Available in PDF, EPUB and Kindle. Book excerpt: OECD reports have become influential in Mexico. The “2016 OECD Review of Health Systems, Mexico” is analyzed in this note from an industrial perspective. Health reform has been debated in Mexico for decades. However, the map of suppliers and insurers, as well as of costs and rules of access is the same than in past decades. Law and regulation determine the hard vertical integration of public agencies, from financing to provision of services. In consequence, Mexican health insurance and service markets are segmented. Buyers cannot identify and access low cost sources of services and families are tied to a specific public insurer. A result is the extensive use of private services to overcome waiting times offered by public providers. So far, public policy has tried to face the problem through centralized mechanism, an approach supported by the OECD. Probably, a decentralized approach that allows the revelation of costs and value of needs will be needed to overcome the costs of fragmentation of the system.

Book excerpt: In this book the authors explore the state of the art on efficiency measurement in health systems and international experts offer insights into the pitfalls and potential associated with various measurement techniques. The authors show that: - The core idea of efficiency is easy to understand in principle - maximizing valued outputs relative to inputs, but is often difficult to make operational in real-life situations - There have been numerous advances in data collection and availability, as well as innovative methodological approaches that give valuable insights into how efficiently health care is delivered - Our simple analytical framework can facilitate the development and interpretation of efficiency indicators.

Book excerpt: ?This academic but passionate and controversial work should be read by specialists on Mexico and Latin America, as well as by those interested in healthcare and social policy in general.??Carmelo Mesa-Lago, University of PittsburghHas Mexico, twenty years after beginning the process of decentralizing its health system, realized the anticipated benefits of increased community participation and improvements in efficiency and quality? Addressing this question, Decentralizing Health Services in Mexico presents a thorough historical and theoretical grounding, as well as representative case studies of decentralization at the state and local levels. The authors combine qualitative and quantitative data in their examination of the transfer of authority over fiscal, human, and physical resources in the health sector. The result is a major contribution to the ongoing debate over the advantages and disadvantages of decentralization in varying political, cultural, and economic contexts.Nuria Homedes is associate professor at the University of Texas School of Public Health-Houston.
