L'entrée dans les soins est souvent tardive ; la durée moyenne de psychose non traitée s'élève à deux ans. La stigmatisation atteignant les personnes souffrant d'un trouble psychotique est un enjeu important de santé publique, du fait de ses multiples conséquences altérant la qualité de vie. Objectif : Notre étude a donc pour objectif principal de mettre en avant les éléments clés utiles à l'élaboration d'une intervention de lutte contre la stigmatisation des troubles psychotiques. Ce travail a pour objectif secondaire d'étudier les liens entre ces campagnes et la durée de psychose non traitée (DUP). Méthode : Une revue systématique de la littérature a été réalisée, incluant des articles issus de la littérature internationale et portant sur l'efficacité des interventions de lutte contre la stigmatisation des troubles psychotiques (chez les personnes indemnes de trouble psychotique au moment de l'étude). Résultats : Parmi les 1364 articles identifiés sur Pubmed, la lecture des abstracts a permis de repérer 41 articles (parus entre 2003 et 2023) répondant aux critères d'inclusion et lus en intégralité. L'analyse de ces articles nous a permis de repérer différents éléments-clés pour une intervention efficace. Ainsi, il faut privilégier l'association de différentes stratégies (éducation, contact social, protestation). Les méthodes éducatives doivent s'appuyer sur le concept de continuum en santé mentale, et éviter les informations sur l'origine biogénétique du trouble. Les vidéos brèves et témoignages (contact indirect) sont efficaces ; il est essentiel d'inclure des messages porteurs d'espoir axés sur le rétablissement. Il existe des résultats encourageants pour certaines modalités de contact indirect (théâtre, films, contact imaginé), et pour les méthodes éducatives centrées sur des concepts étiologiques psychosociaux. Les preuves sont insuffisantes quant aux interventions employant la réalité virtuelle ou uniquement la protestation. Concernant les liens entre DUP et campagnes anti¬stigmatisation des troubles psychotiques, certains programmes montrent que l'emploi de stratégies de déstigmatisation (éducation et contact essentiellement) est associé à une réduction de la DUP. Discussion/Conclusion : Concernant les limites de ce travail, nous notons une hétérogénéité importante de la qualité des articles inclus. De plus, les études s'intéressent peu aux ressentis et aux changements constatés par les personnes souffrant de trouble mental. Les liens de causalité entre DUP et stigmatisation nécessitent des analyses plus approfondies. D'autres pistes sont également à investiguer, comme le changement de dénomination de la schizophrénie et la place de l'auto-stigmatisation dans ces campagnes. Pour conclure, notre revue permet d'établir des recommandations innovantes pour mener une campagne de lutte contre la stigmatisation des troubles psychotiques.
