This primer addresses the increased demand for mental health care by supporting stakeholders (help-seekers, providers, and policy-makers) to collaborate in enhancing care outcomes through work that is both more meaningful and sustainable. Our current mental health system is organized to offer highly intensive psychiatric and psychological care. While undoubtedly effective, demand far exceeds the supply for such specialized programming. Many people seeking to improve their mental health do not need psychiatric medication or sophisticated psychotherapy. A typical help seeker needs basic support. For knee pain, a nurse or physician might first recommend icing and resting the knee, working to achieve a healthy weight, and introducing low impact exercise before considering specialist care. Unfortunately, there is no parallel continuum of care for mental health and wellness. As a result, a person seeking the most basic support must line up and wait for the specialist along with those who may have very severe and/or complex needs. Why are there no lower intensity options? One reason is fear and stigma. A thorough assessment by a specialist is considered best practice. After all, what if we miss signs of suicide or potential harm to others? A reasonable question on the surface; however, the premise is flawed. First, the risk of suicide, or threat to others, for those already seeking care, is low. Second, our technical capacity to predict on these threats is virtually nil. Finally, assessment in our current culture of fear tends to focus more on the identification of deficits (as opposed to functional capacities), leading to over-prescription of expensive remedies and lost opportunities for autonomy and self-management. Despite little evidence linking assessment to treatment outcomes, and no evidence supporting our capacity to detect risk for harm, we persist with lengthy intake assessments and automatic specialist referrals that delay care. Before providers and policy makers can feel comfortable letting go of risk assessment, however, they need to understand the forces underlying the risk paradigm that dominates our society and restricts creative solutions for supporting those in need.

Book excerpt: Despite astute critiques and available resources for alternative modes of thinking and practicing, individualism continues to be a dominating and constraining ideology in the field of pastoral psychotherapy and counseling. Philip Rieff was one of the first to highlight the negative implications of individualism in psychotherapeutic theories and practices. As heirs and often enthusiasts of the Freudian tradition of which Rieff and others are critical, pastoral theologians have felt the sting of his charge, and yet the empirical research that McClure presents shows that pastoral-counseling practitioners resist change. Their attempts to overcome an individualistic perspective have been limited and ineffective because individualism is embeddedin the field's dominant theological and theoretical resources, practices, and organizational arrangements. Only a radical reappraisal of these will make possible pastoral counseling practices in a post-individualistic mode. McClure proposes several critical transformations: broadening and deepening the operative theologies used to guide the healing practice, expanding the role of the pastoral counselor, reimagining the operative anthropology, reclaiming sin and judgment, nuancing the particularagainst the individual, rethinking the ideal outcome of the practices, and reimagining the organizational structures that support the practices. Only this level of revisioning will enable this ministry of the church to move beyond its individualistic limitations and offer healing in more complex, effective, and socially adequate ways.

The book draws upon two recent initiatives in the United States: the Tuning process, adapted from a European approach to breaking down siloes in the European Union educational space; and the Degree Qualifications Profile (DQP), a document that identifies and describes core areas of learning that are common to institutions in the US. Many of the examples are drawn from site visit reports, self-reported activities, workshops, and project experience collected by the National Institute for Learning Outcomes Assessment (NILOA) between 2010 and 2016. Book excerpt: Beyond Fragmentation is an inquiry into the development of mission studies in evangelical theological education in Germany and German-speaking Switzerland between 1960-1995. This is carried out by a detailed examination of the paradigm shifts, which have taken place in recent years in both the theology of mission and the understanding of theological education. David Bosch's proposal of an emerging ecumenical mission paradigm is examined with reference to the schools in membership with the Konferenz Bibeltreuer Ausbildungsstatten (KBA). The KBA schools have been greatly influenced by the work of Peter Beyerhaus and the Frankfurt Declaration (1970), and, as such, defend a conservative theological position and resist the challenge of Bosch's mission paradigm shift, the key issue being that of hermeneutics. Ott further explores the emerging new paradigm of theological education in both the Western and Two Thirds World contexts. While the evangelical Bible school movement has historically embodied many of the features of this new paradigm, they nevertheless have jeopardized these by their pursuit of academic accreditation. Ott believes that theological conservatism has caused the KBA schools to resist changes in the areas of contextual and inductive learning. Finally, Ott studies the schools' pattern of change and change-resistance through combining the insights of Thomas Kuhn, Hans Kung and Alasdair MacIntyre to hermeneutical and epistemological issues.
