All rights not granted by the above license are retained by the author. Abstract: Inadequate bone volume on the posterior maxilla limits dental implant rehabilitation. Maxillary sinus grafting is often indicated to increase the bone volume in this region. Bone marrow aspirate concentrate (BMAC), a cellular-based therapy, has been recently introduced as a chair-side procedure. The overall aim of this thesis is to develop a single-visit clinical stem cell therapy for maxillary sinus bone grafting in atrophic maxilla using bone marrow derived MSC from BMAC. The study aimed to evaluate the quantity of nucleated cells, MSCs and their differentiation capacity in the bone marrow aspirate and BMAC in rabbits and humans. The bone regeneration and healing of maxillary sinus graft using different grafting materials in combination with BMAC were evaluated in rabbits and humans. Materials and methods involved an in vitro laboratory experiment, an in vivo animal experiment and a preliminary human clinical trial. The bone marrow aspirates from the rabbits and humans were submitted for the quantitative and qualitative analyses of the nucleated cells and the differentiation capacity of the MSCs before and after concentration. The rabbit maxillary sinus model was used to test the different grafting materials. 24 rabbits with 48 sinuses were randomized into 4 groups: sham control, autogenous bone, bovine bone mineral (BBM) and BBM+BMAC. The rabbits were sacrificed at 2, 4 or 8 weeks and the sinuses were analyzed by histological examination and micro-CT scan. In the human clinical trial, the patients were divided into 2 groups basing on their grafting need for dental implant rehabilitation. On bilateral sinus grafting (study 1), it was a split mouth design comparing BBM and BBM+BMAC and on unilateral sinus grafting (study 2), it was a randomized controlled design comparing BBM+BMAC and autogenous bone. The bone regeneration capacity was assessed histologically by trephine biopsy and cone beam CT scans for graft volume and graft height. Results of the in vitro study confirmed that the quantity of nucleated cells and colony forming unit-fibroblasts after concentration have increased substantially when compared with before concentration in rabbits at a factor of 1.84 and 3.76, respectively, and in humans at a factor of 4.43 and 106.88, respectively. The BMAC could retain their in vitro multi-differentiation capability of osteogenic, chondrogenic and adipogenic lineage. In the in vivo rabbit study, bone regeneration and satisfactory healing of maxillary sinus grafts using either autogenous bone, BBM or BBM+BMAC was confirmed. The quantity of new bone increased and demonstrated interconnection with no sign of inflammatory changes. BBM or BBM+BMAC retained more graft volume rather autogenous bone. In the preliminary clinical study, bone-aspirating technique did not produce any morbidity and higher patients' satisfaction when compared with hip harvesting. On evaluation of bone healing after maxillary sinus grafting, both BBM and BBM+BMAC underwent similar trend of slow graft reduction. The BMAC+BBM could retain greater volume than autogenous bone. In conclusion, a single-visit stem cell therapy for maxillary bone grafting in atrophic maxilla using BMAC+BBM have been developed based on in vitro laboratory and in vivo rabbit experiments and substantiated by a preliminary clinical trial. Book excerpt: Purpose : The Purpose of this study is to show the total survival rate of implants with maxillary sinus grafting and the effects that reach the survival rate by classifying types of graft materials, implant type, operation method, residual bone height and evaluate graft material resorption rate after sinus graftingMaterials and methods : 61 dental implants placed with sinus bone grafting in 24 patients at Wonkwang University Sanbon Dental Hospital were installed simultaneously or after regular healing.Various bone grafts (autograft, xenograft, allograft, alloplast) and fourth implant type(GSII, Xive, Implantium, Novel biocare) were used. All implants were investigated clinically and radiographically, being with average 20 months follow-up period after installation.Results : 3 fixtures were lost, resulting in 95.1% cumulative survival rate of 61 osseointegrated dental implant. Survival rate according to bone material type, Implant type, operation method, residual bone height, have no statistically significant differencies.The mean preoperative residual alveolar bone height was 4.75mm, average postoperative height of graft materials 10.8mm, vertical bone resorption rate was 10% after 2 years. Resorption rate according to operation method was 7%(simultaneous) and 5%(delayed) after 1 year.It can be suggested that maxillary sinus grafting may have predictable result with various bone graft materials and implant type, residual bone height, operation method.

However, it is still controversy regarding what grafting material is the most suitable.Aim: To compare the potential of platelet-rich fibrin (PRF) and deproteinized bovine bone mineral (Bio-Oss) in augmenting bone volume after maxillary sinus lift with simultaneous implant placement.Material and Methods: 37 patients with a total of 40 sites of maxillary sinus lift surgery were included in this study. The 40 sites were randomly divided into two groups, the PRF group and the Bio-Oss group. All sites underwent transalveolar maxillary sinus lift using a CAS kit. PRF was placed into the elevated sinus cavity in the PRF group. while Bio-Oss was placed in the Bio-Oss group. Dental implants were installed simutaneously. Cone beam computed tomography (CBCT) scan was performed preoperatively, immediately after surgery and 6 months after surgery to measure Schneiderian membrane thickness (SMT) , residual bone height (RBH), sinus width (SW), implant protrusion (IP), the height of sinus lift and intra-sinus bone gain. The integrity of Schneiderian membrane was checked by CBCT.Results: In the control group, length and diameter of implants were 10u00b10mm and 4.57u00b10.31mm, respectively, and the corresponding values in thePRF group were 10u00b11.19 mm and 4.59u00b10.39 mm, respectively. In the control group, Schneiderian membrane thickness, sinus width and residual bone height were 1.02u00b11.01 mmuff0c13.40u00b12.38 mm and 5.83u00b11.69 mm, respectively, and the corresponding values in the PRF group were 2.52u00b13.49 mm, 13.51u00b12.99 mm and 6.69u00b11.42 mm, respectively. In the control group, implant protrusions, the height of the maxillary sinus lift and bone gain were 3.44u00b10.787mm, 7.56u00b13.37mm and 4.65u00b12.65mmuff0crespectively, and the corresponding values in the PRF group were 3.44u00b10.787mm,7.56u00b13.37mm and 4.65u00b12.65mmuff0crespectively. No significant difference was detected between the two groups for all the tested parameters. Statistical analysis showed that bone gain was related to the height of sinus elevation, but not strongly related to age, SMT, sinus width (SW), RBH and implant protrusion (IP).Conclusion and Clinical implications: Both materials were beneficial for new bone formation after maxillary sinus elevation and the higher the maxillary sinus lifted, the more bone volume was gained.
