Résultats : Les résultats montrent que la place des généralistes apparait bien plus importante et diversifiée que ne pourrait laisser supposer la faible proportion d'informations préoccupantes émanant des médecins libéraux reçues par le SODED, la cellule de recueil des informations préoccupantes de l'Hérault. Discussion : L'analyse de leurs expériences et de leurs ressentis a permis d'identifier trois profils de médecins. Le médecin qui est impliqué et satisfait de son rôle même s'il est source de difficultés. Le médecin qui perçoit l'importance de son rôle mais dont les difficultés éprouvées l'empêchent de le mettre pleinement en pratique. Le médecin qui fait le choix de ne pas s'impliquer particulièrement dans les différents champs de la protection de l'enfant en dehors des situations graves de danger immédiat. Leur bonne connaissance des facteurs de risque sociaux familiaux et des signes d'alerte en font des acteurs clés de la prévention et de la détection de la maltraitance du petit enfant. Améliorer leurs connaissances et la collaboration avec les acteurs de la protection de l'enfance, judicaire et départementale, pourrait permettre aux médecins généralistes qui le souhaitent, d'occuper une place privilégiée dans la prise en charge de ces enfants aux parcours de vie difficiles.

Objectif : mettre en évidence des stratégies de repérage des maltraitances infantiles reproductibles en cabinet. Méthode : une étude qualitative a été menée par le biais d'entretiens semi-dirigés auprès de quatorze médecins généralistes des Landes ayant réalisé au moins une information préoccupante pour suspicion de maltraitances infantiles. Résultats : ce travail propose des axes d'amélioration des pratiques à partir des stratégies adaptatives développées par les praticiens interrogés. Parmi ces dernières, différents sous thèmes ont émergé : utiliser des messages de prévention, créer un refuge pour l'enfant en amont de toute violence, identifier les signes d'alerte, se positionner en tant que « médecin soutien » auprès des familles, adopter une neutralité bienveillante, devenir systématique, et surtout, ne pas rester seul face à des incertitudes. Conclusion : le médecin généraliste devrait être un acteur central de la protection de l'enfance. Afin de tendre vers cela, il nous semble primordial d'évoluer vers un nouveau paradigme, à savoir : reconsidérer le rôle du médecin généraliste dans le soutien à la parentalité, et repenser la consultation pédiatrique en allant vers un dépistage systématique des violences intrafamiliales et scolaires.

Les données sur le parcours de soin des victimes de maltraitance infantile sont pauvres. Nous avons voulu connaitre les expériences et les attentes d'anciennes victimes de maltraitance infantile sur la place du médecin généraliste dans leur parcours de vie, découvrir leur histoire pour comprendre les représentations et jugements sur les acteurs dans la lutte contre la maltraitance et faire émerger des suggestions pour l'amélioration de la prévention. METHODE : Enquête qualitative par entretiens individuels semi-dirigés auprès d'anciennes victimes de violences infantiles dont les modes de recrutement ont été variés : forums sur internet, service de psychiatrie, patientèle de médecins généralistes, auteurs de livres autobiographiques et centres sociaux. RESULTATS : Douze entretiens ont été réalisés d'avril à septembre 2015. Le dépistage et le signalement des situations de maltraitance par le médecin généraliste sont ressentis comme primordiaux mais nettement insuffisants pour ces anciennes victimes de violence. Mais les parades pour tromper les médecins sont nombreuses : nomadisme médical, évitement des médecins, fausse image de la bonne famille. Difficultés pour l'enfant de parler quand il consulte avec le parent maltraitant. Malgré ce défaut de signalement, une fois la situation de maltraitance certifiée, lorsque le médecin généraliste est convié à la prise en charge de ces patients, il affirme son rôle de pilier pour l'accompagnement de ces enfants maltraités ; leur assure une écoute et un soutien bienveillant tout au long de leur parcours de vie. Malheureusement, le médecin généraliste est souvent écarté du suivi de ces enfants, les enfants placés n'en auront pas l'accès et les enfants signalés n'auront pas de suivi médical exigé. CONCLUSION : Le médecin généraliste a une place déterminante dans la prise en charge d'un enfant maltraité pour lui assurer un suivi et accompagnement de qualité afin de construire une résilience durable. Il serait intéressant de cerner les difficultés des services de protection de l'enfance pour désigner un médecin référent pour le suivi de ces enfants.

Le médecin généraliste a un rôle central à tenir afin de repérer précocement les situations à risque. 58 dossiers du tribunal de grande instance de Toulouse, portant sur des cas d'inceste sur mineurs, ont été analysés. Les filles sont plus souvent agressées sexuellement que les garçons (respectivement 69% et 31%). Les enfants les plus exposés sont âgés de 7 à 12 ans. Les agresseurs sont dans 95% des cas de sexe masculin et sont en majorité le beau-père ou le père. Un quart des mères des victimes ont elles-mêmes subi cette violence dans leur enfance. Près d'une sur deux a eu son premier enfant à 20 ans ou moins. La violence conjugale et la violence physique sur l'enfant se constatent dans respectivement 47% et 43% des situations. L'alcoolisme est retrouvé chez 41% des agresseurs majeurs. Les familles recomposées avec beaucoup d'enfants sont plus touchées. Dans l'ensemble des familles, les aînés et les seconds sont plus souvent les victimes. 43% des familles ont été repérées en danger préalablement aux abus sexuels. Le médecin généraliste doit s'informer et se former à rechercher et repérer ces situations à risque en se basant sur des recommandations validées.
