However, because of physical limitations, even with IMRT a certain dose to organs at risk is unavoidable, so in treatment planning often a compromise between different plan criteria has to be found. This thesis presents a new optimization concept that is dedicated to this type of problem. It is based on the fact that there is no single optimal plan, but many so-called Pareto optimal plans, which cannot be improved in one criterion without worsening another. A database with all Pareto optimal plans is generated and the decision which compromise is clinically the best is left to the planner. Because of the higher calculation complexity, the dose calculation of the existing planning system was accelerated by a factor of three through statistical sampling of the calculation matrices. To describe a plan, a vector containing the 'equivalent uniform doses' (EUDs) of each structure is used. Therefore two EUD models were analyzed, the 'max & mean' and the 'generalized mean' model. An algorithm was developed to include the 'generalized mean' model in the plan optimization. At the end of the thesis, the database containing all Pareto optimal solutions of a clinical test case with 1 target volume and 2 organs at risk was generated and visualized. The planner can see at first sight the dependencies of the different plan elements and can decide which plan is clinically the best.

Book excerpt: The aim of intensity modulated radiation therapy (IMRT) is to kill tumor cells while at the same time protecting the surrounding tissue and organs from the damaging effect of radiation. Given the number of beams and beam directions, beam intensity optimization needs to determine beam intensity profiles that yield the best dose distribution under consideration of clinical and physical constraints. In this book, we first review existing mathematical models and computation methods for the beam intensity optimization problem. Then we formulate the beam intensity optimization problem as a multiobjective linear programme (MOLP) with three objectives. The rest of the book is dedicated to developing methods to solve this large MOLP efficiently and to the application in the beam intensity optimization problem. The book provides the necessary mathmatical foundation of multiobjective optimization to solve multiobjective linear programming problems. It is well suitable to be used as a reference book on multiobjective optimization. Moreover it should also be useful to professionals who may be considering using multiobjective optimization.

This book was released on 2021-01-12 with total page 53 pages. Available in PDF, EPUB and Kindle. Book excerpt: Cancer is a widespread class of diseases that each year affects millions of people. It is mostly treated with chemotherapy, surgery, radiation therapy, or combinations thereof. High doserate (HDR) brachytherapy (BT) is one modality of radiation therapy, which is used to treat for example prostate cancer and gynecologic cancer. In BT, catheters (i.e., hollow needles) or applicators are used to place a single, small, but highly radioactive source of ionizing radiation close to or within a tumour, at dwell positions. An emerging technique for HDR BT treatment is intensity modulated brachytherapy (IMBT), in which static or dynamic shields are used to further shape the dose distribution, by hindering the radiation in certain directions. The topic of this thesis is the application of mathematical optimization to model and solve the treatment planning problem. The treatment planning includes decisions on catheter placement, that is, how many catheters to use and where to place them, as well as decisions for dwell times. Our focus is on the latter decisions. The primary treatment goals are to give the tumour a sufficiently high radiation dose while limiting the dose to the surrounding healthy organs, to avoid severe side effects. Because these aims are typically in conflict, optimization models of the treatment planning problem are inherently multiobjective. Compared to manual treatment planning, there are several advantages of using mathematical optimization for treatment planning. First, the optimization of treatment plans requires less time, compared to the time-consuming manual planning. Secondly, treatment plan quality can be improved by using optimization models and algorithms. Finally, with the use of sophisticated optimization models and algorithms the requirements of experience and skill level for the planners are lower. The use of optimization for treatment planning of IMBT is especially important because the degrees of freedom are too many for manual planning. The contributions of this thesis include the study of properties of treatment planning models, suggestions for extensions and improvements of proposed models, and the development of new optimization models that take clinically relevant, but uncustomary aspects, into account in the treatment planning. A common theme is the modelling of constraints on dosimetric indices, each of which is a restriction on the portion of a volume that receives at least a specified dose, or on the lowest dose that is received by a portion of a volume. Modelling dosimetric indices explicitly yields mixed-integer programs which are computationally demanding to solve. We have therefore investigated approximations of dosimetric indices, for example using smooth non-linear functions or convex functions. Contributions of this thesis are also a literature review of proposed treatment planning models for HDR BT, including mathematical analyses and comparisons of models, and a study of treatment planning for IMBT, which shows how robust optimization can be used to mitigate the risks from rotational errors in the shield placement.

Book excerpt: Intensity-modulated radiation therapy (IMRT), one of the most important developments in radiation oncology in the past 25 years, involves technology to deliver radiation to tumors in the right location, quantity and time. Unavoidable irradiation of surrounding normal tissues is distributed so as to preserve their function. The achievements and future directions in the field are grouped in the three sections of the book, each suitable for supporting a teaching course. Part 1 contains topical reviews of the basic principles of IMRT, part 2 describes advanced techniques such as image-guided and biologically based approaches, and part 3 focuses on investigation of IMRT to improve outcome at various cancer sites.

Book excerpt: Tackle the most challenging problems in science and engineering with these cutting-edge algorithms Multi-objective optimization problems (MOPs) are those in which more than one objective needs to be optimized simultaneously. As a ubiquitous component of research and engineering projects, these problems are notoriously challenging. In recent years, evolutionary algorithms (EAs) have shown significant promise in their ability to solve MOPs, but challenges remain at the level of large-scale multi-objective optimization problems (LSMOPs), where the number of variables increases and the optimized solution is correspondingly harder to reach. Evolutionary Large-Scale Multi-Objective Optimization and Applications constitutes a systematic overview of EAs and their capacity to tackle LSMOPs. It offers an introduction to both the problem class and the algorithms before delving into some of the cutting-edge algorithms which have been specifically adapted to solving LSMOPs. Deeply engaged with specific applications and alert to the latest developments in the field, it’s a must-read for students and researchers facing these famously complex but crucial optimization problems. The book’s readers will also find: Analysis of multi-optimization problems in fields such as machine learning, network science, vehicle routing, and more Discussion of benchmark problems and performance indicators for LSMOPs Presentation of a new taxonomy of algorithms in the field Evolutionary Large-Scale Multi-Objective Optimization and Applications is ideal for advanced students, researchers, and scientists and engineers facing complex optimization problems.
