Book excerpt: Abstract: Expressed Emotion (EE) describes emotional attitudes of family members toward one another, including criticism (critical EE) and emotional overinvolvement (EOI), and has been shown to predict outcome in adult psychiatric disorders. The current study investigated the stability of maternal EE from kindergarten to second grade and the role of maternal EE at kindergarten as a predictor of second grade child functioning in a large community sample of youth, oversampled for risk. First, maternal EE stability over time was examined, including stability of overall maternal EE, critical EE, and EOI, gender as a moderator of maternal EE stability, child functioning as a predictor of maternal EE stability, and maternal EE stability as a predictor of subsequent child functioning. Second, kindergarten maternal EE, critical EE, and EOI were examined as predictors of subsequent child functioning, and gender moderation was explored. Mother-child dyads (n=199) were evaluated when the children were in kindergarten and in second grade. Maternal EE was assessed using both categorical and dimensional ratings from the Five Minute Speech Sample (FMSS); children's internalizing, externalizing, and overall symptoms were evaluated with the Child Behavior Checklist (CBCL and CBCL-TRF); and children's social competence was measured using Adaptive Social Behavior Ratings (ASBR). Findings in relation to EE stability were that overall EE, critical EE, and EOI exhibited modest yet significant stability during this developmental period and gender moderated dimensional EE and critical EE stability, suggesting greater instability for the mothers of boys than girls. Further, EE stability predicted second grade CBCL Externalizing scores for boys but not girls. Findings regarding maternal EE as a predictor of child outcome were that kindergarten maternal EE and critical EE (but not EOI) predicted second grade child CBCL Total Problem, Externalizing, Internalizing (at the trend level for critical EE), and ASBR scores and gender moderated some of these relationships. However, after controlling for initial child functioning, maternal EE and critical EE were less strong predictors of child outcome. This study suggests that earlier maternal emotional attitudes may affect children's symptoms and functioning later in development and that their impact may differ according to the gender of the child.

Book excerpt: The purpose of this study was to better understand the relationship between drug exposed infants and their mothers and how this relationship may be associated with mental health outcomes for the child. Hypotheses predicted that drug exposed infants and their mothers would have more relationship impairment and a higher incidence of relationship disorders than non-drug exposed infants and their mothers. It was also hypothesized that greater relationship impairment would be associated with worse adaptive social behavior and more emotional and behavioral problems for children. A nested case-control design was used, involving secondary analysis of data collected from a larger prospective cohort study. Two measures were used to assess relationship problems: the Parent-Infant Relationship Global Assessment Scale and a Videotape Rating Scale for Relationship Disorders that was developed as part of this study. Both measures were based upon Axis II of the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood. These measures were used to code videotaped, feeding interactions that occurred of the infant in the home at 3 months of age. When the child was 2 years of age, adaptive social behavior was measured with the Vineland Adaptive Behavior Scales and emotional and behavioral problems were assessed using the Child Behavior Checklist. Results indicated that, in general, drug detailed comparison of drug exposed and non-drug exposed dyads indicated a higher incidence of specific relationship disorders among drug exposed dyads. The most prominent relationship problems were over-involvement and a mixed relationship disorder that included a gestalt of neglect, anxiety and anger/hostility. Relationship impairment was associated with better adaptive social behavior by the child in domains such as daily living skills and socialization. This finding suggests that children in more dysfunctional relationships may learn to readily comply with their mother's demands to avoid feared consequences and to become more self-sufficient in the face of maternal neglect or insensitivity. Relationships that involved anger and hostility predicted more depression, anxiety and withdrawal for children at age two.

Book excerpt: The construct of emotion regulation is defined as the process of initiating, modulating, and sustaining emotional states and expressions and is important for successful social and emotional development. A child's emotion regulation skills are influenced by numerous factors, including parenting characteristics such as sensitivity, intrusiveness, and responsiveness. Based on evidence of early social deficits, children with very low birth weight may be at risk for delayed or impaired social competence and emotion regulation skills, and research suggests parents of these children have more difficulty perceiving behavioral and affective cues from their children. The current study explored child emotion regulation, maternal behavior, and mother-child interactions in a sample of 3- and 4-year-old children with very low birth weight compared to a normal birth weight control group on two laboratory tasks designed to elicit child frustration and disappointment. In addition, mothers completed questionnaires on child behavior and temperament and parent characteristics. The observational tasks were videotaped and coded using the Parent Child Interaction System. Findings revealed that for one of the tasks, children with very low birth weight were less expressive than the normal birthweight controls. For one task, greater maternal sensitivity was predictive of less autonomous coping and more child negative affect, maternal responsiveness was associated with less autonomous behavior and greater child expressiveness, and maternal positive content was associated with less autonomous coping, though these findings did not vary by group. Results are interpreted in the context of the demands placed on the child and mother, and the need for future research is emphasized.

Book excerpt: Depression is a widespread condition affecting approximately 7.5 million parents in the U.S. each year and may be putting at least 15 million children at risk for adverse health outcomes. Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.
