Book excerpt: Il sistema sanitario italiano è ancora immerso in un processo di trasformazione, del quale non si intravede il termine e soffre di contraddizioni, che determinano incertezza sia tra i pazienti che tra i suoi operatori. La stretta congiunturale ha spinto gli organi di governo a porre ulteriori freni alla spesa pubblica, ma nessuno dei protagonisti del sistema vuole cedere alcuna delle conquiste socio-sanitarie acquisite nell’ultimo trentennio. Ciononostante, il sistema regge e gli indicatori dimostrano che il complesso dei servizi erogati continua a soddisfare i bisogni principali del singolo e della popolazione. Permangono però condizioni critiche che vanno superate e cioè: diffusione di farmaci innovativi ad alto costo;costante ondata migratoria e ricomparsa di malattie infettive;diseguale capacità di accesso alle cure sanitarie;senso di insoddisfazione degli operatori sanitari;crisi di competenze sia a livello centrale che periferico;difficoltà di comunicare le politiche sociali e sanitarie. Necessita quindi implementare la governance sanitaria intesa come sistema attraverso cui i governi possono indirizzare e modulare l’offerta di servizi sanitari, mantenendo elevati standard etici, clinico assistenziali, adattando gli stessi al contesto culturale-socio-economico di riferimento e alle risorse che lo stesso contesto è in grado di esprimere. Questo si propone il Manuale, elaborato grazie ai contributi di oltre cinquanta co-autori (esperti dei vari argomenti ed operatori sanitari impegnati ai più alti livelli gestionali ed operativi) appartenenti a diverse regioni italiane e che si concretizza in un compendio, che aiuti a districarsi in un groviglio di leggi e regolamenti, tagli lineari e concessioni incomprensibili, spinte idealistiche e norme anti-corruzione: un utile strumento per chi voglia capire ed approfondire, sia che si trovi ai primi rudimenti della sanità sia che abbia già sperimentatole asprezze e le incomprensioni della burocrazia della salute.
