Book excerpt: Preclinical experimental transplantation research that is based on microsurgical models in rats fulfills two indispensable conditions for modern organ transplanta tion research: Almost all organ grafts can be performed on the rat with an amount of technical effort that is still justifiable. Thus transplantation models that are analogous to human organ transplantation can be developed, tested, and evaluated. This fulfills a necessary condition from the standpoint of surgery. With the species rat, we have a great variety of genetically different inbred strains. From the immunological point of view this is an indispensable prerequisite for the investigation of preclinical transplantation models that can be expected to produce controllable, reproducible results. In vivo experimental results can be supplemented by and correlated to in vitro tests. Lately these experimental results are being greatly expanded and more precisely defined by the application of immunohistological methods that have been established recently in Kiel. In this book we hope to present a cross section of the microsurgical models in use today and of current immunological and immunohistological models. Furthermore, we wish to record the present state of microsurgical organ transplantation research and to show its relationship to the current state and development of clinical organ transplantation. A special aspect of our Kiel research group is the long-term, well-functioning, interdisciplinary cooperation between surgery, immunology, and pathology. Through this cooperation we attempt to provide an atmosphere in which theoretical and practical viewpoints can mutually influence each other.

Book excerpt: Since the first successful digit replantation in Japan in 1965, the field of microvascular surgery has rapidly progressed throughout Japan and the world. Experimental and Clinical Reconstructive Microsurgery draws on the experience of a large number of experts in the areas of experimental microsurgery, limb and digit replantation, and composite tissue transplantation. The result is an extensive monograph covering the history and future prospects of microsurgery, essential microsurgical techniques for laboratory research, and the fundamental methods of harvesting tissues and their grafting techniques. Because the field of microsurgery includes a broad range of clinical disciplines, this book is a valuable resource to all orthopedic, traumatic, and plastic surgeons with an interest in microsurgery.

This book addresses all aspects of orthopaedic microsurgery, from development of the principles to their applications. Replantation of fingers, hands and extremity parts are well summarized by highly experienced microsurgical surgeons. The methods for tissue transplantation and microsurgical reconstruction of tissue defects, which have been proved to be successful tools for saving severely injured extremities, were developed by experienced orthopaedic surgeons in cooperation with plastic surgeons. Injuries to peripheral nerves and the brachial plexus are also discussed extensively in this monograph. All illustrations and tables were meticulously selected and are easy to understand. The book was written for all microsurgeons who work in the fields of orthopaedics, plastic and hand surgery. Guoxian Pei is a professor at the Department of Orthopaedics, Xijing Hospital, the Fourth Military Medical University, Xi An, China.

Book excerpt: Main focus of the new book will be the description and discussion of rat and mouse models for organ transplantation. Various microsurgical techniques will be presented which allow transplantation of functional organs in syngeneic systems. In particular, the extremely difficult methods necessary for organ transplantation in mice will be presented and evaluated. Besides these practical aspects the book will also cover the theoretical sides of organ transplantation like the immunobiology of allotransplantation. Special emphasis will be given to the resurgent field of xenotransplantation. The results from xenograft models developed in the recent years using rats or mice will be reviewed and their impact on future human xenotransplantation will be discussed.

Available in PDF, EPUB and Kindle. Book excerpt: Mohs Micrographic Surgery, an advanced treatment procedure for skin cancer, offers the highest potential for recovery--even if the skin cancer has been previously treated. This procedure is a state-of-the-art treatment in which the physician serves as surgeon, pathologist, and reconstructive surgeon. It relies on the accuracy of a microscope to trace and ensure removal of skin cancer down to its roots. This procedure allows dermatologists trained in Mohs Surgery to see beyond the visible disease and to precisely identify and remove the entire tumor, leaving healthy tissue unharmed. This procedure is most often used in treating two of the most common forms of skin cancer: basal cell carcinoma and squamous cell carcinoma. The cure rate for Mohs Micrographic Surgery is the highest of all treatments for skin cancer--up to 99 percent even if other forms of treatment have failed. This procedure, the most exact and precise method of tumor removal, minimizes the chance of regrowth and lessens the potential for scarring or disfigurement

Available in PDF, EPUB and Kindle. Book excerpt: For many years, the operative manipulation of the most minute struc tures was frustrated by the limitations of the human eye. With the introduction of the operating microscope by Nylen (1921), fine detailed anatomy was made visible, so that structures could be prepared, pre sented and manipulated. Nylen used the microscope for treating dis eases of the middle ear. In 1946 Perrit in the U.S.A. introduced the operating microscope into the field of ophthalmic surgery. In 1964, Smith, Kunze and Mitchen published reports on their clinical experi ence using the operating microscope in nerve surgery. Using this tech nique, it was possible to divide a peripheral nerve into individual fascicle bundles and reapir each bundle precisely. Following on from these developments, the term" microsurgery" was introduced to cover that branch of surgery which was carried out using the" equiped" eye - either by using 10upe spectacle magnifica tion or, more commonly, the operating microscope. The term "micro vascular surgery" developed for the operative treatment of small ves sels below 2 mm in diameter. Experimental studies for the use of microsurgery in the clinical situation developed alongside its use on small research animals for transplantation models in immunological study. Microsurgical techniques were used in the transplantation of rat liver, kidneys, pancreas, heart and other organs. The clinical appli cation of these microsurgical techniques is nowadays called "recon structive microvascular surgery."

Book excerpt: On April 23, 1906, Alexis Carrel presented his manuscript entitled "The Surgery of Blood Vessels" to the Johns Hopkins Medical Society. His work on the tech nique of small vessel anastomosis and the transplantation of visceral organs earned him the Nobel Prize in 1912. The use of magnification in operative surgery began in 1921 with the work of Nylen, who first employed the microscope for reconstructive procedures in the middle ear. Jacobson and Suarez published their initial observa tions on the use of microsurgery for the anastomosis of small vessels in 1960, and two years later, Malt reported the historic replantation of an upper extremity. To surgeons involved in reconstruction of the extremities, the evolution of micro surgery has provided the most significant advances of the past three decades. The dramatic clinical successes of replantation surgery and free tissue transfer have substantially improved functional and cosmetic results in addition to decreas ing morbidity in patients who present with complex reconstructive problems. Mi crosurgical Reconstruction of the Extremities crosses the traditional surgical discip lines and will be invaluable to orthopaedic surgeons, plastic surgeons, hand sur geons, and general surgeons, as well as to residents and fellows.

Available in PDF, EPUB and Kindle. Book excerpt: This unique publication was written by experts who have made significant contributions to the development of reconstructive microsurgery and vascularized composite allotransplantation and are likely to shape its future. The book is divided into three major sections. The first presents the state-of-the-art of autologous microsurgical reconstruction. It summarizes current achievements, highlights the shortcomings of currently available techniques, and prepares the reader for the next evolutionary step: allotransplantation. Section two is a comprehensive review of allotransplantation, from immunology to surgical techniques. Finally, for those interested in establishing a comprehensive center for vascularized composite allotransplantation, section three provides important lessons from the successful Chang Gung Vascularized Composite Allotransplantation Center. From Auto- to Allotransplantation is indeed the only up-to-date and complete reference available on the topic. Scholars and research fellows interested in transplantation will benefit greatly from this work. It is also an invaluable resource for plastic, orthopedic, hand, ENT, oromaxillofacial, and general surgeons as well as for residents.
