Book excerpt: Excerpt from Monthly Cyclopaedia and Medical Bulletin, 1908, Vol. 22: Consolidated Much has been written on this subject, yet we can never sound the note of warning too often, especially to those whose experience with the handling of active poisons has been limited. The possible danger of carelessly handling and having vials containing active poisons mixed with other vials containing drugs is great, no matter who handles them, for no one is infallible. The writer, only a few days ago, had the painful experience to administer antidotes to a young physician who had taken fifteen grains of bichloride of mercury, tablet form, in a mistake for lithium tablets. He being of a rheumatic diathesis and his kidneys being somewhat in active, he was in the habit of taking two lithium tablets in a glass of water before retiring. During the day he had bought at the drug store bichloride tablets which were dispensed in a vial which was almost identical with the vial containing the lithium tablets. He placed the vial containing the bioblo ride on the same shelf with the lithium vial, and that evening he dissolved two bichloride tablets in a glass of water, thinking he had the lithium without reading the label, and did not discover his error until he had all of the solution in his stomach. This was, of course, a bad and painful mistake on part of the doctor, but had the bichloride been dispensed in a special poison bottle, he would at once have discovered his error as soon as he touched the bottle. All poisons should be dispensed in rough and colored bottles so that they could be distin guished readily at night by the touch of the hand. The classification adopted in the above tables gives, it is believed, the information submitted in correct manner. Table No. 1, for example, shows that, in the experience Of 35 of the 151 medical men replying, careless prescribing and using of morphine by physicians are among the most common causes of enslavement by this drug. The data in this table furthermore show that in a large majority of cases addiction is due to promiscuous use and indiscriminate sale of these drugs. This is amply substantiated by the experience of sanitariums. It requires only a casual review of these tables to become satisfied that predisposition plays a minor part in habituation. The exciting causes are the great factors. In connection with the replies of the sanitariums, it should be remembered that they are in all probability based on the statements of patients, who are prone to charge some doctor with' carelessness rather than indiscretion and follies on their own part. Book excerpt: Excerpt from Monthly Cyclopaedia and Medical Bulletin, Vol. 4 Symptomatology. - The most prominent characteristic of the "acromegalic dystrophy" is, as we have stated, a progressive enlargement of the extremities. Although the deformities are particularly noticeable in naturally prominent portions of the body, they also involve other regions, such as the skull, face, spinal column and thorax, and are very marked in these regions as well. Before studying them in detail, it seems desirable to sketch roughly the general appearance of the acromegalic patient, - that odd, ungainly and unharmouious creature. His enormous, clumsy hands seem all the more massive from the fact that the forearms have retained their normal proportions. They present a "stuffed" appearance, and terminate in thickened, sausage-like fingers. His broadened feet are mere paws, with toes of exaggerated size. The face is long, the forehead narrow and retreating, and the supraorbital arches enlarged; the eyes often project forward from between the thickened eyelids; the nose stretches out laterally its fleshy alae; the lips are enormous, especially the lower, which is everted; the lower jaw is strongly prognathic; the tongue, unusually large, frequently protrudes from the mouth. This repulsive and beast-like head, bounded laterally by ears of monumental size, is bent forward and set deeply between the shoulders. Though of average stature or above the average, the subject appears partially collapsed; the curvature of his back and the thoracic deformity contribute to his humiliation, which is further accentuated by his torpid and melancholy demeanor. From a distance his appearance is so striking that the diagnosis can be made without detailed inspection. When the deformities are fully developed, all acromegalics bear a strong resemblance, and the adage, "ab uno discs omnes" is here truly applicable. The increased bulk of the hands is often the first change to attract attention. The hands become broader and thicker without augmenting in length. The hypertrophy involves all the component tissues of the part, - bones, muscles, subcutaneous cellular and fatty tissues, and skin. The latter is hard, firm, free of edema, and somewhat darkened in color. The interphalangeal folds, abnormally developed, extend between what may be called wads of flesh, - the "main capitonnce." The thenar and hypothenar eminences are greatly overdeveloped, and the linear grooves of the palm are transformed into deep gutters. The fingers are somewhat flattened from before backward, and are of equal thickness distally and proximally. The thumb measures up to 12 cm. in circumference (Lombroso), the index finger 9 cm., and the medius 10 cm. The nails remain relatively small. They become flattened, turn up at the edges, and show longitudinal striations. In exceptional cases a club-shaped deformity of the fingers, or the presence of nodosities at the interphalangeal joints, has been noted. Notwithstanding the unusual proportions of the acromegalic hand, its functions are generally preserved, complete flexion becoming impossible, however, in eases where the palm is markedly thickened. Aside from the original tuberculin there have been many other prepara tions made Koch himself has given us two: T. R, which contains those toxins of the tubercle bacillus which are not readily soluble in distilled water, and T. E., which is an emulsion of dead tubercle bacilli. The watery extract of tubercle bacilli, von Ruck, is made by pulverizing the bodies of the bacilli in an agate mortar and extracting them with distilled water after the fats have first been removed by extraction with alcohol and ether. Denys' tuberculin is the filtered broth upon which the bacilli have been grown, and differs from Koch's old tuberculin in that it has not been subjected to heat. For certain operations and for certain patients anesthesia by inhalation is undesirable. For operations in the vicinity of the upper respiratory passages, and in certain operations in which the patient is placed in the prone position, anesthesia by inhalation may be either inconvenient or it may interfere with the work of the operator. Again, for certain patients it is desirable to produce unconsciousness without the patient's, knowledge that is being induced, while diseases of the respiratory. System frequently contraindicate irritating inhalations. For these and, perhaps, other reasons, a number of attempts have been made to produce general anesthesia in other-ways than by inhalation. A year had not elapsed after Morton's 'famous demonstration of ether anesthesia before Pirogoff had produced. Anesthesia by the introduction of ether vapor into the rectum. Rectal anesthesia, although revived from time to time, has never become popular, since it is difficult to manage, irritating to the intestinal mucous membrane, and tends to produce undesirable distention of the bowel, and at times even death through the rupture of a previously unrecognized intes tinal ulcer.
