The stroke risk over a five year follow-up period could be reduced by 55 % and the combined stroke and mortality rate was shown to be a mere 2. 3 %. The efficacy of surgical treatment in also reducing stroke rates in the case of a symptomatic carotid stenosis was proven in the NASCET and ECTS prospective randomized studies. Of extreme importance in these procedures is, however, precise quality control and quality assessment. This is presently a topic of tremendous interest in reconstructive vascular surgery and is constantly being discussed in specialist circles and beyond. Documentation and the possibility of accurate reconstruction of the intraoperative situation are in high demand. Perioperative monitoring of neurological function, particularly the monitoring during carotid surgery, are the aspects underlying constant revision and reassessment to ensure quality control and assurance of a negligeable mortality and morbidity rate. Book excerpt: Background and Aims:The perioperative complication rate of carotid stenosis patients should be controlled in fewer than 6% of symptomatic cases and 3% of asymptomatic cases.Therefore, we evaluate various factors and select the surgical option, carotid endarterectomy (CEA) or carotid artery stenting (CAS), whichever is safer. However,in case of long carotid plaque formation, both CEA and CAS could be high risk options.Based on our clinical experience, we developed hybrid surgery combined CEA andCAS and got successful outcome.Methods:We prospectively analyzed 238 consecutive surgical cases in patients with carotid stenosis in New Tokyo Hospital, between January 2016 and December 2018. As our first option for carotid stenosis with short lesion, we choose CEA and for CEA high risk cases including re-stenosis type or long lesion type, we choose CAS.In case of long vulnerable plaque, as unusually high-risk group, we performed hybrid surgery combined CEA for vulnerable plaque part and CAS for distal or proximal part of carotid stenosis at one time.Results:We performed CEA for 185 cases, CAS for 43 cases and hybrid surgery combined CEA and CAS for 10 cases. The perioperative complication rate was 0.8% in total cases including one acute occlusion case and one postoperative bleeding case in wound area of CEA group. During postoperative 30 days, we had no cerebral infarction case or fetal case.Conclusions:We demonstrated the successful surgical outcome of carotid stenosis cases with developing hybrid surgery combined CEA and CAS.

It contains the continuing devel opments of the extra- intracranial arterial bypass (EIAB) for cerebrovascular occlusive disease since the First International Symposium in Lorna Linda, California, in June, 1973, and the Second International Symposium in Chicago, Illinois, in June, 1974. The EIAB was developed by Donaghy and Yasargil, and it consists of a microsurgical anastomosis of the superficial tem poral artery or the occipi tal branch of the external carotid artery to the middle cerebral artery on the surface of the brain and, very recently, the anastomosis of the occipital artery to the posterior-inferior cerebellar artery for the treatment of verte brobasilar insufficiency. From the presentations at the sym posium it became evident that the greatest potential in the pre vention of a stroke in a susceptible patient is the presence of an adequate collateral cerebral blood flow. The basic diagnostic procedure is cerebral angiography in cluding both carotid and vertebral arteries. Regional cerebral blood flow (rCBF) studies and computerized tomography add further diagnostic information and are particularly important in the selection of potential surgical candidates.

Book excerpt: Background and Purpose: We have two surgical methods for the treatment of severe carotid stenosis, although the choice of carotid endarterectomy (CEA) or carotid artery stenting (CAS) has not been established. This report presents appropriate treatment strategy using plaque diagnosis and the clinical results of CEA and CAS. Materials and Methods: After plaque diagnosis was made by carotid ultrasonography (carotid US) and black blood magnetic resonance imaging (BB-MRI), CEA was first choice for patients with soft atherosclerotic plaques and severe calcified plaques. CAS was first choice for patients with stable plaques and plaques at higher cervical level. Fromu3000January 2001 to December 2018, we surgically treated carotid stenosis in 303 lesions by CEA and 337 lesions by CAS. Results: Stenosis of carotid arteries was relieved in all cases after CEA or CAS. Perioperative mortality with CEA and CAS was 0.3uff05 (1/303) and 0.3% (1/337), respectively. Morbidity by ischemic stroke with CEA and CAS was 2.3% (7/303) and 1.5% (5/337), respectively. Surgical morbidity was not high in patients with medical risk factors. Conclusions: Carotid stenotic lesions can be treated with comparably low morbidity and mortality rates using CEA and/or CAS even with high risks, when appropriate surgical methods are selected considering each characteristic of carotid stenosis using plaque diagnosis.
