Book excerpt: Background: Autogenous bone is still considered one of the most predictable materials for reconstruction procedures in implant dentistry. Many bone-harvesting protocols have been described in literature and mandibular ramus represents one of the most common location to harvest autogenous bone. Aim: The aim of this study is to evaluate the anatomical characteristics of the posterior region of the mandible (mandibular ramus and external oblique line) and the surgical relevance of these anatomical characteristics related to the bone harvesting procedures. Methods and Material: 50 retromolar sites providing from mandibular CTCBs of patients (18 males, 32 females) which should be treated with mandibular autogenous bone harvesting and alveolar ridge augmentation were analyzed considering the donor site anatomies. For each retromolar site, linear measures in cross sectional CT scans were taken to record perpendicular distances between the mandibular canal and the external bone walls. The mandibular canal lenghts were also recorded. Results: The mean value of mandibular canal lenghts was 64,1mm u00b1 3,96 (range: 55,0 - 70,0) and the median was 64,6 mm. Linear measures showed a characteristic trend: the distance from the mandibular canal and the vestibular bone wall is lower in the retro molar area than in the molar area, reaching values under 2 mm in 26% of cases. Although the distance between the mandibular canal and the crestal bone wall is higher in the retro molar area than in the molar area. Conclusions and Clinical implications: There is less thickness of bone in the distal ramus area where the posterior vertical osteotomy of some bone harvesting procedure is performed. This could expose the inferior alveolar nerve to damage.

One of the anatomical factors affecting the stability of the anchor screws is the bone characteristics (shape, bone mineral density, cortical bone thickness). It is necessary to understand anatomical factors in the area of placement with preoperative diagnostic imaging.AimIn this study, we measured and evaluated the buccal cortical thickness of the mesio-distal interdental space at the mandibular first molar with multi-detector low computed tomography (MDCT) images of jaw deformity patients.Materials and MethodsMDCT images of 28 jaw deformity patients participating in the study were acquired and used for the preoperative planning of sagittal split ramus osteotomy (SSRO) surgery. All patients had been previously diagnosed as having an Angle class III malocclusion. After acquisition of MDCT data, all data were transferred to a workstation and evaluated using Zio Cube software. The standard plane of all MDCT data was set at the occlusal plane.The cortical thickness on the mesial and distal part of the first molar were measured on the reconstructed cross-sectional images at the level of 5 and 10 mm inferior to the cement-enamel junction (CEJ) on each mesial and distal side. On the same images, the inclination of the mandibular bone was also recorded. The MDCT images were excluded from this study if: (1) Pathological findings were seen in the posterior mandible; and (2) The images were found to exhibit insufficient image quality due to metal artifacts from acquisition or patient movement.ResultsInitially, all MDCT images were screened. From these, 5 were excluded according to the exclusion criteria. Thus, a total of 46 sides from 23 patients were used in the analysis. The 23 patients consisted of 15 females and 8 males with a mean age of 22.9 years (range from 18u201333 years). The average thickness at the 5 mm and 10 mm level form the CEJ on the mesial portion of the first molar were 1.44 mm and 1.91 mm respectively, and on the distal portion were 1.80 mm and 2.68 mm respectively. Statistical differences were confirmed for buccal cortical thickness at 5 mm and 10 mm from the CEJ in both the mesial and distal portions of the mandibular first molar. The average lingual inclination of the mandible was by 80.5u00b0 n the mesial and 71.2u00b0 in the posterior portion. ConclusionA detailed understanding of the anatomical characteristics is absolutely critical in maxillofacial surgical procedures, including the placement of anchor screws. To reduce postoperative discomfort, the clinician should be aware of each patientu2019s morphological characteristics.

Book excerpt: Abstract: The purpose of this study is to evaluate a protocol that can be used to facilitate treatment planning in the harvest of a mandibular ramus onlay graft using a panoramic radiograph and a defined surgical method. The validity and reliability of the protocol will be tested as it relates to the area and volume of bone harvested. Additionally potential mandibular nerve morbidity will be examined. The protocol is intended to provide the oral and maxillofacial surgeon, regardless of experience level, with an accurate tool when treatment planning the reconstruction of an alveolar defect using a mandibular ramus onlay graft. Eleven human cadaver mandibles were obtained and panoramic radiographs were taken of the mandibles. Two experienced and two inexperienced oral and maxillofacial surgeons predicted graft area and volume using a specific tracing protocol based on landmarks present on a panoramic radiograph. Twenty ramus grafts were harvested from the eleven cadaver mandibles. Area and volume were measured and the integrity of the inferior alveolar nerve was examined. Validity for bone area and bone volume was assessed using linear regression with the protocol value (a mean of both predictions) and surgeon experience as the independent variables and actual bone area and volume respectively as the dependent variables. Reliability for bone area and bone volume was assessed using the intra-class correlation coefficient and 95% confidence interval. Coefficients were generated for the experienced, inexperienced and combined data sets. The protocol did not accurately predict the area or the volume of the ramus grafts. Experienced surgeons exhibited moderate reliability and novice surgeons exhibited poor reliability using the protocol. Five of the harvest sites had evidence of inferior alveolar nerve exposure. One of the harvest sites showed transection of the inferior alveolar nerve. The average graft size was 10.9 mm by 22.0 mm with an average volume of 878 mm3. The prediction protocol did not exhibit a satisfactory level of reliability or validity, however several clinically relevant findings were noted.
