Book excerpt: La conciliation médicamenteuse (CTM), associée à l'analyse pharmaceutique, est une pratique à mettre en œuvre pour lutter contre la iatrogénie médicamenteuse. Dans une volonté de développer des activités de pharmacie clinique au sein du CH de Libourne, la PUI a priorisé ses actions dans les services de chirurgie, plus à risque d'erreurs médicamenteuses (EM). Plusieurs activités ont été évaluées en orthopédie. Premièrement, une étude prospective sur la CTM à l'admission a été menée sur 9 mois. Les patients inclus sont les patients ayant effectué un passage par les urgences avant d'être transféré dans le service. Une autre étude a été effectuée sur la CTM de sortie. Les patients inclus sont ceux pour lesquels un relais anticoagulant est nécessaire, sur une période de 6 mois. La deuxième cohorte sont les patients pris en charge pour infection ostéo-articulaire, sur une période de 9 mois. Sur 684 patients inclus, 71% des patients sont conciliés dans les 24 heures, 164 patients ont bénéficié d'une CTM proactive et 382 patients d'une CTM rétroactive. Parmi ces derniers, il a été détecté 2.12 divergences non intentionnelles par patient dont l'omission est à l'origine de 70.9% des divergences. Une proportion de 39.13% de patients sous anticoagulants per os a été conciliée à la sortie. Des EM sont retrouvées chez 10 patients : 36.11% des patients ont une EM. De même, 46,67% de patients sous antibiotiques sont conciliés à la sortie. Des EM sont retrouvées chez 3 patients : 21,43% des patients ont une EM. La pertinence de ce travail est mise en évidence par le nombre non négligeable de divergences retrouvées lors de la CTM. Cette expérience au centre hospitalier de Libourne est encourageante mais des améliorations semblent nécessaires en sortie d'hospitalisation, en particulier une meilleure coordination avec l'équipe médicale. La PUI souhaite renforcer cette activité au sein du service de chirurgie, voir la développer dans d'autres services.

La conciliation médicamenteuse permet, en partie, d'y contribuer. L'implication du pharmacien va au-delà de la prise en charge médicamenteuse et a un véritable rôle à jouer dans l'information du patient sur les dispositifs médicaux. Le service de chirurgie orthopédique et traumatologique nous a paru propice à la mise en place de la pharmacie clinique, tout en l'appliquant aux dispositifs médicaux implantables (DMI). L'objectif de notre étude est ainsi de sécuriser et d'optimiser la prise en charge pharmaceutique du patient par la mise en place de la conciliation médicamenteuse à l'entrée et à la sortie et la dispensation d'informations au patient sur les prothèses orthopédiques de hanche, genou, épaule via la réalisation de livrets d'informations. Ceci nous permettra d'explorer les spécificités du service, les écarts de prescription et d'évaluer les connaissances des patients vis à vis des prothèses orthopédiques via un questionnaire. L'étude a été réalisée sur une période de 5 mois, 161 patients ont été inclus à l'entrée et 39 patients à la sortie. 62,7% des patients ont présenté au moins une divergence non intentionnelle (DNI). Il apparaît que le nombre de DNI augmente de façon significative si l'admission se fait par les urgences. De même, le nombre de DNI augmente avec le nombre de traitements pris au domicile par le patient. La gravité des erreurs médicamenteuses (EM) semble être en lien avec le nombre de DNI. Quant au niveau de connaissance des patients sur les arthroplasties, il apparaît faible. Seul le fait d'avoir des antécédents de prothèse augmente de façon significative le score du questionnaire. Le type de prothèse ou le mode d'admission n'a pas d'impact significatif sur ce score. Cette étude nous a permis d'explorer les spécificités d'un service de chirurgie orthopédique et traumatologique. Les résultats obtenus montrent l'impact positif d'une présence pharmaceutique renforcée, en interceptant des DNI et en prévenant les EM. Le faible niveau de connaissance des patients montre l'intérêt de la démarche d'information des patients sur les DMI.
