Book excerpt: The management of and attitudes toward children and adults with Down syndrome have undergone considerable changes in the course of the condi tion's long history (Zellweger, 1977, 1981, Zellweger & Patil, 1987). J. E. D. Esquirol (1838) and E. Seguin (1846) were probably the first physicians to witness the condition without using currently accepted diagnostic designa tions. Seguin coined the terms furfuraceus or lowland cretinism in contradis tinction to the goiterous cretinism endemic at that time in the Swiss Alps. Esquirol, as well as Seguin, had a positive attitude toward persons who were mentally ill or mentally subnormal. Esquirol pioneered a more humane treatment in mental institutions and Seguin created the first homes in France, and later in the United States, aimed at educating persons who were mentally subnormal. The term mongolian idiocy was coined by J. H. L. Down in England (1866). The term is misleading in several respects: (1) Down identified the epicanthic folds seen in many children with Down syndrome with the additional skin fold in the upper lid occurring particularly in people of Oriental (Mongolian) descent; and (2) Down also erred by assuming that Down syndrome represented regression to an ethnic variant of lower cultural standing. Such an interpretation might have been understandable at a time when the myth of Anglo-Saxon superiority was widely accepted by the British. Charles Darwin's then highly acclaimed theory of origin of the species may have contributed to such a concept.

This book was released on 2009-12-04 with total page 529 pages. Available in PDF, EPUB and Kindle. Book excerpt: For nearly fifteen years Practical Decision Making in Health Care Ethics has offered scholars and students a highly accessible and teachable alternative to the dominant principle-based theories in the field. Devettere’s approach is not based on an ethics of abstract obligations and duties, but, following Aristotle, on how to live a fulfilled and happy life—in short, an ethics of personal well-being grounded in prudence, the virtue of ethical decision making. This third edition is revised and updated and includes discussions of several landmark cases, including the tragic stories of Terri Schiavo and Jesse Gelsinger (the first death caused by genetic research). Devettere addresses new topics such as partial-birth abortion law, embryonic stem cell research, infant euthanasia in The Netherlands, recent Vatican statements on feeding tubes, organ donation after cardiac death, new developments in artificial hearts, clinical trials developed by pharmaceutical companies to market new drugs, ghostwritten scientific articles published in major medical journals, and controversial HIV/AIDS research in Africa. This edition also includes a new chapter on the latest social and political issues in American health care. Devettere’s engaging text relies on commonsense moral concepts and avoids academic jargon. It includes a glossary of legal, medical, and ethical terms; an index of cases; and thoroughly updated bibliographic essays at the end of each chapter that offer resources for further reading. It is a true classic, brilliantly conceived and executed, and is now even more valuable to undergraduates and graduate students, medical students, health care professionals, hospital ethics committees and institutional review boards, and general readers interested in philosophy, medicine, and the rapidly changing field of health care ethics.

Available in PDF, EPUB and Kindle. Book excerpt: The skills required by children’s nurses are many and varied, and are undertaken in a variety of settings. Sick children are cared for at home, in children’s units and hospitals, and sometimes in adult wards. In addition, children with disabilities may need nursing care while at special schools or in residential settings. This book provides a clinical manual of common practices. Each practice is research-based and presented in a common format which covers: Objectives Rationale Factors to note Guidelines Positioning Equipment Method Special observations and possible complications References and further reading The guidelines have been written specifically for children’s nursing by contributors from well-known children’s units. They are not simply adaptations from adult practices and therefore recognise the unique differences between adult and children’s nursing. The introductory section covers common concepts and infection control. There is also a chapter on complementary therapies and an appendix on the use of play as a distraction. All hospital and community nurses dealing with children, whether at home, in specialised units or in community settings, will find the book of great practical help. The common format of each practice makes the book easy to use and refer to in a clinical setting. Each practice is based on the latest research to ensure it is evidence-based and up to date Introductory chapters cover issues common to all practices to make the book easier to use. The appendix on play reinforces its common usage as a distraction technique A chapter on complementary therapies explains these as they are becoming more widespread All practices have been reviewed by regional centres to avoid parochialism Content reorganised to reflect the Essential Skills Clusters from the NMC New chapter on communicating with children All chapters updated to reflect the current evidence base
