Book excerpt: For decades, the American Cancer Society (ACS) explicitly forbade acceptance or use of taxpayers' funds from government at any level. However, as public support for programs began to diminish and revenue growth leveled off, the ACS reversed this policy. It now actively seeks taxpayers' funds. In this sense it reflects a model of how America's major health charities are abandoning their traditional goodwill purposes and becoming political organizations. As donors become disenchanted, the charities view the taxpayer as an alternative, and far more reliable, source of funds and devote their political activity to raising taxes and earmarking the increased revenues for themselves. Health charities subsequently lose their independence as the distinction between government and private charities becomes blurred. CancerScam investigates Project ASSIST, the joint undertaking between the American Cancer Society and the National Cancer Institute (NCI). CancerScam details the charities' collaborative efforts to divert millions of dollars in federal cancer funds--under the guise of improving the public health through reducing smoking--to build political coalitions. Bennett and DiLorenzo suggest that the antitobacco campaign is a smokescreen for raising taxes on tobacco and earmarking the increased revenues for the financial benefit of ACS and its allied charities. CancerScam reveals how concern about the AIDS lobby's success in obtaining scarce research funds motivated the NCI to build political coalitions at the grass-roots level which could lobby for federal funding of cancer research. Bennett and DiLorenzo believe that public support of the ACS will be undermined when its emphasis on politics becomes better known and its reputation erodes as it is perceived as little more than an extension of government, subject to bureaucratic regulation and loss of independence. It is a brave effort that brilliantly shows how government bureaucrats steal funds intended for the highest public purposes and use them for narrow political advancement. Geary shows how illness and poverty reacted upon each other. The poverty resulting from great population growth that continued until the arrival of potato blight in 1845 had a severe effect on the health of the country's population, and the Famine itself caused around one million deaths from starvation and disease. This was a period of great change in medical and charitable services. In the eighteenth century the sick had come to be regarded as the deserving poor, therefore having a better claim to public assistance than those whose poverty was the result of their own dissipation, idleness or vice. A network of charities evolved in Ireland to provide free medical aid to the sick poor. The first voluntary hospital in Dublin opened in 1718 and Geary traces the establishment and development of voluntary hospitals and county infirmaries throughout the country.These had a strong Anglican ethos and bias, but after Catholic emancipation in 1829 the nepotism, sectarianism and divisive politics that were rife in these organisations came under increasing scrutiny. Medical practitioners saw considerable progress in the development of a regulated profession. Geary describes developments in policy making and legislation, culminating in the 1851 Medical Charities Act, which he describes as part of a process that characterised the century and more under review in this book: the unrelenting pressure on philanthropy and private medical charity and the inexorable shift from voluntarism to an embryonic system of state medicine.

Book excerpt: The Clinton administration's failed health care reform was not the first attempt to establish government-sponsored medical coverage in the United States. From 1915 to 1920, Progressive reformers led a spirited but ultimately unsuccessful crusade for compulsory health insurance in New York State. Beatrix Hoffman argues that this first health insurance campaign was a crucial moment in the creation of the American welfare state and health care system. Its defeat, she says, gave rise to an uneven and inegalitarian system of medical coverage and helped shape the limits of American social policy for the rest of the century. Hoffman examines each of the major combatants in the battle over compulsory health insurance. While physicians, employers, the insurance industry, and conservative politicians forged a uniquely powerful coalition in opposition to health insurance proposals, she shows, reformers' potential allies within women's organizations and the labor movement were bitterly divided. Against the backdrop of World War I and the Red Scare, opponents of reform denounced government-sponsored health insurance as "un-American" and, in the process, helped fashion a political culture that resists proposals for universal health care and a comprehensive welfare state even today.
