Aussi, une bonne connaissance des répercussions esthétiques des ostéotomies sur le profil cutané facial est nécessaire. Pour l'aider dans cette démarche, différentes études sont réalisées, et des outils de simulation sont à présent disponible afin de mieux appréhender les modifications du profil que le patient peut espérer en bénéficiant d’un protocole orthodontico-chirurgical. D'abord photographiques, puis céphalométriques sur téléradiographies de profils, ces set-up préchirurgicaux offrent de plus en plus de simulations tridimensionnelles, grâce à l’apport de la tomodensitométrie. Après avoir rappelé quelques notions concernant l’esthétique du profil, son évolution selon les époques et ses moyens d'évaluation, nous évoquons l’approche clinique des classes III squelettique, ainsi que les indications et les techniques chirurgicales. A travers une revue de littérature, nous décrivons les répercussions esthétiques des chirurgies de correction de la classe III et abordons enfin quelques moyens de simulation et de prévisions de ces modifications du profil cutané.

Les ombres obtenues à partir de ces st-up sont jugés par des profanes et des orthodontistes. Résultats : - Des similitudes sont trouvées entre les orthodontistes et les profanes. - Le recul mandibulaire isolé est régulièrement considéré comme donnant le meilleur résultat esthétique. - L'avancée maxillaire isolée est peu plébiscitée, et est plus sévèrement sanctionnée par les profanes.- Pas de corrélation entre le score attribué à un type de chirurgie et SNA, SNB, ANB ou GoGN/SN. - Progression dans le classement lorsque le recul mandibulaire est préféré.

Le but de notre travail est d'obtenir une évaluation détaillée des modifications du profil cutané après une phase thérapeutique orthopédique de correction d'une classe III squelettique, afin de répondre aux interrogations suivantes : - quelles sont les modifications du profil cutané qui accompagnent une phase thérapeutique orthopédique de correction d'une classe III squelettique ? - les modifications objectivées par l'analyse des données morphométriques correspondent-elles aux modifications esthétiques perçues par différents opérateurs ? - par conséquent existe-t-il une justification esthétique à la mise en place d'une phase thérapeutique orthopédique de correction d'une classe III squelettique ? Notre étude personnelle porte sur un échantillon de trente patients en classe III âgés de six à douze ans et ayant bénéficié d'une phase thérapeutique orthopédique, sur lesquels trois types d'analyses ont été réalisées : - l'analyse de l'évolution de données morphométriques (données objectives), - l'analyse de l'évolution de l'esthétique faciale (données subjectives), - la comparaison des résultats de ces analyses. Nous avons ainsi montré que les visages après phase thérapeutique orthopédique de classe III sont globalement jugés plus favorablement que ces mêmes visages avant thérapeutique. La réalisation de tests statistiques nous a en outre permis de déterminer les variables morphométriques (squelettiques, dento-squelettiques et cutanées) dont les variations reflètent le mieux l'évolution de l'esthétique faciale.
