Despite many attempts to overcome their unwanted side effects through chemical modification of the hemoglobin molecule, none of the potential solutions has been approved for clinical use in Europe or the United States. In recent years, the vasoconstrictive activity of hemoglobin in the plasma was identified as the pivotal problem of hemoglobin-based blood substitutes, compromising nutritional perfusion and thus impeding oxygen unloading at the site of the microcirculation. One of the prevailing assumptions is that the precapillary vasoconstriction and the ensuing tissue underperfusion is caused by the high affinity of free hemoglobin for nitric oxide. To resolve this problem, a number of recombinant techniques involving site-directed mutagenesis as well as several chemical approaches involving polymerization and pegylation have been developed. This volume summarizes the latest research on the effects of some of these new hemoglobin solutions on the microvasculature and tissue oxygenation. It is recommended reading for all those interested in finding alternatives for donor blood in transfusion medicine, including emergency specialists, anesthesiologists, surgeons, trauma surgeons and other clinicians who are frequently confronted with blood loss and the need for blood replacement.

This symposium, held in Heidelberg in January 1980, was planned not only for the exchange of concepts and ideas, but was expected to be in addition partially a teaching session; the basic scientists should be directed toward a better under standing of the clinical problems, and the clinicians should learn more about the basic mechanisms regulating substrate and ion exchange in such an impor tant organ as the heart, and furthermore the theoretical limitations of some of the diagnostic and therapeutical procedures should be taught. Without a lot of help we would never have succeeded in organizing the sym posium and editing its results.

The concept is not new; however, its application has been limited because of lack of interest and willingness to make the extra effort required. Also there has been concern about the safety of the procedure. Homologous blood transfusion is easy and has allo wed us to salvage lives and perform procedures which would not have been feasible without the availability of whole blood or its components. Transfusion with homologous blood products is safe; however, com plications can and do occur. The transmission of AIDS by transfusion of homologous blood products has significantly increased patient and physician awareness of the dangers one faces. This potential complica tion has promoted autotransfusion. It is now commonly used and reques ted by patients and physicians. Blood for autotransfusion is collected pre or intraoperatively by phle botomy and retrieved intra and post operatively from sites of hemor rhage. Once collected, the blood is either stored or returned to the patient as whole or processed blood. To have a successful autotransfusion program requires anticipation of its needs and coordination and training of involved personel in the blood bank, emergency room, operating room and post-surgical care units. Pre deposit and processing of collected blood requires remission of donor criteria and labeling and storage procedures.

Book excerpt: Disturbances in peripheral O extraction can be produced in dogs treated with 2 endotoxin and thereby provide an opportunity to test theories for the origin of pathological O supply dependency or to try different treatment modalities. The 2 most serious deficiency in the current animal models is the inability to mimic the increased O demand that is observed in patients at 02 delivery rates in excess of 2 normal. A particular feature of this increased O demand is that it apparently does 2 not stimulate increased 02 extraction, although the limitation in O extraction has 2 not been explored in patients by lowering 02 supply, for obvious reasons. At least two possibilities to account for increased 02 demand could be investigated in animal models, however. The amount of 02 that is utilized in extramitochondrial pathways, which is normally on the order of 10%, may be greatly increased in ARDS and sepsis by O radical formation. There is presently no information 2 concerning how much 02 might be used in this way. Another strong possibility is that mitochondrial injury, perhaps as a result of 02 radical formation, uncouples oxidative phosphorylation. Some evidence presently in the literature supports this idea [19]. Indeed, the association of increased blood lactate levels with higher than expected 02 demands makes uncoupling a very attractive hypothesis that warrants further investigation in animal models using such agents as 2,4-dinitrophenol.
