animal models of neuromuscular junction disease. Book excerpt: Neuromuscular medicine is constantly advancing in terms of accurate diagnosis, pathophysiology, and treatment. Many disorders that have been discovered within this field are either autoimmune or genetic. This publication provides the latest updates of the more common and treatable autoimmune neuromuscular diseases affecting the spinal cord, peripheral nerves, neuromuscular junction and muscles. The authors, renowned experts in the field, offer clinical presentations as well as the best diagnostic and treatment approach to the readers. The chapters are dealing with acute and chronic neuropathies, nonsystemic vasculitic neuropathy, dysimmune neuropathy and autoimmune autonomic ganglionopathy. Further, disorders such as myasthenia gravis, Lambert-Eaton myasthenic syndrome, idiopathic inflammatory myopathies and stiff person syndrome are discussed.Providing an excellent source of up-to-date information, this publication is highly recommended not only to neurologists, but also to internists, rheumatologists, allergists and immunologists.

Book excerpt: Myasthenia Gravis (MG) is a chronic autoimmune neuromuscular disorder caused by autoantibodies against post-synaptic proteins at the neuromuscular junction. These antibodies mostly attack the acetylcholine receptor (AChR), the muscle-specific kinase (MuSK), and the low-density lipoprotein-related protein 4 (LRP4). The main clinical characteristic of the disease is the presence of fluctuating fatigable muscle weakness involving the extraocular, facial, bulbar, respiratory, cervical, and proximal limb muscles. It is known that the thymus has an important role in the pathophysiology of the disease, and some patients associate a thymoma.
